    Accutane or isotretinoin is a vitamin A analogue . It doesn't work on hormones. It causes differentiation of cells in the pilosebaceous unit ( hair follicle and oil forming gland unit ), reduces oil secretion as well as prevents blockage of follicular opening.
    It doesn't put a "brake on acne" . It actually corrects the underlying pathology of acne which are increased oil secretion, hypercornification ( cells of hair follicle lining becoming thicker ) and inflammation. The fourth pathology being overgrowth of p acnes bacteria for which certain antibiotic course may be given for a short period of time. Isotretinoin is a very effective medicine and can be life altering medication for many struggling with severe acne.
    Yes, stress and hormonal imbalance do exacerbate acne. That must be addressed to correct acne .
    There are no "toxins" in the body which cause acne.

      Just here to acknowledge the amount of restraint you must have mustered in replying to this level of ignorance around the acne pathology seen in this video. I also sympathise with and apologise to cancer researchers for putting in a lifetime of work. At the same time, a person in a podcast can say 'everything is caused by hormonal imbalance including A CANCER'. Wow, man! The audacity of claims made here is bonkers and the ignorance on display is encyclopedic.
      I have nothing against these two gentlemen, also I know my writing here probably means nothing but, I guess, I just wish that just like you authoritative figures from most fields call out or at least bring to light reductionist dumbed-down ideas in most videos that are bereft of critical thinking and victim of framing bias( everything must fit in their predefined world-view - e.g hormones, toxins ) in this channel. So, thank you.
      Wish makers of this podcast realise that smartness comes through reasoning and thinking through things, not some narrow reductionism that gets relied on minus the nuance( for example hormones in this video). Listeners deserve better.

      And just to add to your point, when Ranveer says most of the people he knows have been on Accutane forever, it implies that isotretinoin is safe. Far from it. Accutane is teratogenic, meaning it can cause defects in your unborn children, and needs to be stopped 3-6 months prior to conception. It has other long term adverse effects that need to be monitored by a certified dermatologist.

      Been on Accutane, isotretinoin,azelaic acid, benzoyl peroxide,clindamycin nothing works until you get educated. Acne/skincare in INDIA is a football which people choose to kick in their own preferred direction and call it the right way :)

      I had take this medicine for 10 months regularly, my hormonal system get imbalance, I gain almost 20 kgs, my bone get weaken. I m geneticist and I know all doctor say this is very good medicine for acne treatment but for me I did worse. I had taken this medicine on the prescription of skin specialist. Even though I know that this medicine is a form of teratogen.
      I always told everybody to put on its consumption

    Those who have sever acne,they can take zinc and omega 3 suppliments with a healthy diet(no sugar,no fried foods,no dairy products).exercise regularly.maintain a skin care regime.

    I've taken it for 6 months.. I left it n acnes were back but it exactly same time I figured how to minimise it its all about cleaning your blood by putting yourself off of food n maintaining 8 hours gap between meals.. dont stuff yourself with packaged food go natural..n try intermittent fasting, make it a life rule

    I had also suffered from acne so i went to dermotoligst then he said you have take care of diet and reduce your suger intake nd provided me facewash, moisturizer along with zinc tablet, clindamycin and niacinimide gel which cured my acne. So diet is one the main thing but you need to also takecare of your hygiene from cleaning your face with diff towel separately to changing your pillow covers timely. I hope it helps ppl and those ppl who are suffering from these, may your acne cure forever cuz I've been through the same! 😊

    For all the people who think that your acne will forever be with you try changing your diet for 1 month eat vegetables and fruits, reduce cooked foods + add at least 3 tender coconuts per day. I'm speaking from experience, I had cystic acne plus nodular acne and this helped me alot even if you you include tender coconut in your diet and eat less the pain decrease.

    To all the healthcare people entering the discussion pointing out he messed up the mechanism of action of Accutane and its not a hormonal medication- You're right. Still doesn't change his point. I had cystic acne for years. My derm wanted to put me on Accutane. Once I heard all the side effects I said no thanks. I did a deep dive into my diet and physical activity and cleared it up on my own. P acnes live on our skin regardless of age. That still doesn't explain why teenagers break out nor why adults do. Neither does it explain why some may have surface level acne whilst others deep painful cystic acne. The P acnes are a component of the equation but they are not the cause. The catalyst is something else entirely and in many cases it may be many catalysts. Conventional medicine has answers but it doesn't have all the answers. The one big downfall of conventional medicine is that it shames people for their anecdotal evidence then years later come out and say that anecdote was right because xyz trial proved it. Before saying diet, hormones, exercise, etc are *not* the cause of cystic acne perhaps take a step back and remind yourself that conventional medicine does not know everything about everything yet and that's why evidence based practice is key and why there is a continual effort to update said knowledge and why practice guidelines do change and often. Adapalene was the *it* drug in the mid 2000s. I remember having to see a derm and pay upwards of $50 yet imagine my surprise when I was walking around the grocery store and saw it sitting on the shelf over the counter no derm prescription needed for $10. Big pharma plays a huge role in medicine and to say otherwise is disingenuous. If we were told the root cause of our diseases and conditions there would be no pharmacy companies making trillions of dollars. After all medicine today is about treating symptoms and covering up the root cause.

    Dealt with acne for 10 years almost.....took accutane & isotretinoin both & a tons of other meds...didn't work. Switched to Satvic diet, intermittent fasting & Patanjali meds prescribed by an Ayurvedic doctor & wow....it worked. Your food is your medicine guys.

      @@sanjanakhadanga I consulted a Patanjali Ayurvedic doctor & she prescribed me Patanjali meds. On top of that 16-8 intermittent fasting. Loads of water rich foods as Luke mentioned, absolutely no sugar, 5 days yoga, good sleep. Most important thing that worked on me is the Patanjali meds & clean food.

