  • Most prostate cancer surgery patients get worried about incontinence after their surgery. Well, here is my experience, which happened to be a great one. Dr David Samadi's prostate removal technique saved me from cancer, but also kept me 100% continent. I don't even leak. Here is my story.

    I’m on day 6 recovery after Prostate surgery. Have had some pretty low times during this period. Thank you mark for all your videos. It really helps seeing someone who has gone through the same experience and can pass on useful advice on what to expect and things to look out for. Also really great hearing from someone who is so positive.

      Proper kegal exercises vitamin b1 and 4 table spoons nutritional yeast. Extremely important for 3 months.

    Great vid, thx. I see this is now 7 yrs old… I hope you and all the guys who’ve commented have recovered well from their prostate surgeries. I had RALP surgery 2 yrs ago. Fortunate to have had good bladder control; the Kegel exercise definitely helped. A few unexpected dribbles early on, but that stopped and never had to wear pads or a diaper. A year ago I underwent 35 radiation treatments because my PSA, although very low, was still registering and increased a bit. Two PSA checks since then have been “undetectable” and that’s been good news. If you’ve just had the surgery, hang in there! It takes some time, but your body does recover as best it can.

    Some things to point out. First, the time for regaining continence is basically an individual thing. This operation (robotic procedure) is pretty much standardized at this point as it is now going on 20 years. So any surgeon licensed and qualified to do the surgery will generally get the same results. Therefore don't fret if you are not completely continent right after surgery thinking that your surgeon screwed up. 95% of all men regain complete continence within 12 months. Some are completely continent immediately after removing the catheter and others take weeks or months.
    Secondly, even if you gain continence in a short time wear the pads for at least 6 months after you gain continence. This is because you can go two or three months without having any leakage and all of a sudden one incident you get leakage. After you've gone about a year without leakage you're pretty much completely continent. Also the pads are not like diapers. It's not like you'll be wearing Pampers. The pads themselves are the size of an athletic cup. They cover only the penis area and that's it, so they're not that intrusive at all. And as you heal it's comforting knowing that if you do have a leakage accident you don't have to worry about your pants getting wet.
    Third, when you do have leakage it will be just that, only 'leakage'. A few drops may come out. It's not like you'll be pissing like a racehorse inside the pads. Also take comfort in knowing that for the 5% who don't gain complete continence on their own there are additional treatments that will bring even them back to full continence. Medicine has advanced so far from many years ago. And most importantly, do the Kegel exercises regularly as instructed. They will help you restore your continence quicker.

      Is anyone researching why some men remain incontinent after prostate surgery? Doctors say things like “it just kicks in” which is not medical language just voodoo.

      @@artmaltman No research is being done, my urologist says. I Had the surgery a year ago and | am incontinent and impotent. Read any article about this type of surgery and it's all about the incontinence and impotence. I think it's far more than 5% that does not regain continence.

      @@kennethhudson4769 I tried Viagra, but my penis coloured a bit red and that was it. My nerves are damaged, right removed and left not maximal spared! After one year I could feel a bit of an orgasme again. Flacid of course.

    I had my RP 8 days ago and had the catheter removed yesterday. So far so good with minimal leakage initially and totally dry overnight. Was concerned about the feeling that I had to pee every hour or so during the night. Watched this video again when I woke up and my experience was exactly the same as your 1st night Mark so it was encouraging to hear that I'm not unique and it should get better. So far my experience has been quite similar to what you outlined in your videos. Thanks again Mark for all your hard work in sharing your experience with us all. It truly made the experience so much easier.

    I am also a patient of Dr Samadi who's surgery is scheduled in two weeks. I have full faith and confidence with Dr Samadi who showed a keen interest in my case and the ability to get everything right. At 73 years old my cancer was diagnosed at the early stage and looking for a positive outcome. Mark, your videos are great in providing confidence and easing anxiety. Thanks so much

    Had my prostatectomy a week ago. I am getting my catheter and JP drain removed tomorrow morning after exactly 7 days. Can’t wait! Thank you for letting me know what to expect afterward.👍

    A decade back, I had Green Light Laser surgery that was very effective and only required catheter from Friday until Monday. Problems was, this is when I discovered my contact allergy to Latex! Needless to say, for the protestecomy they used a silicone catheter, and it has been MUCH better!

    Also being in good physical condition helps in post surgery.....I too had surgery and came out on top...never incontinent and was having erections within five weeks....further treatment changed my sexual function but when it was over I regained it and have no trouble...sensations are stronger yet different from the passed yet I feel great and blessed in the end!...ps my surgeon was at Yale and was fantastic!

    Thank you for a great video. I'm 6 days post operation (Robotic Prostatectomy) and I am due to have my catheter out on day 8 and must admit that it is my (current) biggest fear. Your video has helped me feel better about having the catheter out and, although I don't think I'll be as good you, it does give me confidence that I will eventually regain control. In addition, I have been a little like one of your other commentors, and had some bladder stress issues with the catheter. We'll see if this affects me after it is taken out.

    Day 13 after the surgery for me. I understand the set backs. I just fill like am peeing through a stir straw ! I pray that gets better. Anyone have or had this issue ?

    It’s been said by a friend, who also had this procedure, that if you are in good mood mode, you won’t leak. But, if you hear bad news, the leakage returns, especially under coughing, sneezing, laughing and standing-up conditions. I had the Davinci robotic procedure two weeks ago and so far, leakage is frequent, but not excessive. All the best for you, sir.

    I did my prostatectomy 14 days ago and the catheter was removed on day 8. Since then I have incontinence issues with some days worse than other.
    I can sleep through the night without any leakages and wake up and use the bathroom as normal but after that I start leaking and have wear my adult diaper for safety.
    I know it's only 14 days but I feel as if I've made a lot of progress!

      My PSA test returned a 1000 Litres a day to just under 100g within a inconvenience guard.
      Mild ED which is being treated with Cialis and the use of a pump.
      I grated for life even i know life has it was will never be the same.

    So sad I don’t live in NY and neither my secondary insurance cover there or my financial status can fit it. I red about Dr Samadi and his honesty explaining his technique. No all surgeons are so open, many are prepotent and arrogant( unfortunately), don’t give you the chance of discuss or compare their technique with another specialist. But it is already done...

    At NO time (in the UK), was I told by my GP, hospital doctors, nurses, or the surgeon, that I would (or May) suffer urinary incontinence AFTER prostate surgery. three months ago It came as a HUGE NASTY surprise, very UNwelcome. Now I am left to find a way of handling this gross inconvenience. Am I upset and angry?? YES!!!

      I just had my cancer surgery done. I'm in same boat. No control. Do proper kegal exercises, vitamin b1 and nutritional yeast, and long walks. It can take up to 12 months for pelvic muscles to maybe control it. Should see improvement in a month. Study proper exercises on you tube fir leaks.

    I did my prostatectomy 4 years ago. Since then I have had both ED and incontinence issues every day with some days much worse than others. But I am cancer free so there is that.

    Hi Mark, happy to hear that you are cancer Free. My boyfriend had his prostate removed 15 days ago, he is doing well, urination is almost normal. One concern I have is that he is going back to work (out of state) in the next three days and none of his pants fit. He has been walking and eating healthy, but it seems to me that his stomach is bloated, did you have to deal with that? And how did you handle it?

      ybarnfield The first 30 days after surgery are the most critical. The body needs to heal, and there are all sorts of possible (temporary) side effects. Bloating can be caused by a number of factors, but the two most common are gas and inflammation, and often both. It should be temporary, but if he watches his diet, things could improve quickly.       The best fastest way to reduce inflammation is intermittent fasting. Eating only 4 hour window in a day. Eat 4 eggs and or salmon and salads and hamburger meat. Stay away from sugar, pasta, grains, junk food till inflammation is gone

    I had my surgery June 21, 2023 18 days ago. My catheter was removed after one week from the surgery shortly after that I contracted infection in my urinary tract and then one of my testicles. I’m recovering very slowly. I can’t walk very much at all. If I exert myself the pain my infection flares up terribly I feel so since the surgery I developed a new hernia not fun.

    I just finished prostate cancer surgery, and the catheter was removed 4 days ago. Zero control every 10 minutes 24 hours a day. Very depressed. I had an extremely large benign prostate, so pelvic floor muscle was never used and weak. Studied 3 different pelvic physiotherapists on youtube each had different approaches. I knew I had to get it right if I had any chance. Found penis clamp and good external catheter system if it ends up that. Yesterday, I finally saw the slightest improvement controlling. I'm level 4, no control. 3 control laying. 2. Control walking. 1 control standing up, sitting, lifting, bending, coughing. Now, some control laying down. Went through 20 diapers and 80 pads in 3 days. Vitamin b1 and 4 tablespoons of nutritional yeast are extremely important for 3 months. I pray I'm not stuck with a bag or clamp for the rest of my life driving a truck. Up to 12 months before pelvic muscle to maybe control it if exercise correctly. Say a prayer for me it's awful. Long walks and proper diet also extremely important. Only 1 cup of coffee or tea and before 3 pm. No water after 6 pm. Water and electrolytes during the day.

