Maybe a little bit, but the cloth is tucked away in the cartridge when plugged in, so I'd imagine the dirt is pretty minimal. Besides, those chunky dinosaur ds's could survive a nuclear winter and still run all the Pokemon Yellow you wanted. I'm sure they could handle a little dust.
You probably want to clean the cloth every couple times you use it. Besides the Japanese perception of 'dirty' probably isn't anything like that of the average American.
I've actually got a smaller version of that made to sit flush in the DS Lite. Came with a whole set of other stuff, but that's the only part I still have.

I had a 3ds xl one, a Dsi xl one, and a regular 2ds one. My friend got one for her 2ds and my other friend got one for the Dsi XL he had. Some random student got the 3ds xl one so I was glad to find people who could use screen protectors that I couldn't. It's one of my favorite things I got for my 3ds and Dsi along with a case, styluses, game holders, and etc. I was happy about getting it as a gift for Christmas one year.
