    When I was diagnosed with male pattern hair loss 10+ years ago, I felt overwhelmed trying to decide which DHT blockers (if any) I should use.
    Which DHT blockers work? Which ones put me at risk of side effects? What does the real evidence say versus marketing hype for the latest DHT-reducing supplement, topical, or drug? The answers may surprise you.
    Today, as a medical editor, I'm making this video so you can get to the facts (not fiction) about DHT reducers. This way, you can bypass all of the misinformation online, avoid my treatment mistakes, and uncover which DHT block actually work (and which ones are a waste of money).
    Specifically, we're going to evaluate 6 DHT blockers in terms of their POWER versus SAFETY profiles. This is, in my opinion, the best way to organize this information.... so you can climb up (or down) the ladder depending on your needs and preferences.
    Despite what marketers lead us to believe, effective DHT reducers (natural or conventional) don't need to cost a lot of money. But some "natural" options may not be as safe as we think.

      Men are mostly affected by Androgenic Alopecia - So nothing really beats Finasteride/ Minoxidil combo with Microneedling.
      Anagain, Procapil, Baicapil, Capixyl, Redensyl etc. can help, but NOT to that extent.

    I have been taking oral finasteride 1mg per day for over 2 years now. Zero side effects. My only regret is I wish I had started taking it much sooner than I did. When I purchased the first bottle I kept it in the cabinet for about 6 months too afraid to take a pill thinking it would destroy my life, reading all the others stories about side effects. Looking at the actual studies, reported side effects are about 2 percent which was the same as that experienced by the placibo group. So I started taking it and have never had an issue. It has stabilized my hair.

      @@gergelyzoltan8422 Actually, as men get older they become more sensitive to DHT than someone who is younger. That's why as men get older they are more likely to have hair loss even though DHT levels are less. In that sense, finasteride proves to be even more effective in my case as it has stopped hair loss. As far as side effects, a billion dollars worth of double blind studies since the drug was introduced has shown consistently that about 2 percent experience side effects, which was the same as the placebo group experienced, so the drug is very safe. For those who do unfortunately experience side effects the studies also show that the vast majority stop experiencing the side effects once the drug is stopped. The side effects in most cases are mild as well. That is why it is important to speak to your doctor and not get your information from 3rd party stories from the internet.

    These side effects matter to guys more than the medical establishment realizes. A good friend of mine was taking Dutasteride for prostate enlargement issues and started growing breasts. Dropped it and said he’d rather use a catheter to pee.

    Man, I got into hair loss treatments 13 years ago when I discovered I was losing hair. Had severe side effects from fin and decided to just let it go. I see now that after all this time nothing has changed at all. Same treatments, same problems.

      @@PerfectHairHealth Why don't you ever talk about topical dutasteride or dutasteride mesotherapy? Topically many people report that dutasteride does not pass as easily into serum and affect body system as easily as topical finasteride. Many can't tolerate fin. Lots of clinics in Spain offer Dutasteride mesotherapy, plus is you only need to get it once a month, and for topical dut - once a week because of long half life of 4/5 weeks..
      Would love to hear your thoughts on the matter!

    This video is perfectly structured and addresses the same concerns many of us share. Two questions.
    1. Would any of the alternative natural DHT blockers compete with Saw Palmetto? I'd be interested in seeing a video on the ones not covered in this video, unless they are all "F".
    2. Can an SSRI such as Prozac cause hair loss (not in the short term but while on it long term)?

    Noticed my hair thinning in May 2023, and by July I can see more of my scalp mainly the front and top. Can’t pinpoint if it’s TE or something else. No genetic reason. I had a stressful family event last fall. It’s still a lingering stress. Got on minoxidil a week ago. Started microneedling today. Getting on nutrafol tomorrow 😅. And seeing my dermatologist next month to get a full bloodwork analysis etc done. I said all this because as stressful and near to tears I am about this sudden hair loss, your channel really helps keep me informed and somehow I feel comforted. So thank you. So much.

    Some people have found that the Edgar Cayce (a medical intuitive from the early'ish 1900's) advice of crude/unrefined Pennsylvania "sweet" crude oil occasionally applied to the scalp helps to stop hair loss and boost growth.