    Thank you for this! Had my prostatectomy yesterday and came home today. Only thing different with me is they didn't give me any gel for the end of my penis. Still can't stand up straight, everything hurts.

    My biggest problem was dealing with that catheter. That constant feeling of having to urinate drove me nuts. It was like having to urinate and had having to hold it .

    Just curious about the technique your surgeon use to do your procedure it’s seems that it help you out a lot just want to know why they won’t share this technique to help others to have a move successful and faster recovery and better chance that at life after the removal.

      This is 2024 and I just made a comment to Mark to ask him the same question. I found that a new procedure is called the .... "Retzious Sparing" prostatectomy. I live in Florida and I discovered a urologist in Weston Florida at the Cleveland clinic who does this procedure. It takes approximately an hour more and they go in from ...Behind" rather from under the bladder. Protecting the vital area that messes with incontinence. It's a lot more difficult but a tremendously better outcome relating to incontinence!!!! I am debating either of that approach or proton therapy.

    Hey Mark! I've got prostate cancer. In one section of prostate I have gleason 8 so I've scheduled a surgery. I've also had a pet/ct scan and there is no other cancer so I'm relieved. I'm looking at some other options at this point like radiation treatment. What was your decision process? I've been listening to prostate cancer research institute and they recommend radiation. Surgery is not encouraged.

      Hey Rick, sorry about your diagnosis. I had a prostatectomy back in July, I'd definitely recommend a RALP over radiological because you could get secondary cancers. Think a lot depends on your age.

      Recovery time wise and doctor was able to see more while going through the process. Robotics for me.

    I would do it as a last option and can’t wait any longer. He is one of the lucky ones. Incontinence and other issues is very possible. No guarantees. But good info

      What do you mean last option? You're talking about radiation instead? I am debating proton therapy or prostectomy. I discovered a new procedure called "Retzious Sparing". Only a handful of doctors perform this procedure.

      @@kennethhudson4769 I still have many questions as to wether I should of had the surgery, having said that, my urinating track works good, I urinate more when it’s cold and I do leak when I lift heavy objects but overall all it all works normal.

    Hi Mark, my second message to you. Had the surgery on the 2nd of August, and I’m having the catheter removed on the 14th.
    I must say I’m a little nervous about my first night and there after. I guess I’m going to wear dippers for the first week just in case. I hope also my functions return as fast as yours, and others who have committed on here. Needless to say it will be one day at a time.
    I’m more worried about my biology results to be honest. I think its also something people should be asking the doctor about. When they return to have the catheter taken out. The results should be in by then.

      At +10 weeks. All signs of the cancer gone PSA at 8 weeks was 0. So far so good. Off the pads at +19 days. 100% continence restored. Still working on other aspects of rehab. Just need to establish a new daily routine .. will be a long journey but its not too overwhelming at the moment. Seems like i have a lot more energy now the body is not fighting the cancer. Sleeping better etc. So yes, a positive experience so far 👍🏻

      @@MrSteamDragon That is great to hear. I'm 2 months out of surgery but I still have incontinence. Sometimes I think I have it under control but then I have that leak while I'm standing. Did you do anything special to alleviate that problem? I purchased a pump for the ED and it works for the most part. I also am investing in injections for the time being until I get over the problem. I hope to get over the problem.

      @@kennethhudson4769 Pre-op my physio gave me a strict program of pelvic floor exercises - I stuck to these 3x per day for 6 weeks prior to the op. She believes that helped. I had no leakage at all as the catheter was removed @ +7 days. Wore pull-ups to sleep for about 9 days but moisture content in the AM was mainly usual sweat. Swapped to pads at night till +19. Try to take note of when you normally drip/leak, what were you doing at the time etc. Then try to anticipate this when you can and squeeze out a kegel to ward off the leak. It's hard to get this synchronised at first but with practice it did work for me. This can take time, just keep at it - don't get impatient - it will improve over time.
      Forget to mention earlier - I had pretty severe perineal pain for 7-8 weeks post op. Taking painkillers and Celebrex was/is managing this - but still annoying. It is starting to improve now and surgeon suggests after 12 weeks it should be mostly gone.
      At 10 weeks (tomorrow), still get the very occasional drip at odd times, so I wear washable undies when going out, mainly for peace of mind really. Re the pump, yes the usual post-op rehab regimen with 5mg Cialis every day. We find this works at the moment. Will see how things progress for now before embarking on something else.

