Nice of you to be so kind to your 2 am caller! I'm a (veterinary) general practitioner and always appreciate how gracious many specialists are when calling for advice.
As a veterinarian’s wife, I was thinking I’m sure his wife is looking forward to undisturbed nights as well.
My spouse graduated in 1991 and was employed by a large mixed animal practice in a time before cellphones were as common as they are now. “Being on-call” meant that I had to be home at 5:00 pm until the office opened again at 7:00 am, because if my spouse was out on a farm looking at a cow, he couldn’t answer the phone.
1 night a week, 1 weekend out of 6.
We later moved to a smaller, large animal only, practice. Every 3rd weekend, and I forget how the week nights were assigned.
He is now a solo practitioner doing nearly 100% swine only. Very few after hours exams. 😊
It confuses me as to why hospitals put so many hours onto doctors and nurses. How can they possibly do their best work with such a lack of sleep? You sound like an amazing doctor. Your patience and kindness should be appreciated by the hospital and all the patients that you serve.
Same, like do I want to put my life in the hands of someone who is sleep deprived? No thank you. Unfortunately we don't have a choice.
Currently limping my way home post call and appreciating this sentiment.😊 Lol. We do our best despite not being necessarily at our best. Things are changing but it's slow.
@@Auric-BraiNerd doing your best is all anyone could ask for. Most people appreciate the dedication of healthcare workers, but the loud minority likes to complain.
Getting through med school is really hard so there's not enough people to staff ideally. Much of medical school (especially for surgeons) is also repetition so that you can do what you need to do from muscle memory when you're exhausted
@@stephenchurch1784exactly!! all of the data is that the only thing that benefits from that repetition Is surgical procedures And even then you just need to get a certain number of each kind and then the benefit to further experience plateaus. Then the argument about exposure improving with more time working has been also studied and experience with rare presentations or outcomes goes up but retention of those events goes down when tired. So you're not actually remembering the interesting cases anyway. The amount of textbook reading done by the average residence goes up with more free time and opportunity to sleep. There was actually a study that correlated the number of missed important family/friend events with worst performance on subject matter exams. I.e. Being able to attend your sister's wedding helps you be a better doctor.
The other thing is that medical school is a bottleneck but it's not the biggest one at the moment. Insufficient residency spots is currently more of a problem because there are plenty of great medical students who are not getting residencies. Since residencies are subsidized by congressional funds they need to approve more money to allow for more spots. That would allow happier healthier training physicians as the workload would be split. When you're on service you still learn a lot from your co-residents cases during rounds. Sharing the burden does not really prevent you from getting experience
My sister would love to have you as a consult call, as she's told horror stories of having to call doctors at night (even the on call doctor) and just getting berated (even cursed at). They actually desperately try to avoid calling the cardiologists, as apparently those were the worst at her facility.

Congrats on finishing strong! I’m on call once a month for the medical engineers team (medical equipment repairs) and I dread it every time. You’re very right though, you have to be calm and courteous because they are calling you for help! Congrats again!🎉

Congratulations with being done with nigh cal! Whenever I was on call overnight, I had a hard time sleeping because I was anticipating the phone ringing.
Retired G.I. nurse here. Congrats on finishing your fellowship. As for Call number 6, your on call team thanks you for not calling them in. Working nurse jobs that required being on call for 20 years, I understand your happiness for being off call for three months. I wish you well as you begin you career as an attending physician.

My mom took call most of my life. She’s an ultrasound tech and I often had to go with her in the middle of the night for ER stuff. It meant she couldn’t go to the movies in case someone called her in, and was pretty stressful overall. She made better money on call but it was a big thing to deal with for her and me. “I can’t, I’m on call!” Is something I heard countless times!!
Your place of employment was OK with you being taken into work with your mom? Even at night? No judgment, but we had a retail manager where I worked who would bring her young son into work overnight with her. It's not the same situation, granted, but still. She got monitored by child services for a few months and was forced to go to a workaholic's anonymous group for a while. Not judging at all, just curious. Also, my doctor from about 20 years ago who was a single mom also brought her young kids into the er when she worked shifts there. People were surprised, but she wasn't reprimanded. I was a single mom as well, so I understand the challenges. Luckily, my mom was always available to help, as we lived together. I know I was lucky to have a mom like that who lived with me!
@@michelelein9094 babysitters sadly don't work on an on-call basis. When my mom got called in, she'd drop me off with a friend if it was a reasonable hour or drop me off with pediatrics if it wasn't
I’m a clinical laboratory scientist and work evenings and nights and I hate when I get a outpatient specimen with a critical value and it doesn’t get ran until late (after 8pm) and then have to call the on call doctor…. I had to page a Hem/Onc fellow tonight around 2200 about a platelet count and felt bad because I know I woke them up even though I called as soon as I got the result because I didn’t want to end up calling after 2300 or have the next shift call literally after midnight…. I try to take into account the residents/ fellows needing sleep
“Until then I’m going to sleep well” ….with baby crying in background 😂
Now that is commitment and dedication! Huge commemoration and appreciation!

