First, you can check the respective website for each university to which you are applying, which will present their specific requirements. Most will utilize the Common App, while others will use the Coalition Application, or their own systems.
After creating an account, you can fill out the application, which will ask you about your course history at university, as well as a few essay questions. Most universities will require you to:
1) Explain why you wish to leave your current school
2) Discuss why you hope to join the one you’ve selected
There may also be some additional essays, such as those involving your community involvement or experience in overcoming a challenge.

Unfortunately, most awards are for U.S. citizens and permanent residents only. However, there are need-based grants available for international students who are within one semester of graduation. You can find information about the grant and its application here:
