I had a female patient in the ED who apologized multiple times for "probably making a big deal out of nothing" when she had actual gallstone pancreatitis.

This is my grandma all over. In her mid-seventies, she got into a motorcycle accident and refused to take any painkillers for her injuries, only paracetamol because she worries it might be too much for her body. She also had a hip replacement and still has the morphine she never used in her cabinet. She is one strong lady 👵💪❤
What the heck is Paracetamol supposed to do? It's basically liver poison and nothing else yet people keep taking it
That's a HIGH pain tolerance being discussed intetchangably with a LOW pain tolerance. Script is all screwed up.

Doctors really just ignore certain patients. I think some medical literature still says things like "black people feel less pain"

Yeah, I get this and the opposite both. In addition, no one knows what I have and no one knows how to deal with it. I have a rare connective tissue disease which results in joint dislocations, fragile skin, spinal problems, and a number of other issues. Doctors refuse to give me the tests I ask for saying they "don't think it's that" until after doing tests I have already done (CT scans, MRIs, EEGs, EKGs, etc) which don't come back positive and haven't for... idk the 6 or 7 years I've been trying to figure it out? I only learned my disease existed from someone else who had it, and now 4 of my doctors agree that I have this disease, but insurance won't cover the genetic testing for it despite multiple recommendations (and some of the tests diagnosing diseases that could spontaneously end someone's life...). The more doctors I see, the more complicated it gets, the more "I have never seen this before"'s I get, or "Maybe it's just anxiety or depression. You *are* diagnosed with those" Meanwhile, my therapist: "There is no way this is linked solely to the state of your mental health, since it seems to be a root CAUSE of your depeesssion and fatigue" And man, it is exhausting. I've dealt with recurrent dislocations and extensive widespread chronic pain since I was very young. Nobody believes these issues have been happening so long or that they're gettung worse. That, and even for the things I have diagnoses for-a hernia and scoliosis? No one will give me anything to manage the pain and issues those cause, and my hernia causes excessive GERD, which famotidine/pepcid has not helped even a little so... ? Oh and don't even get me started on my allergies man. Point is, it's extremely difficult working through the American healthcare system. They'll say "physical therapy will fix it!" But it makes it worse because physical therapy is often focused a lot on stretches which dislocate my joints. It can help if the person knows how to work with excessive hypermobility and easy dislocations, but honestly it's so difficult to find someone who knows how to help you find those limits. I want to find the root cause BECAUSE I've tried so many meds and treatments and none of it has worked. Pain meds don't work, a lot of meds cause a blood pressure drop and mine is extremely erratic- Just tough tbh. Hoping it improves soon
I wish more ppl in the medical profession understood this!! I volunteer with an animal rescue and several years ago I was injured trying to rescue a cat from a trash compactor. Both of my arms had multiple very deep bites on them. When I finally got to triage the guy acted like I was being rude when I told him he couldn't put the blood pressure cuff on my arms. (Both were in severe pain and bleeding) he said that he couldn't proceed without doing my blood pressure and there was no other option than my arms. He documented I refused treatment (that's triage not treatment...) and sent me back to the waiting room. 4 hours later we decided to go to another er in a different town. Since he put refused treatment I was taken off of the list to be seen and wasn't told so. Also the other er was able to take my blood pressure on my leg. They said it wasn't preferred because it wasn't very accurate but since I was young and otherwise healthy it was ok to use.
Yes get paid more for more work, but now incentivizing polyps removal will all of a sudden cause for more polyps to be suddenly diagnosed…. Not all doctors are as fair/honest as you Doc…. Same could be said with all other professions…. And that is why all other professions suffer from the same issue you just mentioned.
I mean, most people get paid the same regardless of how much work they do on a given day

Oh goodness this isn't even the only example of this in the medical community. Honestly though I think the true reason things like this tend to be down the way they are is because if you build them differently insurance would start making you be more scrutinous about what you do and don't remove and they'd be more scrutinous about what they cover.

I went in for unexplained weight loss. I had the same job, eating habits, no exercise at all. I went from a size 10 to 0 in 6 months. I couldn’t talk with friends or family they were like “that’s a good problem to have!” But I was concerned I wouldn’t stop loosing weight. Also I was getting so thin I looked sick. He did blood work and everything came back ok. He had nothing to suggest so I just kept on loosing weight to around 100lbs. At 5’4 my weight should be around 105 but that’s way too thin for me. I haven’t been less than 120 since high school. It was pre menopausal related. I had always been thicker than my mom or sister even after giving birth to 2 babies they went right back to their normal weight. I had to try to loose mine. Well they got heavy during menopause and I got thin. Weird.
This is so true! When I was on call as a Gynae junior doctor, I once had a GP who called me to ask which antihypertensive he can give to a breastfeeding woman. I was like What!? That’s something you can look up in guidelines 😅🤧🤣

How did you come to that conclusion?
We should incentivize you for reducing the number of potential cancers (polyps) provided you do not introduce any complications, not simply and solely for doing more work. Work that results in medical errors should not only not be compensated, it should be penalized. So yes, you should get more money for taking out more polyps and even more of those polyps tuen out to be cancerous, but if your actions cause patient injury that would not have occurred but for your actions, you should not only receive nothing for that procedure, you should have to pay for all of the damage you caused. Results, not work, is what matters.
Most surgeons get paid an agreed monthly amount, just not in your country.

My husband is a retired dermatologist. And, this example is the most relevant in dermatology. People never have just one skin tag, or one mole, or even one cyst. A person comes in to see you, and they want everything done. But there’s no reimbursement for that. So, he would tell his patients that their insurance only covers 3 at a time (true) even if they have 100 to do; they have to come back in three months. Because my husband was one of the last honest doctors; he really made no money at the end of his career. He retired because he refused to play the insurance game over patient care. He did what he thought was correct and often didn’t take payment for his time and hard work. In the end, he made nothing when he sold his practice. And, I’m not talking about a small practice. He had over 20 employees, was a cosmetic dermatologist with lasers. He also did hair transplants and won best small business in 2005. But in the end, he lost. After 40 years of practice, he has essentially nothing to show for it. COVID didn’t help. Neither did his divorce. We basically live social security check to social security check. Doctors are no longer rich. People should know this.
If you're really, REALLY hurting and have no other liver/kidney related health issues, the answer of what to take might be both. The effects do stack without a huge uptick in risk. I've had seriously nasty migraines that wouldn't budge with nearly anything I tried until I hit them with this combo.

they should make it illegal to accuse patients of "Lying" without running thorough tests to prove it. So many people are dying and hurt due to these lazy egotistical docs
But the pt. would have injected the med. only once since there was no resulting euphoria. It was a good show. Early on they made mistakes but there seemed to be far fewer as the seasons passed (or I was too rusty to notice them).

I had RNY gastric bypass. The doctor had to move my liver and gallbladder out of the way to do the procedure. 4 months later, I had to have my gallbladder removed. They admitted that it was probably bad when I had my first surgery, but if they did both procedures at once, they’d only be paid for the primary procedure. So I had to be sick for 4 additional months and have the risk of a second surgery because of insurance reimbursement.
