The first officer is holding the ingntion to on (ground). The captain is holding on to the fuel-flow lever, once the N2 of the engines is about 20%, he will raise the lever which introduces fuel into the engine and this is when the engines ignite!
@luislaurencio MD-80 aircraft are not capable of making over-sea flights. They only have enough fuel for flying national or across state lines (in the states anyway, otherwise over certain countries in Europe). 767's, 777's, A330's, A340's, etc.. are capable of flying overseas.
overhead its the ignition to be hold manualy, down the levers are the fuel switches...but they are rather levers also here. ;-)

Yes they use rudder pedals while on the ground (at low speeds), and the brakes are on the same pedal.
I think he meant he went to Dusseldorf on the MD-80, to hop on another aircraft for a (long haul) flight to Miami. Probably via Zurich or Amsterdam.

On engine start, what buttons are they pushing on the overhead panel, and what levers or buttons is the pilot holding down behind the throttles??

Was the long haul flight after this in an MD-80 also? Doesn't seem like it would make the distance.
When they taxi do they use their feet to steer? And is there a break by their feet?
