The black one on the steaming nozzle is a nozzle cover. You have to remove when using.

Actually if you use the black one it froths better which is ideal for cappucino but if u remove it then its gonna froth less and make the milk velvety which is ideal for latte arts

No the cup does not come with it. I got it from noon. it is very easily available online.

it has a reusable steel filter in it. I wash it and use it again. I am not sure about size but it makes two cups at a time.