    Great content Rob you are an hero in this field and pretty sure you drove me to avoid all the "natural useless" solution and simply go talk to my dermatologist. since mid september I am on minoxidil and finasteride , microneed 1 per week, soon I will start to use an electric scalp massager. Starting to see improvements.
    One doubt: can you inquire more about "man who can t produce dht never go bald"? Isn't this a confirmation that dht6 is the cause (not a correlated phenomenon) of Alopecia?

      Thank you! Currently, I'm still employing stimulation-based therapies (microneedling, massaging, etc.) - albeit less consistently than in the past. No topicals, oral supplements, medications, or devices. I mentioned this in another comment below, but because my time is so constricted today versus when I started doing these interventions 7+ years ago, I'm also considering beginning a DHT reducer in the next few months. I'll be sure to make a video about my protocol soon!

    oral finestride and 5% monoxidyl definitely showed some improvement after 3-6 months (if taken/ applied daily) - however - the side affects hit me early on (1-3 weeks) and it was really disappointing as it does not affect all users - tiredness / aches and pains / and worse of all memory issues and depression .... it may work for you but it really does come at a high risk from my experience. Id rather have energy no aches and pains and a good memory and not be depressed than a head of hair.

    I think the majority of people who worry about Finasteride/Dutasteride side effects get spooked off too soon. When I first started taking Fin I had a side effect twice early on & then never again. For those who worry about sides I would take the doses for at least a good month before throwing in the towel. Of course if you're having bad side effects constantly I would stop also but I think too often people let one sign of a side effect scare them off when it wasn't going to end up being a normal occurence

    One day hair loss will be prevented - its a strange process, ive noticed sensations in the pattern baldness areas that feel like burning - the sensation comes and goes and ive noticed it generally marks the start of further loss and receding in the area. Also, and this has been documented the actual thickness of scalp skin reduces in balding areas plus when i get goosebumps they only happen with any strength in the side areas of the scalp. Anyone else noticed any of these things?

      Definitely felt it. My pinnacle was when I cut my hair really short, then used shampoo to thoroughly clean my hair and scalp. After that I could not stand being outside as my scalp felt like it constricted my skull and it was also super dry. Now after doing massages, using lard and oils, and having a longer hairstyle, I don't feel that burning feeling anymore.

    Saw Palmetto is truly magical i even noticed the oil production got lower but however low DHT causes some other things.I have noticed my muscle growth slowed down , i got itching on certain areas and back pain.

    You say 0.2 is enough for blood saturation of propecia.
    My doctor has me taking 1mg every Monday Wednesday and Thursday to help my testosterone. Is it better to take 1mg 3x a week or daily 0.2 for increasing T and still helping my hairline?

      Since the mechanisms behind massaging and microneedling are suspected to more so target tissue remodeling (i.e., angiogenesis and fibrosis attenuation) in addition to the stimulation of growth factors / signaling proteins that initiate anagen (i.e., the growth phase of the hair cycle), I'm inclined to believe that people with long-term adherence to microneedling / massaging have an easier time holding onto results. We've seen this reported by some members of our community, and there's also evidence in studies of this happening. For instance, a study on minoxidil vs. microneedling vs. microneedling + minoxidil showed that 6 months after quitting all treatments, nearly all of the participants using minoxidil lost their hair gains while nearly all of those who had done microneedling or microneedling + minoxidil held onto their hair gains.
      In terms of genes, there appear to be at least 220 genes associated with hair cycling and/or androgenic alopecia. To my knowledge, there's no clear evidence of baldness being more inherited by one parent versus the other.

    I took finesteride for almost 20 years starting in my mid 30s due to baseline elevated PSA. I didn't get side effects until 12-15 years later. When I stopped at just under 20 years, the side effects had gotten really bad. Unfortunately several years later, they never reversed. I would highly caution anyone from using this drug if they care about their sexual health.

