How to Get Rid of a Double Chin | Dr Davin Lim

  • The 4 common procedures for double chins. Everyone's case will be different and might require additional or different management, but here are my thoughts on the usual treatments.

    Double Chin Treatments or submental fat is due to genetics, weight gain & age. Though the problem is easy to visualise, the optimal treatment may be complex. My algorithm encompasses non-surgical procedures including fat dissolving injections, dermal fillers, HIFU energy devices, RF skin tightening, Coolsculpting as well as surgical techniques such as liposuction.

    CoolSculpting is a good method for reducing unwanted fat under the chin. This procedure has less downtime & pain in comparison to fat dissolving injections. CoolSculpting can also improve skin laxity, further enhancing the contours of your chin & jawline area.

    HIFU or High Intensity Focused Ultrasound is also known as Microfocused Ultrasound. The two stand out devices are Ultherapy & Ultraformer III. I use the former for lower face lifting, whilst the later can be used for double chin reduction. It works by delivering energy below the surface of the skin, into the fatty tissue damaging the fat (off label).

    There are many ways to address submental fullness, from Belkyra/Kybella fat dissolving injections, Coolsculpting Mini, heating devices like Ultraformer III, off label microneedling RF, Vaser liposuction, RF tightening devices like Facetite as well as surgical options including liposuction.
    Here are the facts: the most predictable, cost effective & probably the best solution is with liposuction. Here is why-
    Predictability: Liposuction relies on the surgeon physically removing excess fat & fat cells. Non-surgical treatments including Ultraformer III, fat dissolving injections & cryolipolysis (fat freezing) rely on your immune system to destroy & carry fat away. This is unpredictable.
    Downtime. Recovery from liposuction is really quick. A neck support brace is worn for 24 hours post procedure, then only at night for one week. Swelling goes down within 48 hours after liposuction. Conducted on a Friday or Saturday am, you can go back to work on a Monday. For fat dissolving injections, expect swelling to last one week. Multiply this by 2 to 3 and you have up to 3 weeks downtime.
    Adjunctive skin tightening: Remembering the two most important causes of double chins are skin laxity & accumulation of adipose tissue/fat. During liposuction I also employ techniques to tighten skin. The results? More predictable results & jawline definition. Most of my cases I combine laser, HIFU & or RF devices to produce skin tightening.
    Cost effective. By the time you add up the cost of fat dissolving injections & cycles of Coolsculpting, liposuction is a cost effective alternative.
    Of course you may not be suitable for surgery, or have a personal wish not to have liposuction. In this case my advice is to consider cryolipolysis with Coolsculpting Mini. This treatment takes 40 to 80 minutes, depending on the location of fat & extent of treatment. I do believe that cold therapy is more effective than heat therapy. We currently run 2 fat freezing devices including Coolsculpting & Cooltech.

    Other options include fat dissolving injections (I hardly use this, purely based upon the downtime to the patient), as well as heating - microfocused ultrasound with Ultraformer III. The latter I use more often as a tightening device, at the same time as liposuction.

    I think the model Linda Evangelista had a negative side effect from coolsculpting whereby it has permanently disfigured her. Could you discuss this issue, Dr. Davin?

    I’ve had submental lipo with facelift. But beware. After the fat is reduced, it can also leave skin laxity. RF Microneedling under my chin has helped the laxity some. Also ask your surgeon b/f doing anything if you have submandibular low hanging glands b/c these will prevent the result of a “chiseled” under chin result!

    I tried Kybella back in 2013 and OMG the SWELLING!!!! It was HORRENDOUS! The injections didn't hurt much, but my skin was so itchy as it was so stretched. My gums also ached, which was kind of weird. After a month, there was no difference. Ugh. (though I only had one session)
    I had lipo in 2015 and it was EXCELLENT. Unfortunately I've put on a bit of weight since then, I'm 44 years old and now some fat has reappeared there.
    I've had 3 sessions of HIFU and I have to say that there was only a VERY subtle difference that appeared after about 2 months. After that, it seemed to fade.
    I am now going to try Coolsculpting! Fingers crossed!
    PS. I would just have lipo again, but I can't afford it.

    Thank you for posting this. I'm seeing a doctor next week actually and this had educated me. I think lipo is the way to go.

    Thank you for going through all the different types of treatments feel very self conscious of my double chin even loosing weight hasn't helped going to save up and go for the liposuction one day .

    Do not waste your money on cool sculpting. I did it 6 times. Double chin is still there. I had lipo, good result. But I might need hifu for skin tightening.

    Hello doc hoping you could answer my question. Im just wondering Why my acne scar (quite wide, deep rolling scars) on my left and right cheek (front lower) keeps coming back even after my 2nd session. The procedures done were (air subcision and ha fillers). I have quite chubby cheeks btw. Hope you could answer my question doc. Thanks a lot

    I get a kybella derivative done & yeah it May swell slightly right after but it went down by next day. It does sting but nothing a couple of neurofen can’t handle. It works well but yeah you probably need multiple treatments

      Some people have double chin that is hereditary...even when slim.

      I eat a balanced diet and exercise regularly. I hover between slightly underweight and slightly healthy BMI for my height and weight. I've always had a slight double chin due to genetics that exercise and diet won't eliminate.

      Because some people have distoclusion (check it up - it's when chin is like pushed back). Fixing it is a very difficult surgery with 6 months recovery period. And this causes double chin. Even if you lose all extra weight in your body, it will be there because techniqally your jaw was not meant to be positioned that way. So no, exercises won't help 😅😅😅
