RECOVERY AFTER PROSTATE SURGERY: What to Expect after a TURP for Enlarged Prostate

  • I put this video together to share with my patients what to expect in terms of recovery following their transurethral resection of prostate procedure for BPH or enlarged prostate. The exact POST OP TURP instructions that I give here may be very different from what your specific surgeon will do and how he manages the recovery after TURP surgery. However, I wanted to share just so that guys might have some picture of what the recovery period will look like.
    Here are the highlights:
    You will be sent home from the hospital with a catheter. Clean around the tip of the penis daily with a warm washcloth and soap and drain the bag when full as your nurse has instructed you.
    It is normal to see blood in the urine for a few weeks after a TURP surgery. If the urine looks like thick red (ketchup) rather than clear red or pink (like cranberry juice) then please let me know
    After a TURP surgery it is common to have some burning with urination, increased frequency of needing to urinate, and feeling that you have to get to the bathroom urgently. This is usually just from the irritation of surgery and may last a week or two.
    DIET: No special diet is required. Drink more liquids to keep the urine dilute and flushing the bladder to prevent clots. Adding fiber or over the counter laxative like MIRALAX or metamucil can be helpful to keep from getting constipated.
    ACTIVITY: No lifting above 10 pounds, strenuous physical activity (greater than gentle level walking) or sexual activity for two weeks, or until urine is clear yellow for several days in a row. The more straining that you do, the longer the bleeding and the recovery will take. Showering and bathing are ok, even with the catheter in place
    MEDICATIONS: You can resume your previous medications. If you were on prostate medications, you can stop taking them (tamsulosin, flomax, finasteride, avodart, rapaflo, terazosin, doxazosin).
    BLOOD THINNERS: If you are normally on blood thinners (warfarin, coumadin, plavix pradaxa) continue to hold these and do not take them until your followup visit when Dr. Chan will tell you when to restart them. Aspirin is ok. When you restart these medications, it is common to have blood in the urine
    PRESCRIPTIONS: You will have been called in an antibiotic and pain medication to your local pharmacy. Take the antibiotic until it is gone and use the pain medication only as needed
    FOLLOWUP VISIT: We will likely have scheduled you for a postoperative visit already when we set up your surgery. If you don't have a visit scheduled, please schedule one for 4 days after your surgery.
    PROBLEMS: Call to report a fever above 100.4, feeling that you cannot urinate, heavy bleeding, chest pain, or shortness of breath.

    I had TURP done here in Abilene Texas on 30 Dec2021. They keep you in hospital for 3 days afterwards thank God. I couldn’t imagine going home the next day, I was wiped out. It’s been 10 days and I’m peeing like a champ now. May God continue to bless smart doctors and caring nurses. 🙏

    I had this surgery 5 days ago. I was in the hospital for about 24 hours. I was not sent home with a catheter. All of the irrigation was done on the hospital by the nurses. I saw none of it except changing bags and pouring out the old liquid. I have spotty blood in my urine since being released. I have walked about 3 miles a day in the past four days. I have used a stool softener and psyllium husks. I have eaten mostly meat and vegetables since my release. I have driven my car every day. I had no pain that required medication except in the recovery room. The doctor, nurses and staff at Rose Medical Center in Denver were amazing! I am 62 and in very good health. Every body is different!

    I had TURP done on 10/02/2023 the best thing I have done. My other doctors were just giving me pills for years. They just about destroyed my bladder. I was having to use catheters sometimes to pee. Just find a good surgeon who knows what they are doing. That is what I did was to find a good doctor, who told me what I needed. I pee so good now, I wonder why they made me suffer for so long. Pills do not work they did nothing for me.

      Okay. Great buddy. But how do you know what is the right procedure for you??? And how do you find a good doctor???? And how do you know if he or she is good????? The websites that rate doctors are crap! They pay into them! It’s like carfax. The dealers pay into it for a good rating!!!

