Yes, that was very helpful! The only thing I can think of adding (in which my school would scold me for) was not checking the head for bumps, looking for lines (of course she doesn't have any here, but you get the idea), and checking the thyroid/lymph nodes.
I get really shy and mousy at Clinicals when I have to meet the patient for the first time while the nurse I'm shadowing is doing something else and I haven't built that rapport yet, so I hope I can get some more practice in so I can do this without looking/feeling/being so awkward. lol
This video is an example of real world nursing.
One thing that is assumed in this video is the pt has clothes on under the gown and every pt will be ok with you pulling down their gown to listen and and ordering them to flip on their side and opening up their gown in the back to look at skin, definitely need to ask permission before you go pulling their gown, up or down.
Any issue with your head, neck, or ears? Patient goes into 15-20 minutes story of past medical problems LOL

Just one thing, I think it’s better if nurse do a set of vital signs first, and ask if there is any pain before doing head to toe assessment.
I totally agree... but when vitals machines are limited and being used this head to toe can be done while waiting too. And yes, obviously to ask: Chest pain, SOB, general pain, Nausea/vomiting right off the bat.
We're told to always start with pain questions so you don't make the pt wait for pain mgmt meds.

Where I work the CNA gets vitals then the nurse comes in for an assessment.
A part of your job can be delegated and it was in NCLEX.

Germany laughing rn. Our legal limit is 10, realistically at weekends like 16. And no nursing aides or anything like that lmao

Thanks.. I really needed this. I have utilized this flow of the head to toe this week in clinical with much success. Most importantly, this gave me some much needed confidence in that first introductory moment with the client- yeah, you know the one where you establish credibility and rapport. Thank you SO much. xo

I am a nurse assistant and I do see things like this in the patients. I see their feet, their back, I assess them but out of mere curiosity. I feel like a scientist when I see this stuff. I feel a registered nurse is a social and analytical profession. I really like Nursing so much but I am very sensitive to even study in a loud room. I need quiet room. I wish I could be a home health Registered nurse. :)
This is probably the best time to be in school for you because everything is online and working from home :) Great way to view it. I used to be a CNA and honestly loved it. The connections I made with my patients was very rewarding. I think if you wanted to pursue it, you'd make a great RN as well.

Wow, hope I can be that fast with my H2T assessment one day. Aside from not having time for longer assessments, I always feel that the patient must get annoyed being poked and prodded for 20, 30 minutes.

You're supposed to explain things as simply as possible. It's not condescending, it's to make people understand easier.
