    I'm a dental hygienist I think I have TOS, my physical medicine doctor dismissed me and said "get another job". I have pain under my left collarbone and left thumb. I feel so upset he disregarded me so quickly

      Sorry for the delayed response. Your job increases the risk of neurogenic thoracic outlet syndrome due to all the abnormal positions you must assume and then hold.     How can you tell the difference between Pectoralis minor syndrome and TOS? Also, i live in Hawaii and we don’t have the expertise here. How can I get an appt

      Hi, @mkirimit-
      There are no accepted diagnostic criteria for pectoralis minor syndrome PMS) at present. Some physicians will perform an injection of local anesthetic into the area of the pec minor muscle, and if symptoms are reduced, will declare that result as evidence of PMS. However, it is clear that the exact site of injection varies among different practitioners, and I have observed injections into the area of the brachial plexus, which would be expected to ALWAYS reduce symptoms. This would not be indicative of PMS.
      In addition, there is little imaging evidence supporting the current theory that compression of the brachial plexus suffers compression in the retropectoralis space, according to the medical literature and in my extensive experience.
      For these and other reasons, I would be cautious of making a firm diagnosis of PMS. Certainly, we know that some patients respond to anesthetic injections into the pec minor (when done correctly under imaging guidance). We also know that some patients respond to release of the pec minor tendon (first done by Jere Lord in the 1950s). There are plenty of other patients who do not respond. Thus, there is no direct and understood mechanism between these actions and the reduction of symptoms. Sometimes, docs suggest PMS when first rib resection fails, but often this is because they cannot think of another reason for recurrent symptoms. in fact, first rib resection is not a 100% successful operation, so we expect some patients will have recurrence.
      Our best advice for any patient is to see an experienced TOS specialist, preferably one who is part of a multidisciplinary team (meaning docs, chiropractors, Physical Therapists, radiologists, and others), given the complex nature of TOS. That team needs to fully assess the original diagnosis, the treatment(s) that have taken place so far, and the prior and current imaging data.

      @@tosmri Since then I've found the website and called many times and nobody picks up, left voice mail that has not been returned and used the contact web form multiple times without any response. I like the videos though. I will say I don't want a gadolinium injection and would like to avoid surgery and do physical therapy.