      @@MrSteamDragon thanks for the feedback. I do kegels throughout the day. My control has improved but I have a ways to go. Before surgery I always concentrated on stopping my urine flow. Getting my continence back would be a major plus in this ordeal. I'm glad you conquered that hurdle. You give me hope that I can do it as well. THANKS!!

    Mark, thank you for sharing your experience. Three days after my robotic surgery (which was done on the 03/23/2017) with the catheter placed I started to get a discharge. I now have a bladder and urinary tract infection. The pain is not pleasant and the constant feel of having a full bladder is not good. I cant understand why I wasn't given antibiotics until a few days ago. My surgeon is one of the best in the UK and I was treated at Christie's. The surgery was a great success. My main issue has been with the catheter. I was in recovery for three hours because of it as I felt like my bladder was going to burst even though it was empty. Oh, and lets not mention the wind pain!

      Michael-Alan Taylor I just finished my antibiotic course (cipro for 8 days)luckily with only minor side effect, but still feel the burning urinating and now it starts with a little blood drops,not sure why!

      That's normal. I know that I didn't want to hear that either. It's normal to think you have a infection or you got to poop, but taking stool softeners or exlaxs can make matters worse. Softeners are ok but, remember that pressure comes from the surgery. After my catheter removed I felt 65% better.

    Had mine removed instead of opting for active surveillance. Why roll the dice with cancer?

      Well I am 66 with Stage T1C and have opted for active surveillance. My Urologist explained radiation, radical prostatectomy and active surveillance. We discussed the pros and cons over about 20 minutes. I chose AS because we caught it very early. If it gets worse I will do radiation before surgery. Each of us has to make our own decisions.       Outcome each time. Its over...

    It's depressing to hear testimonials of people who have little to no incontinence right after surgery. My catheter was removed 15 days ago, and I still have no control over the it. My brain does not get any signal that my bladder is full. It is better at night when I am in bed mainly, I believe, because I am horizontal, and gravity is not working against me. I am okay sitting but the moment I get up and walk I can feel my urine running out of me. I have to change my Depends several times during the day. I am hoping that time will fix this. My gas bloating is gone but I still have swelling in the padded area above my penis. This too will take whatever my body needs post op. My only real pain has been on the right side where one of my lymph nodes were removed. This is most likely nerve pain that will improve in time. I am 68 and my Gleason rating was an 8 out of 10. The cancer was contained to the prostate and no post op sign of spread but it was an aggressive form of tumor that took up about 60% of the prostate gland. It is obvious that men having this surgery in their 50s have a better and faster result than those 10 or 20 years older.

    Oh, Thank you for telling me how Perfect your experience is. I for One have every issue with constant urination - nonstop. As much as I try and have practiced the exercises. I am unfortunately going thru 4-6 Pads a Day and Minimum of 2 Pull Up's. But knowing that everything is Hunky Dory for you is just so special (Not). My Surgery was with the Davinci Robot, same for you?

      I Had the surgery a year ago and | am incontinent and impotent. Read any article about this type of surgery and it's all about the incontinence and impotence. I think it's far more than 5% that does not regain continence. No research is being done, my urologist says.

      I'm in same boat. No control. 20 diapers and 80 pads in 3 days. Just started slightest improvement today. Do proper kegal exercises for leaks. Vitamin b1 and 4 table spoons nutritional yeast for 3 months. Long walks. Best of luck to both of us.

    Don't forget there is pelvic floor physical therapy before and after surgery!! It should be an integral part of your recovery!

    Had my catheter out 2 days ago...first night got up twice and pee with normal control...I where a pad and have had a couple of spurts when I cough or laugh...nothing major...I just felt a little sting at tip of penis so I'll put some neosporin on incase of minor irritation from pad...but I'm happy to say I have normal control and things should only get better..so take it easy first 2 weeks and concentrate on normal control...as far as bowl movements a stool softener helps so you shouldn't have to force anthing...

