    Hard to believe first, that once they determined their nav was not calibrated they did not simply use ground radioed vectors, and then, once the plane bank angle warning blasts them, they still do not fly the plane...simply incompetence..

    They panicked over a technical error that wasn’t a threat to keeping the plane in the air and instead of using simple alternatives to get back going the right direction, they try to fix a problem. Aviate, navigate, communicate. These pilots were also clearly undertrained (edit: I wrote that before I even go to the part which actually described their lack of training).

    Incompetent, unfit to fly, inadequate training, no Crew Resource Management. Criminally bad training, maintenance and other practices by the airline. The list goes on and on.
    Why do airline training and maintenance procedures not have to be approved by the manufacturer and national aviation authorities?

    When you fly as an FO, there are some pretty shitty captains out there! Your job is not to get into their vibe, be assertive and maintain the golden aviation rule: aviate, navigate, communicate. But I can see the figure of authority of a captain, in certain cultures, is pretty intimidating! Be assertive!

    Although there were many contributing factors, the incompetence of both pilots is unbelievable. After the chime that the A/P disconnected surely you would expect one of them to take control of the plane first.
    This reminds me of Eastern Airlines flight 401, were both pilots also were pre occupied with a problem and 'forgot' to monitor/fly the plane.

    The CVR recording has circulated over the internet some years back. It happened in my country & the CVR was in my language. Very eerie to hear (whether the recording was original or not I don't know). Great work 3 green!

      It's valid, it was initially denied by the goverment, but everbody knows it was valid, somehow it was leaked during the investigations.
      There's no way someone could fake that back in 2007.

    Even a PPL Private Pilot with less than 50 hours total flight time would have just flown a steady course, then cross checked their IRS Heading Displays with THE STANDBY COMPASS which was right in front of them. Then selected the corresponding IRS switch to the correct side. It’s also displayed in the FMC-IRS selection! Absolute Basics for a Boeing 737-400 Pilot. As I was for five years.

      It's almost insulting to call it basics to just keep wings level. Everything else is one step of and indeed basics, but I feel you could place near anybody behind a modern plane in cruise and they'd be able to keep it level with working equipment.

    A plane in IMC can go into a death spiral in a few short seconds. The nose increasingly pitches down and the bank angle gets steeper, so the plane is gaining speed very quickly as it dives towards the ground. In a very few seconds the plane is going too fast to recover and breaks apart.

    Just found your channel and it is really good content overall. I hope you will post more videos in the future.
    As you said in this one, the captain not taking care of the plane as soon as it starts to turn seems so bizarre. I don't know anything about flying a plane and everything related to it though. It is still very interesting to see how the quality of the communcation between the first officer and the captain is crucial in these moments. I always thought they would always agree with one another but it seems not to be the case in some/most of your videos.
    Could you write the meaning of the acronyms the first time you use them please? I am getting a bit lost sometimes.

    I enjoyed the video and your channel! Just a couple notes
    - Not sure how corruption was involved in the crash because the embezzlement seems separate from the actual crash. Seems more like incompetence on both the part of the pilots and airline.
    - You say the first officer tries to pull out of the dive at 9:38, but it seems more like the captain was the one in control since the first officer is the one saying "Pull up". The accident report does not state which pilot was in control.

      You are right that corruption wasn't the biggest factor in this crash but it wasn't there and I felt like it related in terms of the lack of maintenance, etc. (fair enough if you disagree)
      And looking back at my notes, it does appear that Widodo (the captain) was the pilot flying so that line is my mistake. Thanks for letting me know.

    9:41 How does a plane “gradually” get ALMOST COMPLETELY UPSIDE DOWN before the pilots realize what’s going on? How does a pilot get that TASK SATURATED and distracted? 😳

      Thanks for the comment mate. So the human body is actually not very good at orientating itself when it can't see the horizon. It can tell using the vestibular system. Which are tubes in your ear which can detect acceleration. But they work by detecting the movement of fluid over tiny hairs inside the tubes. So if the movement is very gradual, you can still feel as though your orientation hasn't changed. Thanks for watching and hope my explanation wasn't too confusing :)

      Spacial disorientation and Somatogravic illusion have been one of - if the largest - causal or contributing factors in crashes of both general and commercial airplane over the last 25 years. Your admittedly normal line of thinking (that you'd of course feel it if you went upside-down) is why it's so particularly dangerous and insidious. If I may, I would suggest you look up the crash of Atlas 3571 (an Amazon-chartered B767) though I could give you many other examples where such a phenomenon has caused the loss of life and aircraft. Spacial disorientation and somatogravic illusion are no joke.

      @@Mr.Chris.P.Bacon1 when they turn you around and you are busy flying it is easy ,pilots in clouds or at night absolutly need those instruments to stay level,kennedy also get disorientated and lost his life flying a plane from miami to newyork ,at night over the sea (i believe)he lost all refrence and crasched the plane in the sea.

