    For all the new interns, just remember, every bad thing you are afraid of will 100% happen to you this year. While scary now, later on that realization will become liberating.

    Yup, that's how it goes. My first day as intern was a 24h-shift in a big university hospital. My computer log-in didn't work, my badge didn't work, and I slept on a chair in the corridor because I couldn't find my room... Thankfully the ICU resident who was on duty that night was super nice and told me I could call him anytime if I needed help. Needless to say I woke him up several times in a panic because I didn't know what to do with my patients... Cried a lot that night. Not exactly the best experience I've had on the job ^^"

      Oh wow. You cried. I could be wrong, but I'm guessing you never worked in a fastfood restaurant. It's the same thing except no one is going to die, but there's a good chance that if you mess up bad enough you will get fired.

      @Nerd Absolutely. As A Student nurse end of my first year( Many Years Ago) Put in charge of 12 sick patients.. We were short Staffed… On a 12 hour Night Shift.., It’s Gobsmackingly Terrifying to know one is responsible for these People’s lives… !!!! In my Country we Still DO NOT have intercoms or load speakers Making regular Announcements For Doctors who are on call for emergencies…
      We had to try track them down with Pagers…. No cell phones in those days… And Many times they were Sooo Grumpy.. Looking back understand they were on call for very much longer… Than our 7 days of Night Shift… 12 hour Shifts…

      Lol, it was like my first day of nursing clinical. I Pressed the "nurse assist" button and all the nurses came running into the room. Apparently, the button is used as a "mini code". It was mortifying lol.

    My first job as a graduate RN took me off orientation way early because someone called out. They asked me if I had an issue with taking the sick nurse's assignment. I was terrified but said yes because I didn't want to be fired. Just like that I was given 24 patients. I was not in complete control of everything around me

    Another appropriate expectations vs reality skit: expecting to see lots of interesting patients with valid inpatient medical issues, but then mostly dealing with patients waiting for weeks on end for placement to rehab and nursing homes.

      Ugh, then finally getting everything done, only the the insurance to declare they're now too recovered to go to rehab so you have to start from scratch to get them home, delaying everything for another three days or so.

    Mr. Jones' order for Dilaudid was accurate. I kid you not, some of my patients on narcotics had timers for their medications and press the call light incessantly leading up to that time. I saw most nurses try to punish said patients by taking their sweet time, then I met an RN who had her own timer and beat them to the punch. Said patients had zero problems thereafter.

      Because the attitude is that ANYONE taking opioids for pain are drug abusers. I live on less than what manages my pain so damn straight I’m on top of it when I can get my next dose. I hope those who deny pain patients their meds based on personal biases have to deal with untreated chronic pain in the future. Sorry not sorry.

      @@jeanetteasmyself442 Exactly! It's hard enough to deal with legitimate pain but those who judge other in pain have no idea what they're going through. Plus, that should be between me and my doctor. The nurse should not be giving her opinion on stuff like that cuz it's not her health. Also long as there is no valid reason for serious concerns.

      A nurse told me I needed to get all off the pain meds cuz I'm gonna get addicted, right after having a huge multiple level spinal fusion surgery. That is ranked as one of top 2 or 3 medical procedures or pains to have done. I've been diligently and compliant on taking pain meds for 20+yrs. This nurse knew for for only an hour at that point. When I told my surgeon that, he was pissed that she stuck her nose where it didn't belong and had a "conversation" with her. And he's a really sweet, nice doctor too.

      @@CynthiasTikka Even if it’s legit health concerns, those issues were discussed with me and my doctor and we did a cost risk analysis. Anyone, and I mean anyone, including the CDC with their bullshit, is not privy to that conversation and deserves no input. It’s between my doctor and myself alone.

    On my first dayJuly 1,1994, the pagers didn’t have displays. They just had a static sound followed by the page operator announcing what number to call. Sometime overnight I slept through a page and woke to the pager still vibrating on the table. I called the operator who didn’t know who I was. I called around the hospital. Then I walked all over the hospital, basically making rounds again to try to find out who paged me. I became hysterical. By morning report I still didn’t know, but I was very prepared. It was a long time before I could even doze off on call again.

      @@carolinevillemaire4339 i an of the Palm pilot generation but my first weeks I had a backpack full of reference textbooks. Nurses madr fun of me but everything was in there. Uprodate is a godsend

      @@carolinevillemaire4339 back in India we had to depend on clinical history, exam and X rays for most diagnosis in 1994. We looked up meds dosages in a drug manual. We did well. I think technology crippled our clinical skills and judgement. Now a radiologist practicing north America, things are completely different. Its changed in India too probably.

    I'm an MA in Family Medicine who recently transitioned to Care Coordination. I worked the clinic floor for years. Love working with our Residents, especially our Interns. They're so young and eager...and have a lot to learn. It's fun teaching them the ins and outs of how things really work and watching them grow in their profession.

    Yeah, the July 1 glow of being a newly minted doctor and the reality is a wake-up call. Worse though was January. Days were dark. Depression was real. In to the hospital in the dark, home after dark. Sixish months done, and too damn many to go.

