    Had my robotic surgery 5 months, thanks to Marks early podcasts I was able to be confident in taking this route and along with my spouse got through this and recovery uneventful....I was fortunate to only have a catheter for 1 week...and light pads after 2 weeks ....no pads at all last 6 weeks

    What is the "LONG TERM SOLUTION". Is ADT just the what all Uroligist resort to when nothing else has worked OR has Identified where the cancer is in my body???

    Gentlemen, if they suggest having surgery to you please investigate the alternatives. The modern treatment is CyberKnife, but you have to go and find it. I have Gleason 4+3 and I was told by a surgeon at my local hospital that the treatment for me was surgery. Long story short, we were talking on the telephone later and he said this to me: ‘when you look into this you’ll find what we do is barbaric’. I looked into it and spoke to two professors and two consultants. I elected CyberKnife with one of them. I’m starting hormone treatment now and I’ll have CyberKnife in about 2 months time. It’s 5 treatments over a week and a half, there’s no pain at all unless you call the lowest grade of irritation pain, there’s no catheter, the prostrate stays where it is, no nerves are cut, the chances of incontinence are virtually nil, the return of sexual function is much better, the list goes on and on. There’s lot’s of information about it here on CZcams about it. As an aside, all the top people I went to see strongly recommended that I have a PET CT scan to check that the cancer hasn’t spread, but when I took this message back to my home town they looked at me as if I had two heads, so I ended up having it in London paid for willingly by my health insurers. I wish you well with whichever path you go down, but if you don’t need to be in a rush, don’t be and check out your options. It can be a lot easier in the long run 😊

    Im 6 weeks post op. Not much leakage. 1 pad per day. A lot of sweating at night though. PSA test upcoming in maybe a week or so. Immediately after the op was not too bad , catheter out a week later. Very badly constipated because of pain relief tablets, but that was expected. Started exercising straight away and am now back using weights , and a LOT of walking. Especially upstairs. Hoping for a good PSA coming up. And determined to get some kind of erectile function going forwards. Much love to all going through this , but stay positive try not to worry. I know its hard. Im 65 , and will gladly answer any questions anyone who is just starting this process has . ❤

    I saw a surgeon today who practices"RETZIUS SPARING" . Told me that a high percentage of people will be fully continent within ONE month! 50% right after the catheter comes out! That is such great news! Westin Florida Cleveland clinic. Google it Mark. They don't touch the pelvic floor area. Remains intact.

    After such a traumatic operation in having a catheter in for a week or more how could you even think of having sex?! You say within 3 weeks? It will probably take me 3 months minimum! Ha! After my biopsy I play with myself over a month later and blood came out! Oh my gosh! I was terrified and it felt terrible. I tried again after maybe a month and a half and the same thing. Now a year later I hardly ever play with myself anymore because THAT experience really turned me off on .....getting off.😢

      You have to keep the pipes clear. After a biopsy, the more often you ejaculate the faster the blood will disappear. The doctors don't warn you, but that first ejaculation after your biopsy is a shocker. In my case, I waited a few days after my biopsy and my ejaculate was bright blood red. Anyway, I took it as a challenge to clear it up in the shortest possible time. Three days a week for three weeks and I was back to normal.

    Hot dogs, bagels and cream cheese french fries, pastrami sandwiches etc! I never thought that you would be eating such junk Mark not a million years. Anyway so when you had the catter in for seven or so days you didn't have a BM? At all? How long did you go before having one. Also didn't you take any kind of laxative? So all that junk food you're eating it was building up you didn't go to the bathroom? Wow! That really surprises me coming from you Mark you seem like such a healthy dude man!       Was this guy eating that junk after he knew he had a high PSA ? Let alone surgery and a recurrence. That’s amazing. I’m surprised he’s drinking a Diet Coke . It’s a known fact the ingredients in those sugar free beverages are carcinogens, I think zero sugar coffee creamer messed me up . I can’t believe he ate that stuff .     Question, do you have a plan to address the likelihood of proctitis from radiation occurring in the next 1-2 years? I live in Jacksonville and Shands UF Health has a major proton treatment center. Several men I have spoken to that had similar pelvic bed radiation treatment ended up with differing levels of proctitis. Doesn’t show up immediately but usually 1-2 years after treatment. You appear to be very proactive in treatment options, just curious if you have investigated your options….if needed…

