The manhole cover on the bomb is true. They filmed it with a high speed camera and determined its velocity was so high it left the earth and has traveled beyond the solar system.

Testing nuclear bombs underground instead of the desert they realised that one of the manhole covers had disappeared. So they recreated the same situation and set up a camera. The speed at which the manhole cover was launched into the air it escaped the atmosphere and is now floating in space.