    I am a physician who qualified 1992. I can relate so much. I still get nightmares. I hear them shouting "doctor... doctor- was so over being a doctor in one day - and when I gave my beeper back a year later - it was an amazing freedom. We would work 29hours on end without sleep. It was rough.

      That is literally what I thought as I read through some of the comments.. “all that stress that must give some PTSD!!” It’s pretty inhumane in my opinion, I highly doubt that the overwhelming stress is necessary. Glad you’re where you want to be now :)

      I agree with you 100%. How do they expect doctors and nurses to do their jobs right when so much responsibilities are on their shoulders. Guess what happens? Jobs are left undone, patients do not get their needs met as they should, the doctors and nurses are stressed out and can't perform well. This needs to stop, for real. This is causing many healthcare workers to leave the profession, especially those who have passion for it.

      Actually this is every new intern’s reality on day one. Your pager goes off 3 times in 3 minutes, you are covering a huge list of patients, you are on call overnight covering the entire floor of patients you are not familiar with (because they are not your patients, you are just covering overnight), and also you have to see patients in ER whilst covering the already admitted patients. Even a call as simple as “patient requesting sleep medication” can be an ordeal because you’d have to review the patient and the chart to ensure there are no contraindications. Yup. This is pretty accurate and pretty universal experience.

    My first personal experience with Dilaudid was during the process of clearing a steinstrassa. I broke the tablet into quarters, put 1/4 under tongue to dissolve and swallowed the remainder. Got relief in a minute or so and it would last 3hr. Always tried to extend the 3 hr. to see whether the pain recurred and sometimes I made it an additional hr. and sometimes I didn't. Then it was like throwing a switch: No pain one minute and a minute later severe pain and the process repeated for 3 days. I knew it would happen exactly that way but when it was happening to me, I found it really interesting. It's just like the books say: You still know you're having pain but you don't care...you're comfortable.

    As someone near the end of her intern year, here's my message for the new interns: you *WILL* f*ck up. You will get yelled at and punished for your mistakes and others. It's an stressfull year, but it's also the year of the most learning your will ever do, you get to make new friends with the whole hospital staff (from the cleaning lad to your attending), find out what specialty you would like to go, truly live a doctor's life before having the complete responsability of it, connect with your patients. Just read and read again, but don't forget to sleep and go out with your friends and collegues. This year has been a blast for me, I hope yours is 10 times better.

    Just past the 6 month mark as an RN. Started just before omnicron. I'm finally (within the last 2 weeks) feeling mostly on top of things and can look back at the first 3 months and finally see myself in this. So. Many things to juggle and so many medically unstable people I just didn't know what to do with. Living on adrenaline was not fun.

    First night as an intern: Code blue on a non-teaching pt. who weighed at least 450 lbs. lying on 2 beds they had bolted together. Had to climb up on top of him to do CPR. Vigorous compression produced no pulse. Put a needle in his pericardial space and sucked out 100 cc of clear, pale yellow fluid. Got a pulse with compressions but couldn't restore a productive rhythm. Traumatic beginning for the journey.

    I still remember I was so excited on my first intern day that I arrived at the hospital half an hour early, only to find the on call recident was charting, and he said "wow, you come so early", and I did NOTHING that day because the doctor was on holiday.😅

    This may be cleverly relatable to those in the medical industry and it's done so very well, but from a patient's point of view it's frightening to think how stressed and rushed the whole system is, not even taking into account the whole administrative side, insurance, private equity, benefit managers, etc. It's a colossal mess. When will it ever be corrected? Will be always accept this messed up system?

    Intern walked into my room and started telling me about surgical procedures that the hospital used that were not factually true.     More like expectation before 4 years of undergrad as a pre-med vs after all the clinical hours you need to even get into med school 😅 Did some shifts in a level 1 trauma center to get my EMT certification and it was barely organized chaos, doing everything I could to help the nurses, and getting yelled at by patients. In a weird way, I loved it though, and that's why I'm still premed 🤷🏻‍♀️

    It sounds like trying to get a good idea of the layout of the hospital when you're not stressed/before you start might help just a little. Like a map and a walk around so you're kind of familiar with the layout?

      Your hospital privileges are specific to the period of your placement there so often your access only start on the day of your job. Prior to that you wouldn’t have access to some floors etc since you have to buzz in through doors for certain floors etc. It’s not like you can waltz in and give yourself a tour.

      @@hollyl5702 can’t imagine how that would be feasible. You don’t get your badge until you are actually working there. Your badge is what you’d use to buzz in to get to the floors.

      As a home health nurse I had to deliver lab specimens . This hospital was built around railroad tracks. No one told me that you had to go to third floor to get her to the other side of the hospital .

    I'm over here getting burnt out in Phlebotomy working 70 hour weeks of frequent doubles...and an ER doc tells me not to become a doctor because then I might have to work blocks of 48 hours back to back with cat naps! Is that even legal? Are doctors human? Anyhow great vid

    I laughed so hard I use to have to help train the vets. ...god they could be so bright but so naive I can say this because one of my best frie D's was a nursing college professor ..she kept a bottle of 1,000 phenobaeb under her bathroom closet....sjes gone now fell down the stairs stroked out hit her head. God bless her ...it takes the,patience of jobs