      Man, I'm so glad to find your comment. I'm about to get the HoLep procedure. I also wonder why it took so long, when there are alternatives and remedies. My conclusion, doctors don't want to lose the patient, especially if he/she is Not a surgeon; not all urologists are surgeons. I hope you are doing well and enjoying your new life 😊

    Thanks Dr. for your informations , as same thing I was told on my discharge the next day of my TURP. I am feeling so great , now it is fifth day no blood but the urine is slightly more yellow that I can see there is very very little blood content. No urgency, not much burning sensation only slightly at end of urination , powerful flow, emptying bladder smoothly. Happy to watch your video uplifting me psychologically . Thanks again.?

    Very helpful, thank you. I had TURP 5 days ago, it's Saturday so clinic is closed until Monday so I hope they will get the catheter out as soon as possible as it's very uncomfortable. I'm told the surgery went well and I went home that night maybe because of the pandemic and the hospital is very busy. I was given an antibiotic before the TURP but nothing after which concerns me since I've been given post-op antibiotics after other procedures. But, as you've said, I'm taking great care about being clean and avoiding infection.

    This was most informative. Most You Tube videos focus on incontinence and sexual functions. This video discusses those and also the healing process with timelines for post-op recovery. Having just gone through a prostatectomy it was helpful to get those recovery details.

      What about food eating how long before I start eating food post operative. Its my 5th day today and I am still npo
      I think the dr accidently hit the urethra and it was steched. I'm not clear if nothing by mouth status would be needed for more than 6 days and why?

    I feel fine after my repeat TURP. I had one 4 years ago at the same VA here in Chicago. The only thing I notice is my HGB 11.1 and HCT 33.4 is lower. I hope it goes back up to normal.

    I wish you were closer. I'm not looking forward to having a TURP but need one. Mine was scheduled then called off due to disagreements between my cardiologist and urologist over how long to be off aspirin. I now have a green light but we just lost all of our hospital urology team to a competing hospital. I am enjoying your videos. Just found them today.

      thats something i didn't want to go through. going through what the cardiologist will require to clear for surgery. I decided to live with the foley catheter. i change it myself urologist wanted me to do intermintent catherization cathing myself a hundred and fifty times a month was not appealing to me. i change my foley once a month alot less aggravation

    I had my TURP op last Friday (I'm 63), operation was at 12:00, I stayed overnight, catheter (a two way one, flushing in saline and it was pretty clear) was removed 08:00 (was then urinating a fair amount of blood, and painful, very), went home at 14:00, very tired.....
    Passing a fair bit of blood for first 2 days, pain slowly subsided (using paracetamol, it takes the edge off it). This has now largely stopped, just small amounts of pain and blood now. It is now Tuesday, and starting to feel normal'ish.
    Just continuing to rest and drinking plenty of water and using stool softener, no longer need pain relief, pain is minor.
    Fingers crossed that I avoid blockages or infection........ ;-)

    Dr Chan, excellent video you put my mind at ease. Question is it normal, and I'm assuming it is to have bright red blood right after the procedure. I am drinking a ton of water staying up at night just to drink water, and to empty bag. The drain on the bag so far clogged up twice. I put half my body in shower used hand held shower head with water hot to flush out blood clot out of the drain portion of bag. Seemed to work is that ok, and I'm also getting a little dripping red blood out of penis where the catheter goes in. Hope you could answer any of that. I will be getting in touch with my urologist tomorrow. Thanks for your videos

I have a question,I underwent TURP surgery 4 was before,now urine flow is normal but still I feel burning sensation during and after urination.I took Bactrim ds antibiotics for 15 days.pl.give your suggestion to eliminate the burning sensation at the end point.Thanks and sorry for the inconvenience.I am 73 year old,BP normal,no diabetics and no other health issues

    Hi Dr. Everyone, did Turp 5 weeks ago and I gotta say my experience is quit dull. I am 40 and have been having BPH for 7 years with treatment on and off until I had enough of it. Dr from Emory advised that it was time to get Turp done. After removing the catheter on day 3, I experienced difficulty passing urine and that was resolved with medication several painful hours of trial. 5 weeks in and the pain is getting worse. I can describe the pain as a burning and tightness around the

      just wanted to follow up with your situation nearly one year later... I hope you are doing good now. I got TURP done 2 weeks ago, and so far, I am having a hard time voiding. Several micro mictions, several times a day. Hope things will improve soon. Juste wanted to have a cheering word from someone who went through this surgery... succesfully.