    Wrong - mechanism of action of accutane / isotretinoin. It doesn’t work on hormones. It helps in addressing various aspects of acne - like regularises skin cell turnover ( so reduces pore clogging), reduces sebum ( prevents build up ) , reduces inflammation.
    Right - role of diet & lifestyle in acne which is often underestimated. It’s very important to look at the root cause and treat accordingly. Skin & hair are often the external manifestations of internal health.
    So diet modification is plays an important role .

    Honestly speaking I had been suffering from acne for 4 years...then I finally went to a doctor after doing everything at home..then I,m on isotretinoin medicine its my 3rd month ...no acne or pimple on my face even though I used to get painful cystic acne...and the best thing u can do with while or after medication..plz follow good skincare routine and good products according to our skin type. All the best 💛
    Use products of Salicylic acid, Benzoyl peroxide, niacinamide
    Must use a good sunscreen.

    I am currently on plant protein + dairy free(I take paneer and curd which is completely safe and dont cause acne) + oil free diet + gymming daily. Did that for 2 months still got various small acnes + few bigger ones. I went to dermat, she described clindamycin cream and one antibacterial pill, been following from 1 month with same diet as above. But only got 20% relaxation, still pimples keep coming. Now only thing left for me is isotretinoin and will start after my next visit

    I used to get lot of acnes. Stopped eating sugary foods, diary products, fried food at all for 1 month and it wasn't still stopping. Then I stopped my whey protein intake too. From that point of time, I have got absolutely no acne. So, you might need to explore the things that you are eating day by day.

    4 years ago i consulted from a dermatologist about my severe acne problem and she prescribed me some pills for 20 days and that controlled my acne problem upto 60% and 3 months ago i started maintaining low carb diet, drinking alot of water and running 5kms daily in the morning, now my acne problem is solved 90% so remaining 10% is due to cheat meals or over eating sometimes. So consulting a doctor is important and then changing you routine will work perfectly.

      Scars will reduce with time for me running until face start sweating even in winters and bathing in cold shower reduce scars fastly and i use acno fight face wash 4 times a day like once after running then while bathing then after comming from college and then before sleeping. I don't put any cream on my face because creams block pores and causes acne, you can apply fresh aloe and lime 2 hours before going to bed and clean it before sleeping don't keep it for whole night, just let your skin breathe keep it as much fresh and natural you can it will heal itself.

    Hello TRS, being a person who has dealt with acne during his whole college life and even at times now (above 30). The information passed on by the guy in this podcast is misleading. I am assuming he is not an actual dermatologist. When you call someone to discuss something like this, please invite someone who is actually an expert in the field.
    Isotretinoin and Retinol are like vitamin A, which curbs the oil secretion. And before taking these tablets, a person has to go through the Lipid Profile test to check the cholesterol level in the body as this would also affect the insulin secretion.
    Sincere request to invite only genuine and actual experts and not someone who has millions subscriber on youtube or intsa, just to gain likes for both of you.

    So let me share my experience on the use of Isotretinoin. I had a big pus on my left cheek and rushed to the doc to get it checked. And upon getting checked, the doc suggested isotretinoin and a Clindamycin gel. I was told to take isotretinoin for 6 months but I stopped after like 1 and half months as my face my cleared. But now (after like 5-6 months of not using isotretinoin) recently I've started seeing a some acne again. So yea....these meds don't completely eradicate your acne. It's probably my diet as I've been hitting the gym and stuff so yea...

      U acne will comeback because u didn't complete the course of isotretinoin its course for 6 to 9 months so that's reason u acne will come again another thing that u said its all about its correct that a healthy diet matters a lot and if u on isotretinoin then u should take the isotretinoin with good meal means a meal with much fatty acid that u required according to ur dose its work it really work but after stopping isotretinoin u have to care ur skin because isotretinoin do the heavy lifting 😅

    In my case isotretinoin worked my acne cleared 90% in 3 months now i stopped taking isotretinoin and i am not getting new breakout. I am happy now

      You can be happy now... U have all the right... But be alert... Acne will return with a bang...it's a vicious cycle dear...M speaking from my experience.. I have been on isotretinoin 4 times in the past 10 years.. N I'm still haunted by cystic acne

    Isotretinoin does not work on hormones. It works by altering cell differentiation and cell cycles. Through this mechanism, it reduces sebum production, involved in formation of acne. It does not directly work on the bacteria responsible or hormones.

    I had mild acne but recurring. My dermatologist advised me to go on accutane/isotretinoin. I used accutane/isotretinoin for 5 months. My skin got worst due to purging then started to clear but left many marks/spots. I faced side effects too, for me it was hairfall and dry eyes. A year later I am having acne again. I would say don't go on accutane unless you have cystic/hormonal acne which is really bad as this medicine comes with a lot of side effects. Try to fix it naturally by trying different things e.g. diet, exercise, cutting sugar/sweets, cutting dairy, cutting carbs, increasing water intake, reducing stress,etc. For me controlling sugar intake and reducing stress works great. Different things work for different people. Don't let acne make you feel insecure. We all are beautiful in different ways.

    I adamantly argue against diet being the root cause of acne. Accutane is clearing up my adult acne when nothing else worked. I totally changed my diet and exercise routine, to no avail. I still stick with the ultra healthy lifestyle however diet was never the source of my acne. My sister took Accutane in 2015 for cystic acne and is still clear now, all these years later

      @Blackpepper I believe most people take it for 4-5 months. My treatment plan is 4 months right now. I started clearing up on day 8! It does dry out your skin pretty bad, just be proactive about it!