      Im 17 suffering from the early stages of male pattern baldness and i already use minoxidil and now a derma Roller, I started questioning if i should start with FIn as well with the intention to just stop when or if the side effects kick in since i thought they would stop if i simply stop (would go bald if that happens but who cares)
      Now that i know that even if i stop using it it can affect my body its a big no no

      If you took it for 20 years it means you are now in your 50's. In your 50's it is normal for men to have loss of libido, sex drive and even ED. It is pretty common

    Rob, this is a DHT video but I need to ask you about oral minoxidil. I've read your article about it. You've mention 2.5 mg as a sweet spot for oral minoxidil dose if one want maximum results with minimum side effects for hair regrowth. Thing is I'm not old but not only do I have progressive AGA (though I still pass off as not bald) but also I'm a severe hypertensive patient, BP ranging anywhere between 150/100 to 180/140 if I don't watch my diet or exercise and if I don't take my drugs. And now I'm thinking if I'm taking blood pressure medication anyways why not try oral minoxidil? It will address both of my issues. Sorry for the long text and for wanting free advice but please suggest me WHAT WOULD BE MY SWEET SPOT DOSE FOR ORAL MINOXIDIL? I consider you to be the best and no. 1 source for hair loss information. Thanks in advance! 🙏

      Europe and South America has micro injections of finasteride in the scalp, BLOOD work shows no change in the blood DHT levels at all. And u only need ONCE A MONTH Micro injection

      Oral minoxidil is quite potent, affecting the heartbeat. For hair benefits, it is being prescribed off-label with a tiny fraction to start with. I would start with a very tiny bite off the tablet and get used to it for a few days, and then assess how much will work for you. Everyone responds differently in different doses. Tinker with it yourself and see what works the best for you. But, always start very tiny to acclimate your body.

    I have used both oral finasteride and oral dutasteride. The dutasteride works great, but it has a detrimental effect on a condition I have called "essential tremor" - it increases the tremor in my hands after continuous use. This makes it undesirable in my case, and I have to revert to finasteride, which is less effective but doesn't affect my tremor.

    For years, I tried most everything on this list and a few things that weren't including minoxidil and a laser helmet (anyone want to buy it cheap?) Then I accidently discovered what worked for me- or actually my wife did. After I moved to the Philippines and married her, she started washing my hair with a native product with somethin in it called gugo bark, from a vine that grows here. After 6 months I had a full head of hair, the first time in 20 years. I buy a 1 liter bottle of it here, which lasts several months for about $7.00

    MY BRUTALLY HONEST EXPERIENCE WITH FINASTERIDE
    I started losing my hair (around my temple area) as a 23 year old. I panicked and ordered oral Finnasteride (1mg) and started taking it daily to prevent more hair loss... at the time i was thinking "2% chance to get side effects? Pff no chance it will happen to me!"
    *famous last words*
    I immediately started having side effects. The first week i developed the famous ball pain and some other side effects like pain when finishing the deed... "if thats it, its not thay bad!" I thought...
    2nd week, my small (existing) gyno on my left nipple got 3 times bigger and developed gyno on my right nipple (which i didnt have prior)... i am a natuarally a muscular guy with a really big chest, so i legit started looking like i have fucking breasts... at that point i was reslly having doubts, but i persistent (bcs i was still scared of losing my hair)
    Month 2... bye bye libido... bye bye morning wood... bye bye sex life... hello 👋 insecurities!
    At this point i freaked the fk out. I didnt even think about sex once for a week straight (and as a 23 year old in a relationship with a girl thats a 10/10, thats a no go...) I literally lost complete interest in sex... I had never ever thought this is even possible...
    I stopped it 3 days ago and im starting to gain my libido back (thank god). The painful balls went away and my soft sausage is slowly starting to harden (lmao sorry for the analogies, trying to be pg12 hahaha).
    So now, im left with man boobs, 4 months worth of finasteride which im never gonna touch... more insecurities, still gonna lose my hair... but hey...
    Id rather be bald than lose my libido. I never realised how important this is for your mental health!
    Also, there is no frrigging way in hell theres ONLY 2% chance of you getting side effects. Look at this logic:
    1: Pharmacies make BANK from insecure men. Youd be surprised how desperation can make you take crazy risks (its in their favour to lie about the low percent of side effects bcs they PROFIT BIG TIME!!)
    2: How the FK are you not gonna get sexual side effects when literally 70/80% of your dht (your sex hormone) is being cut overnight!!! Statistically It makes NO SENSE
    So guys, if youre feeling insecure rn, just know you ARE worth it no matter fucking what ok? Your health is WAY more important.
    P.s. if finasteride works for you, good for you broski! But if youre experiencing side effects, i wouldnt risk getting pernament side effects.

I am thinking that having a poor diet, lacking nutrients and minerals and poor lifestyle would increase the odds of the having bad side effects and doing the opposite would give me some room to be protected of them.

