Bent Posture: Most Neglected Problem with Hemiplegia

- Hemiplegia (weakness on one side of the body) can cause the body to bending sideways. This is called laterally side bending. If the body is left to stay in this side bend position, the soft tissue structures on one side of the body will become shortened. This can make walking difficult. This can also create low back pain. The way to treat this is to stretch (elongate) on the shortened side of the body and retrain the body to maintain a “straighter” position. This video is part one of the best exercises to correct this issue.

⭐⭐ Thank you Dr. Tara for your super respectful language. In Germany, they call it Pusher Syndrome as if I push to my involved side. I always found that disrespectful and inappropriate. Your explanations are science based and very respectful to your survivor community.
And you are very right with the stretching. I was told to do latissimus stretch every day in order to keep a straight and nice posture and it works.

This is a huge problem with almost everyone with stroke. Another problem is affected muscles work well when doing exercises, but go back to being spastic when we stop, how do you fix that? Idea for a new video. Thanks as always dr. T.
My dad sits and lies on the couch all day everyday so he needs stuff that he can lie on or sit on that helps him. 1hr/day does not overcome the other 23 hrs per day.
Please make a video on that.
If my dad had something he could put his affected arm around while sitting watching tv or a chair frame he could sit on while sitting on the couch then he would be doing the right thing all the time

I think I figured out my hip, I took your advice and shifted my weight over my affected leg and that straightened out my hip on my affected side 😃

Good one. Do feel it’s a chain reaction and part of spasticity being part of the body trying to keep itself up. Changing my shoulder strap has been a game changer. The Erixthree from mediroyal was a way better fit and support, I thought it would be good for my subluxation but expect the difference on my overall posture and walking

My 85 year old uncle had a stroke and has been in hospital now for 5 weeks with hemiplegia and the only improvement he has had is eating soft mushed food and drinking thickened drinks. he cannot move his left side of his mouth, arm, hand. leg, feet. He is very scared and cries (I had never seen him cry before) and his family is not encouraging saying they don't believe he will ever walk again, fish again, hunt again, or ever be able to go to the washroom on his own again. He is always complaining but manly about pin.

I have right-sided hemiparesis due to a brain bleed as an infant. I'm thirty five and I have recently started doing pushups for bilateral strengthening. After a few weeks, I noticed that my right humerus was shorter than my left. This was not part of the plan of taking better care of myself. It's one more difference that is now painfully obvious to me as someone who was already self-conscious of their body due to Cerebral Palsy. Ugh.
The right side of my body isn't on fire like it usually is when I engage those muscles. I am definitely seeing results by doing pushups, because I have to engage my core to stay level on the floor. My body isn't a complete mess when I circumduct my shoulder because my other muscles and joints aren't having to work as hard to keep me upright.

Hi Dr. Tara. Your videos are highly appreciated and I really value what you are doing for the people in need. My dad had a TBI a year ago that affected his left side. His left leg feels heavy that he cannot left it up easily when doing simple chair leg rise exercises, he also struggles when trying to stand up. He has some spasticity in his left foot that he uses an electrical stimulation device to help with dorsal flexion, and when he walks he actually leans to the right, on the non affected side. What is your suggestion for his condition especially the leaning issue? Thanks

I am using a hemiwalker as the hand on my affected side is curled tight.
I do have pain on the hip of my right (affected) and my knee turns inward.
The stretches look great but in addition, are there any exercises that would strengthen the hip flexors.
You're my hero as I have asked PT.'s for suggestions but they don't seem to aware of any.

#spinecorrector I think I did have this problem and I think I may still have it slightly. But I started to do a lot of side stretching a couple of months ago and I think that helped me out a lot because my straps on my shirt used to fall down all the time and the more I stretch the more they start to stay in place. I still have occasions when they fall down but it is rare now I’m trying to get to the point where they don’t fall at all. I also noticed when I stretch on that side all the muscles and tendons always felt so tight and stiff it was actually uncomfortable when getting a massage by spouse. That has also improved I’m not sure if it’s similar to what you’re discussing but it made me think about it.

Can you do videos with exercises with specific equipment so we can utilize what we have? I'm building up my equipment since I realize this is a forever thing but it would be nice to know what else I can use them for. I am a right-sided hemiplegia for 25 years. Back in PT after 25 years but no neuro here.
Some equipment I have: wedge, yoga block, wobble board, hurdles, wrap weights, yoga ball, skateboard, parallel bars, gym mat. I’m making up my own exercises sometimes.
I have a off-topic question. I just re-watched the motor control in synergy videos and I was wondering how does one get for instance a flexor muscle to stop firing when you’re trying to extend your arm or your leg. It seems like it would take strengthening the extensor muscle. they fire both at the same time. So it seems like you would have to get a flexor muscle to stop firing somehow in order to extend. Could this be a topic of conversation for a video? It’s frustrating when you try to extend a body part and the flexor kicks on at the same time over powering the extensor of the arm or the leg.

It feels like the drop foot side is the one that's already elongated and that the other side needs to become elongated in order for the feet to be the same length relative to one.s structure

Is the crunched in side the affected side or the strong side?
I have stenosis and scoliosis with hip rotation so this is a real problem for me. Walking is difficult and I struggle with balance problems. I've been exercising and trying to get stronger but it doesn't seem to help much with balance.

After a paraplegia patient can get bowel or bladder movement control get back like normal?
My experience, I got swallowing back in a out a month, bowel movement control within two months. But, I was in comma for about 3 weeks, that may have delayed my recovery. Hope it helps.

Boo! I tried really hard to make this not to be me. I do hike my hip, I still have a bit of subluxation with a winged shoulder blade and back pain. Pretty hard to deny🙄
Michelle. You got this!!! You are undoubtedly a hard worker. How do I know? You continue to watch these videos. 😊 That means you are dedicated to your recovery. And to me, that is 90% of the battle. Long story short, this (lateral trunk flexion) is 90% of stroke survivors. It is not a huge deal, but knowing this, might help you to make your "quality of movement" just a little better. Remember, it is all about that 1%. If you are even 1% better, you are better 😊