      @@richardbrousseau4799 thank you for the message. I am hoping you're progressing. It takes some time. Voiding was an issue for me the first 2 months so keep your head up. I am well almost a year later. My major problems are almost gone. I can sleep well only waking up once a night (I guess normal people do this?), however, I am learning to control drips after peeing. Little drips through the day can be annoying but Kegel exercises are helping. Hoping you're well. Again thank you. Will check on you in a few

      I am not sure about size but I needed surgery on the prostrate and part of the bladder. I am unable to hold a lot of liquids but drinking liquids in moderation helps a lot. I am able to get a good night sleep. If you happen to need surgery, please go for it. The longer you delay the more problems you encounter. Claims that TURP will affect you sexual function is BS.@@IAmTheEggMan111

    Doc. I had TURP, but didn't work, I'm still going to the bathroom at night 7-10 times, still dribbling and can't empty my bladder. Initially I did REZUM, it didn't work so I came back and my Urologist said there's scarring in my prostate so he suggested TURP. I wonder if the scarring causing again my frequent urination and inability to empty my bladder. Any thoughts what could resolve my problem?

    My robotic nerve sparing surgery was done 19 july2021. Cathetier is removed safely. some leakage is their and I walk daily 5 KM. Taking viagera,as prescribed, but have no eraction till now. I wonder if it will come back and when or.........

    I can’t hardly believe what I’m reading in all these comments.
    My husband had his TURP surgery on May 24th 2022 his Prostate was larger then a big size grapefruit and still growing in size , he had a Foley catheter for 5 months until the surgery date because he could not urinate on his own and was in severe pain. Long story short he had lots of complications and problems with his prostate. I was shocked that he was sent home the same day of surgery and was allowed to return to work in a couple of days but light duty for 2 weeks ( he is a FedEx driver/ delivery) he was good for 2 weeks with clear urines after the blood cleared and then very little pain but on week 3 he started with bad pains and cramping in the pelvic area and pressure in the penis unable to urinate again and when he pushed he passed a few blood clots.
    He ended up in the ER and admitted for four days the nurses did bladder irrigation to clear the bladder of clots but they kept coming and getting larger in size.
    Today he just had surgery the hospital doctor found that he was bleeding internally from the TURP surgery, she found open blood vessels and she cauterized them and closed them up .

    Minor TURP thursday morn. Prostate not reall large but was blocking bladder. I just took my catheter out today sunday afternoon. 20 minutes later had urge to pee & got out a little nice color hardly even pink. Am I ok do you think??

    i had Robotic simple prostatectomy surgery(for enlarged prostate) a month ago. After two weeks had catheter removed. But I still have problems with urination: You start to pee normally, when you finish it hurts terribly (climbing on wall). Is it normal or i need to see my doctor?

    My concern is the pain. My urologist told me after a Urolift failure, and now a Cystoscopy that, I should have a TURP. I had the Urolift, in 2019, then a year later my urologist used some green light laser because I have a median lobe. That said, the Urolift in 2019, and the Laser in 2020, failed and worse, at the time urination was horribly painful. It lasted apox 10 days, the first week being the worst. The pain on urination is unbearable. Now, given the decision to get a TURP, I'm just out of my mind with fear. What if anything will ease my mind of this excruciating pain? Intellectually, I know it has to be done but I'm scared. I can't be the only patient who feels this way. Do you have any suggestions?

    Hi Doctor, thanks for this great video. I just had a TURP procedure done almost 4 weeks ago. I finally stopped having bloody urine 4 days ago, and it is finally clear now. Also, the stream is fantastic, and almost too good. However, I'm still having issues with the prostate hurting and frequent urination, especially at night. It's almost impossible to hold it until I can get to the bathroom as it wants to come out with so much force, and the bladder starts spasimiming. Will this improve?