    Great video. What are your thoughts on combining saw palmetto or other herbal remedies with the more powerful pharmaceuticals? Synergistic effect, or unnecessary and waste of money?
    I've also heard the GLA in borage seed oil or evening primrose oil can also help. Any info on this?

      It's a great question, and one that isn't easily answered without data. Technically, saw palmetto and finasteride target to inhibit type II 5-alpha reductase through similar mechanisms. However, saw palmetto also seems to inhibit type I 5-alpha reductase and decrease androgen receptor binding in certain tissues. So, you might get a little extra benefit by combining finasteride with saw palmetto. perfecthairhealth.com/saw-palmetto-for-hair-growth/

    Be cautious with Avodart (Dutasteride). Recent studies have shown links to severe depression. I myself had horrible side effects with this drug.I did get much better hair growth with it, but it isn't worth the side effects. Now I'm with finasteride and while it is not as effective I'm not getting side effects from it

      I got muscle twitches from it. It was awful. I could feel my shoulders and upper arms twitching all the time. I dropped down to Finasteride and it's fine.
      + I am not convinced taking this stuff long term does your brain any good...I wonder if in 10-20+ years time they find an increase in alzheimers or something? How scary would that be?

    Do you put somewhere studies to back your claims about nautral DHT blockers? And do you have any strenght list of each natural DHT blocker? Plus also as I research zinc or EGCG (green tea catechin) they both mostly inhibit type 1 5-AR, not type 2 so that makes them slighly useless for AGA.

    I'm on my second month using dutasteride and I can tell you that it has a much better cost benefits than finasteride. In just two months I already see good results on my hair and with less side effects than when I took finasteride. Actually I had no side effects until now. The good thing about dutasteride is that it increases the testosterone leves by up to 23% according to researchs.
    When I took finasteride it took 6 months to show the same results as I am now in two months with dutasteride. Let's see the outcome in the next months regarding results and possible side effects. I'm optimistic regarding that.

    I have one Question. There are multiple studies on topical finasteride that suggest the same systemic dht inhibition as oral finasteride. As far as I know, there is only one study that suggest a low systemic inhibition with a low dose of topical Fin. However this study only went on for a week and we know that finasteride accumulates in the body, which makes it very likely that even the small dose would reach 70% inhibition at some point.
    So why should topical finasteride reduce side effects by 30-50%?

      This is a great question. These estimations are partly deduced from (1) clinicians who've been prescribing topical finasteride over 2+ year periods and receiving patient feedback, and (2) a slightly less dramatic reduction in circulating DHT with the topical versus oral formulations over months-long trials, which seems to correspond with a reduced risk of side effects. On that note, there was an unpublished clinical trial (by Dr. Chiara Insalaco) of 300 men administering topical liposomal finasteride daily for 18 months. There were no reported sexual or cognitive side effects, and DHT only decreased by 7% (which is biologically insignificant). The data were presented at a conference and did not undergo peer review (or publication), which makes me hesitant to fully trust the information. Nonetheless, there are topical formulations that may help us maximize skin penetration and minimize systemic absorption.

    Hey Rob , my question is a bit irrelevant to DHT reducers and I'd really appreciate it if you could answer it. I got the Pfizer vaccine the other day and since then I have been shedding a lot of hair. I also have muscle pain to my whole body. It's tolerable but it's annoying. My two inquiries are :
    1) Is there a logical explanation for the hair shedding?
    2) Is there any study that prove that the Pfizer MRNA vaccine is safe? I know they got approved very quickly since it was an emergency so there is not a of lot clinical data , but I'm scared for long term side effects and to be honest I started regretting that I got the shot.

    I was taking oral finasteride and developed a worsening in my anxiety pretty quickly. Stopped it and switched to topical a few weeks later. The topical didn't give me side effects for quite some time, but I think around 3 months in (maybe less) i started experiencing sexual side effects. Low sex drive and just wouldn't stay as hard with my girlfriend. Ditched the finasteride, ditched the girlfriend, hopefully not ditching my hair for a few more years 😂

      I took oral finasteride for not even 5 days and it was giving me the worst anxiety I’ve had in my life! It almost gave me an anxiety attack during a college class, out of absolute nowhere. The feeling was close to a sense of doom, it was absolutely awful. I notified my doctor and he took me off of it immediately. I haven’t had any sort of anxiety like it before. It also made my tinnitus louder, and gave me bad headaches (I never get headaches) Luckily I was able to get a refund but I am sticking to topical minoxidil for now. Works pretty good.