    Great video! It was difficult to watch. The first order of business- "maintain aircraft control" was neglected. With disagreeing IRSs in radar coverage simply have the controller vector you to the airway and call out centerline. Then the malfunctioning IRS(s) can be identified. If one is reliable, use it for NAV and the other as backup attitude reference. Back in my C-141 days we had 2 INSs. No FMS- the Inertial Navigation System was loaded with the waypoints and did the work. Occasionally on long ocean crossings INS drift would trigger a master caution and the "INS 10NM DIFF" light. That was never a surprise as we monitored INS performance enroute. We would reference the INS performance log to see if one outperformed the other. After each flight we performed a procedure to determine INS drift. If it was day VMC and we could positively identify an aircraft flying the same track that was an important clue. Lastly, we could use the weather radar which had a cursor. We could "paint" a prominent coastal feature such as a cape or point and see how each INS labeled the lat/long. This was possible within 200 miles and give us time to correct our track before entering the radar environment. Regardless, one pilot flies while the other troubleshoots.

    Isn't it so frustrating that we never evolved to fly (biologically)? And so few of us spend any time piloting that we probably never will. We wouldn't have these problems if we'd descended from birds... or bats! 🦅

    WAIT, I thought when they rebooted the system their attitude indicator would be out for 30sec, so how could either pilot notice the increasing bank angle w/out the AI?? & how could the 1st officer roll the wings level w/out a working AI?? That's likely why he pitched up b/c he had no way of knowing how to get the wings level. In a situation like that, w/out external reference, the instinct is to PULL UP, which he did. He likely had no idea they were inverted. The issue was the inability to keep the wings level while the system rebooted.

    This video reminds of a couple other videos I’ve watched where an aircraft bank angle warning is seemingly ignored both both pilots. Wouldn’t the very first and automatic response be to look at the attitude indicator to determine which direction to bring the wings back to level to the horizon?

    It is quite a horror to hear the CVR in original language. Things happen in seconds.
    When the righ IRS was rebooted, they would have still visual indicators on the left, right? Quite puzzling why they didn't simply level the wings. As per the report they did this twice very briefly only.

    When you watch this type of video, I always think ‘how in earth did they not see the problem?’.
    However, I bet up in the flight deck it’d be easy to notice why. RIP to all on board.

    I don't see corruption having to do anything with this. I see Adam Air being incompetent and not wanting to spend money on maintenance. Pilot interviews found some bad stuff about how the airline treated pilots. On the other hand, pilots also had problems, as proved by Adam Air Flight 172.

    This accident makes me sick. Poor maintenance. Trying to save on maintenance costs. Pilfering by top management owners. To operate an airline business is very expensive. Business people who made large amounts of money on inflated real estate prices attempt to operate an airline and fail. These owners never learned how to develop a manufactured product and the knowledge and skill it takes to operate a real company. These type of owners previously made money from speculation.

    Could they not have deferred dealing with the IRS until landing at the destination airport? Especially if they were not in clear sky and able to reset the system while using visual references to continue to fly the plane? True, had they been concentrated on flying first this accident would not have happened and they had become overly concentrated in the fault, but I am curious would this not have been an issue a pilot under sop would determine could be deferred until landing?

      Pilots say 3 greens, means all 3 landing gears are down and locked. It's shown with 3 green lights on the dash. Anything else is an emergency landing.

    You might want to consider the inaccuracy of how ATC and pilots really communicate. Clearly the voice-overs were read from a transcript because, for one thing, a heading like 070 is not pronounce OH SEVEN OH. It would only be spoken as ZERO SEVEN ZERO.

    737 problem lost control turn left or turn right or pitch down, just like your car skates on black ice from last night and still snowing on top of it. The best hope is don't do anything for 1-2 minutes.

    Pilots missed entirely the priority required for safe flight. Really simple ! Also failure apparently to refer to other instruments, and waited to long to request their position.

    GPS anyone??? Magnetic compass??? IIRC, once you shut the IRS down, you can't realign it in flight; the aircraft has to be parked.

    You know I will not fly at all because I have no wish to get my pilot's license. But here lately thanks to all of the these flying video's,,,i have been reconsidering that notion,,here's why,,,now I have been watching all these recreations of accidents of commercial passenger jets and I am glad I have and never will fly commercial and heres why,,, until the idiots in the FAA make having at least three analog instruments in the cabin of one if those fly by A.I. , wire and fire birds, I think every damn one should be grounded until the is a analog speed meter, and a heading compass and bubble level....they would lighten the stress load tremendously in critical situations with instrument failure of any sorts, them might not be perfect, but I guarantee after the last five years or little more of watching all these videos, I bet a good ,60% or more would never of happened, those pilots are forced to trust and depend way too much in technology, when every damn one of us barely trust the crap in our own homes...they need a back up navigation procedure in those little books they have to read instead of flying that damn plane..don't take me wrong i know why, and all aspects as to the reasons why they are essential, and have improved safety and procedural processes, but damn there are time when it just got to be done by the good ol seat of the britches and skill,,,, so if I ever need to fly anywhere, I will fly myself thank you very much....and I will rest my two cents worth.....be safe everybody...!