      Proctitis is one possible side effect of radiation treatment. There are cases of both short term and long term instances of proctitis. The shorter term version tends to resolve itself and is more common. Long term proctitis is more rare, and sometimes requires treatment. It's important to note that each of us will respond uniquely, and generally some of the factors that will determine our likelihood to develop some form of proctitis are: - Higher total radiation dose - Pre-existing gastrointestinal conditions - Concurrent use of certain chemotherapy agents - Advanced age Proton therapy with use of a rectal balloon is far less likely to develop the condition, as part of the rectum is inflated and kept outside of the high energy field. IMRT radiation covers more area, including the full rectum, and may see more cases. My advice to anyone is to not assume that these side effects WILL occur, but rather plan as best as possible to have treatments that minimize radiation of the rectum.

      I think it’s important to share the facts that the use of the balloon does reduces the chances of proctitis but by less than 15-20% when proton treatment is performed on the pelvic bed. I applaud your optimism and I equally feel men understand the documented risks…

    I had surgery on 7/9/24 . The cramps were horrible until I passed air/gas. Now I'm recovering, the catheter is a pain and I'll be glad when it's out, my catheter is still in , 15 + days. Anxious about incontinence and ED side effects. 🙏

    I had the surgery in 2016 unfortunately the doc couldn't save all the nerves so I have to take the Injections to maintain a election i was 60 yrs old

    On July 3rd, I had sugery around 1pm, spent a couple hours in a recovery room, was discharged and home by 530 about 25 miles from the hospital. At home, climbed two flights of stairs to the bedroom. Going up and down the stairs has been good exercise.

      Partrick, I suspect that you may not have done your Kegels long enough before surgery and with the right frequency. They almost always do the job.

    Had proton therapy in 2017 at Hampton Va. No side effects, no pain an easy run. Not so now. Its baaaccckkkk. Proton people seem not to want anything to do with this. Pushing more to DR. FREEZE.🦇

      The radiation oncologists will rarely radiate a second time. I am not sure why, but for a recurrence after radiation, they now have a host of remedies that will reduced or block testosterone, and effectively keep the cancer at bay. Have you looked into this?

    How to get Rock hard erections. Thank you Mark for telling us about your experience. I had advanced prostate cancer diagnosed at age 68 and was already experiencing ED. I had Lupron injections that destroyed my testosterone levels and radiation treatments that successfully treated the cancer. At that point I had no desire for sexual activity even though I was taking 20 mg of Cialis daily. One year after radiation my PSA remained low and i started Testosterone treatment, 2 pumps every day, within 2 months sexual desire came back with some 1/2 erections. I started stimulation every day and was so surprised that I was having increasing stronger erections. My PSA tests have slightly elevated, which the doctor says is normal. I have been on Testosterone treatment for 8 months. To achieve a hard erection I use a few penis rings under the testicals and over the base of the penis which gives me a rock hard erection for 30 minutes. At 30 minuses I remove the rings to allow blood flow to continue and repeat for another 30 minutes. I do have dry orgasms that are very pleasurable. I never thought I was going to ever recover to this point. I am hoping for increased erection strength over time. I urinate as if I was still 20 years old, and only wake 1 to 2 times a night to urinate. Radiation therapy along with androgen deprivation worked for me.

    Hi Mark, thank you so much for sharing all this info! I had surgery 1.5 years ago, my psa dropped to 0.004, but it keeps rising very slowly since then. I currently am at 0.03. If my psa reached 0.1 my doctor recommends pelvic bed radiation, but he said that erectile disfunction is almost a certainty. Could you please share your opinion? Thanks!

      Any PSA score below 0.1 should be considered undetectable. If you try to monitor and track changes in PSA below 0.1 it is NOT meaningful. Erectile disfunction is not always the case. I am 7 weeks into radiation, and my erectile function is the same as it was pre-radiation. Let's see if that changes.

      Jeffrey, if your doctor fights with insurance they will approve Orgovyx. They have to be coaxed. It’s best to use a Medical Oncologist to do this for you.

      The human mind always wants to be chewing away at solving problems. Mindfulness works wonders. I was told to think of a couple safe places or people in your life past and present while you’re lying in bed. At first your mind will keep jumping back to work on your problems but with time you’ll find that you’ll eventually go to sleep in just a few minutes.