      We're working on a literature review about this! Anecdotally, we've seen that 4-6mg/week of dutasteride topically seems to have very little influence over serum DHT levels, at least for most people. Higher than that amount, and we get a lot more heterogeneity in the data. Some people still see no serum DHT changes even at 30+ mg per week, while others see a complete nuking of serum DHT. The bigger question (for me) is efficacy. There's a big topical dutasteride study that is near publication and we hope to have more info then.

    As a androgenetic alopecia (AA) sufferer of over 30 years, I would dissuade anyone from using nonpharmaceutical solutions: they may be safer, but their real effectiveness is nil. Regarding the pharmaceuticals, the path should be finasteride-->dutasteride-->? (I'm assuming everyone will be on minoxidil simultaneously). I show a question mark because most AA patients will experience a decline in effectiveness of even the best anti-DHT medications over time. After treatment for 20 years or more, most of us will be at a loss of what to do next. More research is desperately needed.

      This is great advice. The problem with all natural interventions is that they lack long-term data. That doesn't necessarily mean that we should confuse the absence of evidence as evidence against those interventions, but we have to keep in mind the "known knowns" when navigating treatment options. For some people, natural DHT reducers work well enough to stave off AGA progression - at least in the short-to-medium term. But what about 5 years down the line? Or 10 years? We don't yet know, because the clinical studies don't yet exist (and they may never exist).
      There is also the phenomenon of waning treatment efficacy in some people on long-term 5-alpha reductase inhibitors. It doesn't happen to everyone, but it happens enough to suggest that other mechanisms end up taking over in AGA outside of 5-AR activity, or (just as likely) that endogenous factors begin regulating those drugs' efficacy long-term - such as the case with androgen deprivation (AD) and prostate cancer (where AD, over time, becomes ineffective). I agree that we desperately need more research.

    In my opinion the scalp massages reduce and stop hair loss just as well as propecia.

      @@mynutripartner I took Finasteride for 2 years but still experienced shedding and hair miniaturization. My hair looked thinner and my hairline receded even more. I decided to drop Finasteride and began doing scalp massages and was shocked by the outcome. My scalp felt amazing, I have stopped shedding and almost 3 months in I can see new hairs where my hairline use to be. So for me scalp massages reduced and stoped hair loss better than propecia.

      @@mynutripartner I went to several dermatologists and all diagnosed me with mpb. However, the possibility that they were all wrong exists. Finasteride does work, however it does not work for everyone, no medication does. People who suffer from mpb should give it a chance since it’s safe and effective but in my case it did nothing. I had clear signs for mpb such as family history, receding hairline and diffuse thinning in the crown. Blood tests also came back normal.

      @@mynutripartner you’re right that sticking with one is best and it’s what I’m currently doing. The first dermatologist I went to was in and out of the office in a matter of minutes, taking a quick look at my scalp, diagnosing me with mpb then told me to buy his hair loss shampoo and conditioner. The second dermatologist I seen was much more in depth and professional conducting proper tests then gave me the final diagnosis. They were also the one to first prescribe me Finasteride. The third dermatologist I seen wasn’t for my hair but for another issue I had in regards to my skin and while I was there I asked them to take a look at my scalp and hair loss and they gave me the same mpb diagnosis. Finally I moved out of town and had to find myself another dermatologist and that is the one I’m currently seeing.

    Red Clover is an excellent DHT blocker too. People seem to forget that one. Then there is licorice, spearmint, nettle, aloe vera, etc.

    Hello, I have side effects after finasteride, I used it for about half a year, I stopped taking the drug 3 years ago, I have further effects such as weak erection, failure to maintain an erection, scrotum not holding the volume (dangling testicles), high testicular temperature, what medications can help me? How can you increase the enzyme 5 alpha reductase?

    In my case, I tried saw palmetto. After about two weeks, I ended up with nose bleeds. I've since tried it a few more times at very low doses, and in all cases, the nose bleeds came back. So that one is out, unfortunately.

    Nizoral isn't going to do much. However, it helps a little. Dutasteride combined with minoxidil, micro-needling, zinc, eating pumpkin seeds (loaded with minerals and vitamins), are the way to go.
