If I wanted to build up strength after I've had rotator cuff surgery, would you go with this exercise at a much lower weight? Would that be good for it?

Great control. If only I could stop the kettlebell from slamming into my wrist everytime
Try gripping toward the thumb corner of the handle so your wrist/forearm can go diagonally across the bell and sit in the bottom corner of the opposite side of the handle. You can see this pretty well when I'm holding the bell in my left hand. The same applies whether you're using a competition or cast iron kettlebell!

The back of my forearm hurts so bad when I hold the kettlebell like this. Is it because my wrist strength is too weak?
Keep doing it. Eventually, your forearm muscles will get used to it. Just make sure to keep the wrist straight and very slightly forward.
Try keeping your fist and wrist straight, like you are punching the ceiling.
Need to see this from the back. If you gonna hike the shorts anyways can the boys at least get a little taste?
I would suggest to try a lighter weight and especially of the Pro Kettlebell brand