      You need to strengthen your pelvic floor muscles maybe , but ask your doctor if that is what is needed as it might make things worse. And please let us know here if your doctor recommends it here

    Hi Dr Chan, My husband got a radical prostectomy last May of this year. His Gleason score is 4+5 by the pathology after his surgery. When his urologist took out the foley catheter after a week, lots of blood came out once it was pulled, is this normal?, and also, what's my prognosis that one of the seminal vesicle was invaded with extra capsular extension but no lymph node invasion, total of 7 nodes and negative surgical margin, does this mean that the cancer has already spread?, thanks.

    Is it normal to use the TURP procedure to conduct a prostate biopsy when traditional biopsy methods cannot locate cancer, yet the PSA levels continue to rise?

    I am looking for persons, who experienced earlier the same as I for shortly (retention, TURP on 19 May 2022).
    And what can I await with the time going?
    I have the following questions to all, who went through TURP for BPH.
    Who had a retention on BPH-medication (finasterid and aglandin)?
    And after it received a TURP.
    And after TURP experienced a retention again, with bood cloths?
    How long was gone, and regained a normal urin?
    The Creatinin and BUN-values became better after TURP again?

    Have you seen any special problems with patients taking Xarelto for blood thinner? Does it require a longer time withholding blood thinner if patient is on Xarelto?

      I haven't seen any different problems from other blood thinners (coumadin, eliquis). I think a lot of the duration to withhold is guided by the cardiologist and how vital it is to restart it as soon as possible.

    Hi, I had a Thulium Laser surgery in June. The took 106 Gramm out, all was going very well. But still I have no control for my urine. They told me to have patience for the recovery of the muscle, but still I live with these pants now for 4 months. Do you have a good idea to get back the control of my urine? Thank you.

    I got TURP surgery last week .
    Much easier than I expected .
    Was out after 2 days and now in my 9th day.
    I am passing blood in urine but told this is normal.
    Also I am frequently still going to toilet but not as bad as before surgery.
    I was told to do bladder training by my surgeon.
    Just wondering is bladder training done by all that have this procedure?
    My quality of life certainly much improved and expect it to even get better.
    Would I do it again ?

    Excellent concise explanation. I am seven days after turp and have experienced most of the things you mention. The end of my penis is very sensitive if it rubs on underwear and is quite sore. I have had the catheter in for 8 days.

    5 weeks after my surgery I had blood clods caused by scar tissue which fell into my bladder causing me to have severe pain. I was unable to urinate as the urethra was plugged so my Surgeon had to take me to the operating room to remove all the loose tissue.

    I'm 21 and I'm not sure if I have an enlarged prostate but I been having little trouble urinating sometimes I need to push to pee and something I'll get the urge to urinate but nothing sometimes I need to hold it. So I'm not 100 % sure what I have but when I urinate it's just fine no hurting or bleeding. Do you know any testing that's done to check for an enlarged prostate?