    Catheter came out on day 7, Not a issue, Urine was still raspberry color, went thru about 6 diapers and 9 pads, Had a small stream off and on, couldn’t control
    Pelvic area still gets a cramping pain every couple hours
    Stomach incisions healing well
    Very bruised on sides, stomach bloated
    Every time I’m push gas out, I urinate
    Pads, diapers gonna be expensive at this rate
    Day by day I guess

      Yes mate, one day at a time is all you can do. every one of us has a slightly different experience. Yeah my stomach was very bloated for weeks - looking ok now. Don't lose sight that time itself is a great healer Stick to your post-op rehab, make sure you get plenty of rest (on ya back looking at the ceiling type of rest. 😀) and you will see improvements in time.

    A great thing to use after any procedure down there is a product called Man1 Man Oil. It is loaded with vitamins, nutrients and anti-oxidants that will ward-off infection, moisturize the skin and generally just speed up the recovery process. It is a very helpful product, guys. Easy to use and all-natural too.

    What a joke. He was very lucky and I don't believe his results are typical. It's almost three months since my surgery and I am still incontinent. I started out with constant leaking; there is just no stopping it. I am seeing a pelvic floor therapist and am finally seeing some results. But there is no strong stream. I'm always dribbling or there is an uncontrollable spray, so standing to urinate (when I go to change my adult pull-up) is very difficult. Everyone says I should be patient, and I'm really trying. It is all so discouraging...

    Your experience is unique to you. I just had my jewel trimmed down. It was a big one. Quit bragging about your control. Everyone will have their own experience. Thank you for talking about the sphincter muscles. My dear friend Spanky suffered over 10 years. His cancer left the prostate to invade his body.i still have my prostate and it is a difficult recovery. Surgery occurred 6/15/2022 First erection Tuesday of this week. Today I had to leave my girlfriend in a hurry due to urinary pisselence. I think it's funny but I will be satisfied when I have total power. I pray for all of you .       I agree 100%! But I think he had the new procedure called the...."Retzious Sparing" prosectomy. Only a few doctors do this procedure. They go in from behind! Not from the front! I found a doctor in Weston Florida at the Cleveland clinic does this procedure. This is currently 2024.

    Mark, glad you and your surgeon had a positive outcome but please stop posting false hope and blanket statements that do not apply to all men! Every surgery is different. Every man's body is different. Every surgeon is different. Every man has a different level of health. Each of your videos present your surgery and recovery, continence, sexual function, etc as if it will be the same for all men. IT IS NOT.
    While I truly appreciate you trying to get information out to men who are facing the decision of having surgery, vs radiation, vs seed implants, etc, and I feel that you are trying to help other men with your positive attitude, I think it is dangerous to make such blanket statements to all men. Yes, share your experience. Tell us what you did and how it worked for you. But don't tell men to not worry, that they will have full sexual function, they will pee normally, etc. It simply. is not the same for all men. Perhaps you should create a "Mark's Cancer Experience Forum" instead where men can ask each other questions and share their struggles and victories so that ALL stories are told and people are not relying on you as some sort of authority to get their hopes up, only to be gravely disappointed.
    Lastly, great that you are happy with Dr David Samadi. Not everyone has the means of traveling across the country to choose one specific surgeon. Insurance (or lack of), reserved funds to self-fund, etc are all obstacles. Finding a surgeon who is experienced and has a good track record is great advice, but to recommend one specific surgeon on CZcams seems insensitive to those who don't have that option.

      Paul I agree . I am 2 weeks since catheter removed and I have leakage, still wearing pads, but I hopefully will regain control . I too had an excellent surgeon, I am reasonably fit and still doing exercises .

      Hello Art,
      I’m sorry that you are faced with this challenge, but I’m glad you’re still working with it (at least you can still walk, right?) There’s always a bright side when you look for it!
      Since my robotic prostate cancer surgery, I’ve had an incisional hernia repair with mesh (that failed), and a second hernia surgery to repair it, with deeper mesh (that is also failing. I’m told by my doctor that the more it is repaired, the higher the chance it will fail. So I’ve decided to live with it! But I can still walk!
      Getting older comes without a warranty, so I just focus on enjoying the things I AM able to do, and accept the things I cannot.
      Hang in there, and take what you find on the internet with a grain of salt!