If I was on call at night i would just stay at the hospital - cant be anything worse than being woken up from a cozy sleep, then still have to drive there! Thank goodness my husband is not a doctor and doesn't have to be on call at night - he crashes at 9pm, and quite literally the house can fall down around his ears and he won't wake up! I think its from being in the army in active combat for 2 years when he was young. But even if you manage to wake him up, the glazed eyes and the stagger and not being able to string two sensible words besides "huh" together, and a bit of grunting, would put any patient in grave danger😂😂

I work as a nurse triager after hours for providers from our entire regional hospital system. Of all the provider guidelines I follow, our GI’s are the most intricate and specific. They take NO outpatient after hours calls and even consult calls have fine tuned criteria.

Thank you for being kind on calls! I used to be a nursing student and came from a school that took a lot of pride in its nursing program so I’ve met a lot of nurses/nurse to be. I’ve heard some horror stories. From a patient perspective I also appreciate it because I don’t want people to be scared to call the specialists if I need something and they have no clue what to do about it! Medicine is already lacking a lot of compassion just between staff and I love that there are those able to remember that you’re all just trying to make sure the patient is able to walk out of the hospital on their own two feet as much as possible! Being snippy once in awhile is understandable, but I’m glad you’re still doing what you can. On behalf of the patients, thank you for being a dependable doctor and making sure that an expert is there when things go sideways! (Side note: I have gastroparesis and so I triple extra love the GI folks because I’m only able to eat with even a shred of freedom because of what you all do. Thank you bunches!!!)

Glad you were patient and kind about the 2am call and actually acknowledged that they are not specialists and didn't know it was nothing serious. I have heard some on call Doctors can be jerks about being called it the middle of tye night.

Even an intense fellowship experience is often easier than being a specialty/subspecialty attending on call at a large, busy hospital.

Congrats on being able to get a few nights of uninterrupted sleep. I like the fact that you are kind to not only the patients, but also to other health professionals. That reminds me of the old fashioned doctors from the 1970 when it was about the patient and not about the money.
Wish all doctors were like you. The last hospital I worked in was an LTAC. This type of hospital service was new to the community and one doc who was not very nice anyway said we would be closed down in 2 years. Over 12 years later the hospital was still there. They did end up closing but that’s because another hospital in the area whi liked to play the one up game waited in the wings to see how we made out. We took alk the criticism-and boy was there a lot-we listened to the doctors harping, every fistful of mud that could be slung, flew in out direction. Finally they got all their ducks jn a row, opened up their own LTAC, and whoever makes these decisions decided not to keep us. Plus the hospital we were in was being torn down and there wasn’t a place for us in the new hospital !
A couple of years ago my hemoglobin count came back at 1.5 after I collapsed and went to the emergency room by ambulance. I was severely anemic and was in the hospital for ten days and had blood transfusions everyday. I still have to have iron infusions at the level 1 trauma hospital that's about sixty miles from my small town.
Surprised that labs are working after hours!

A question I've always wondered: why does the frequency of call shifts decrease in latter years of residency and fellowship? Wouldn't hospitals want the more experienced residents/fellows available for urgent consultation rather than the less experienced residents/fellow who wouldn't have their attendings available for guidance if needed?
You learn the most by being on call so it’s helpful to start off with it. And older fellows don’t want to have to do it anymore haha. An attending has to be available to supervise regardless so that’s honestly not a huge factor
his loud is your ringer so that you don’t sleep through a call night?
Do the residents get the call before you since they are in the physically in the hospital?
I do not know what a PPI is and I don't think Google is going to help. That's a question.
Thanks for the rest of this because although I'm not all that interested in GI and I'm never going to be a doctor it was nice to listen to somebody talk about medical things instead of how they put their makeup on and what they take for lunch to work as I have seen in the past from some residents, not fellows. I used to be interested but then I discovered there was no medical content at all. Even if they made it up they didn't bother to make it up. And I don't care about their lifestyle. So I just stuck to the rheumatology fellow up in Canada because she always had details. And now she is either an attending or at least the Canadian version of board certified and taking some time off to work in other areas and see other things before settling down somewhere.
In the meantime I've been following some people in Ukraine and Poland and No Longer In Russia.
And some doctors who react to medical drama. Or scrubs. And some ducks
Well in a GI context, a PPI is normally a proton pump inhibitor if you’re talking about a medication. It works by preventing your stomach cells from pumping the stomach acid out of the cells and into the space that is your stomach. That raises your pH of the stomach acid, which means it’s less acidic, so it’s a more serious way of treating something like heart burn or stomach acid that’s too acidic than just taking some over the counter antacid tablets.
Are medical Fellows required to live in hospital housing, or close to the hospital they work at, so they can be close by in case there is an emergency call that requires them to go into the hospital in the middle of the night? I tried to google this but didnt find a quick answer. I never thought about this until I started watching these videos. I love all of the Doc Schmidt videos. So funny and informative. I was a CNA but never got answers to questions like this because we CNAs never associated with the doctors or nurses we worked with.

You guys are way too busy compared to us here in Australia. We medical registrars are awake all night receiving all the phone calls for all specialities and only we call the fellows or consultants if we're worried. We're rostered on the night shift though, so don't need to come in in the morning.