    Here in NZ the surgeon called it a rebore yesterday after telling me that's what they want to do. With my mechanical engineering background, that sounds like a major procedure. Kind of is, because it is 2-3 days in hospital here.
    What bothers me (surprisingly) is they said I will not be able to spawn brats after this surgery. Two marriages did not result in any little sprogs running around, but I am not 100% it will never happen and my dad hassles me about making him extinct, i.e., not continuing his family tree. I am laid up in not good health at 51 after a L5/S1 discectomy, so it is less likely it will still happen, but it still bugs me.
    Here in NZ surgery under our public health system is graded and scheduled for priority according to urgency. Elective stuff like getting my nose fixed took several weeks to get done, while my hand being reconstructed was done over two surgeries the week after it was squashed in port, i.e., super urgent. They want to do this TURP in two weeks, i.e., quite high priority, because I have been getting sick form infection etc. I actually prefer longer to consider it all this time. GP is calling today to discuss. I have read the NZ public health stuff about TURP. I am not so keen and wonder if there is another way.
    I also want to know why a 5mm fragment of my L5/S1 disc was left in there after a non fused discectomy and is now cuddling up to my sciatic nerve squishing it. Nobody tells me jack. The pain clinic tells me not to worry about it, take Gabapentin and do my PT, which I do a lot, because PT in my 20s got my fingers working again after months of them not moving after the accident. They like to blame everything on neuropathy, but I am pretty sure that nerve getting squished is what hurts so much (they lecture us about phantom leg pain like we do not understand about ongoing pain becoming an issue --- then they get to trying to blame it on your childhood etc -- drives me up the wall).
    A few years now and it just gets worse, so I struggle to walk more than a few minutes, due to numbing in my legs etc. I now get disability concession for half price taxis and buses etc (I take 100-200mg tramadol SR per day, so should not drive). Government pays for my apartment, utilities, food and spending money etc. I am lucky I live somewhere they have your back if stuff goes wrong, but it still sucks not being able to do my job any more etc (deep sea fishing manager at sea down Antarctica way where the seas are furious and lots of people end up having surgery on stuff that wears out etc). Whoever invented ageing needs a good kick in the bollocks huh? Figuratively speaking of course.
    I decided to study a degree part time when I realised I would not be able to return to sea, i.e., study for a desk job or the like. Almost half way thru with high GPA. I have not been able to get back to study due to my health. Sometimes I tihnk if I won Powerball I could get it all fixed, get a dog, and spawn brats. But my GP says she knows people who mortgaged their homes to go the private spinal surgery route and still did not get better. I dunno. Something for a channel on spinal stuff I guess.

    Since the urethra passes through the prostate, don't you have to cut through the inside wall of the urethra to get to the prostate tissue? Does the urethra have to heal from these wounds?

    I am going on my third TURP surgery. My first one was pretty good no problems my second one after they took the catheter out I had blood clots and I’m doing another one this month February 28 I’m on blood thinners because I had a heart attack and I fecal incontinence and urinary incontinence. I’m concerned about getting blood clots in my catheter I don’t wanna be with a new catheter why am awake? What do you think doctor

    Thanks for posting this video Dr Chan. Right now I am on day 2 of 3 days before my catheter is remove. I've experienced lots of blood loose. My problem comes when I urinate most of the urine goes outside of the bag. My doctor put me on Oxybutnin. Wonder if you think this is s good solution.

    The dry ejaculation, due to urethan being swollen and possible nerve damage does this improve over time say six months healing and one is able to ejaculate normally?

    I had my TURP 3 weeks ago I don't have blood in urine, only a few drops. But when I sit or stay/walk more than 10 minutes it feels uncomfortable. My doctor told me to wait 6 weeks post operation. I also got antibiotics. Can you tell me that it's also normal too feel uncomfortable, it's not really pain, but I have to lay down and calm a few minutes. It's also burning when I urinate 3 weeks after TURP. Thank you Doctor for your help. I'm relatively young for this, I'm 41 and live in Austria. Sorry if my English is not the best.

    Thank you, Doctor for this information. I’m 68 and I had the procedure done three weeks ago. I have very light blood in my urine still but my flow is so much better. I haven’t had sexual relations yet but I’m finding that I do get erections. That alone surprised me…lol

      hi eileen, there's a lot of factors that you'd have to consider before coming up with a decision. I'd check with a urologist in person to evaluate all those things.

      great questions. the decision to the remove the catheter is one dependent on your surgery. as for eating and drinking, in general, it's fine to eat and drink after surgery.

    Hello and thank you.
    I had the procedure 8 days ago. The catheter was removed the next day. I ended up in the emergency room the following day. I could not pee. Catheter was put back in for four more days then removed. I have had the catheter out for two days now. I am peeing frequently, at night I get up at least 5 times to go to the bathroom. I feel slight pressure constantly and it results in anxiety build up. Are there any tricks like an ice pack or heating pad to help ? I used Xanax to sleep the dr did not like that.
    So I get stressed out ....how do suggest I deal with the stress and the pressure?

