Straight to IV abx, such is the American way, whereas the evidence says many uncomplicated diverticulitis cases will resolve with trial of bowel rest and adequate oral fluids and if not then oral abx trial before IV +/- hospitalisation.

This approach is everywhere. Water cannot solve that problem. 100% of this pathology depends on the small intestine. The problem needs to be looked for above. Almost always, these are internal hernias (from personal experience). Impaired food transit leads to premature formation of feces in the colon, which creates these diverticula.
