Team-Based Person-Centered Ward Round

  • At the department of Internal Medicine at Kungälv’s hospital in Sweden the institutional ward round no longer exists. We have developed a working model that narrows the gap and facilitates the collaboration between the different health professions. The ward rounds have become more effectively run, resulting in better medical care. The ward team is now meeting the patient at the wardstations, not in multibed rooms, securing the patient’s integrity and giving them a chance to participate in their own care. Today we try to listen more carefully to the story of illness from the person who became a patient.

    This approach really works! About 75% of adult medical inpatients can come to the room for consultation, the remaining 25% are bedbound. This process respects the professionalism of nurses. The night nurse team decides which patients need to see the team that day. If someone is stable and waiting for a test he may not been seen. This releases more time for those needing more attention.