      You're welcome. thanks for watching. I'd obviously check in with your surgeon first with his specific recommendations. As for the stress , I'd recommend using the headspace meditation apps. I use it and find that it is very helpful with stress.

    Does a longer catheter duration increase the risk of strictures? I'm getting TURP in 2 weeks, my doc said the catheter will be removed before leaving the hospital, 1 night stay in the hospital. Do you recommend Phenazopyridine for a couple days post TURP to reduce the sting?

      hi mike, as far as i know, i don't think there's a correlation between durationof catheter and stricture formation later on. I'd check with your urologist to see what he thinks about the phenazopyridine.

      Hi Dr. Chan, I've included a link to a stricture report. The Abstract paragraph lists "catheter time" as one of the "Suggested etiological factors of urethral stricture formation".     Hello...I had undergone prostrate surgery 10 days ago.Catheter removed after two days of surgery.I could pee normaly without any issue.But there is burning sensation before and after urination.Can I take DISODIUM HYDROGEN CITRATE SOLUTION to reduce burning sensation...?Shall I continue my early medicine TAMSULOSIN HYDROCHLORIDE (PR)and DUTASTERIDE capsule...?

      usually for my patients, I really dont want them straining when having a bowel movement, so i highly encourage drinking plenty of fluids, fiber or miralax as needed.

    I had a TURP 31/01/24.
    Still peeing blood still burning as I pee. Still feel like I’m sitting on a rock when I sit on a hard chair.
    Peeing very well but will be so glad when burning stops

      no, it's not contraindicated. sometimes, we perform a TURP on patients with prostate cancer with voiding difficulty prior to radiation therapy. in general, doing a TURP after undergoing radiation therapy is a lot riskier as the chance of causing urinary incontinence is much higher.

      it's very specific to your surgery, like if it was nerve sparing or not and how well the nerve sparing was done. i'd check with your surgeon as he or she knows the details so that they can give you their thoughts.

    Had turp March 20, 2024 it is now May10, 7 weeks later. I am going thru some of the worst urges to pee that you can imagine. Frequency is the same as before the surgery. For me it does not seem to help at all, probably worse than before surgery. I have to see the surgeon on may 17. I can only hope that he has some answers to these problems.

    I am a 61year old Male,I had a Procedure done called a Cystoscopy+Thulep . after the first catheter was taken out after 7days i had urine retention by the end of the day and a second catheter was put in .two days later i got a fever as i had picked up an infection. Took Megnex to cure the infection removed the second catheter after 5 days started flotral tab 10mg .started urinating but as days passed the flow became less. now 20 days later when i urinate it starts with a spray then becomes a weak stream then drips then after a while again another short stream and its starts dripping. Do i need to be worried.

      I had TURP done 6 weeks ago, had UTI two weeks after the surgery. The doctor changed the antibiotics but my urine flow was very weak and I had to struggle to pee. After 10 days urethral dilatation was done, which has helped a lot. Please explore the possibility of urethral dilatation. Best of luck to you.

      Yes i started using a plastic catheter number14 and self dilated twice a day for two weeks then once a day for a month. Then every alternate day so after a month i will self dilate once a week then the doctor says that i will be ok

      @@bruce3860 What a coincidence, I was advised self catheterization yesterday and did my 1st dilatation today. The catheter didn't pass all the way to the bladder but I passed the urine. Could you please share your experience?? I am 64 years old and live in Darjeeling, India.

      @@dipaksharma8952 one complication after an operation is a stricture so the stricture has to be dilated first twice a day for at least two to three weeks then once a day. Do it gently after applying a jel onthe catheter and insert it with a slow rotating action once it has passed the stricture urine will cone out dont let the urin come out slowly pull it out and immediately urinate to flush out any chance of getting an infection. I find that you should do it when you have an urge to urinate

      Obviously talk to your doctor, believe in this video and elsewhere it is said that you can discontinue Flomax after surgery. Suggest watching video again.
