I was at this game and was sitting behind the Pacers bench. Prior to the malice, the fans were belligerent and completely disrespectful. In fact, I had to check a guy for his language because I had my daughter with me. If I were a Pacer, I would have reacted the same way.

This point is important. Fan behaviour was a known issue at the Palace. Celtic guard Erick Strickland had a drink poured on him going into half-time of a playoff game in 2002 - and HAD TO BE RESTRAINED FROM GOING INTO THE STANDS. It's all on film. Not addressed in the documentary, but they only had an hour.
Ron Artest talking about his mental health struggles is compelling viewing.

I just watched Malice At The Palace. It’s crazy how there were so little security to control fans. The ref didn’t diffuse the situation well either. How can the security not realise the game is getting heated, fans were agitated and security didn’t do enough to stop fans from coming down.
Ron Artest went on the scoring board to calm himself down. Then that idiotic coward fan, John Green threw the cup of beer on him. Fuck that, in an interview, he said the threw the cup and find it amusing that another guy got beaten up. That was really stupid of the fan to do that. Just because you paid money to watch games, you don’t have the right to physically abuse the players.
There will always be fights in sports. In my country, we fight because of football/soccer. But the difference was, the security stopped fans from going to the players. The police had batons and beat the shit out of the hooligans. And the fight are mostly between fans.

Let me preface this by first saying that fan, John Green, should be left in a room alone with Artest for at least 5 mins and get the beat down he deserves. He is an absolute scumbag and didn’t get nearly enough punishment… with that said, I think you’re looking at the whole thing with too much ‘hindsight 20/20’ because the actual results weren’t too bad in regards to who got physically harmed when it was all said and done. The potential for serious damage to multiple people was very very high though. I was at the game safely on the other side of the court and saw it all. There were women and children in the area. Let me give you a scenario; Artest runs up in the stands and knocks a random kid over. Jackson follows and accidentally tramples that kid. The kid’s dad looks in horror, freaks out n pulls out a knife (or keys or whatever) and starts stabbing Jackson. Maybe Jackson never plays again or worse, dies. Now there’s 2 deaths resulting from them going into the stands. I feel like the perception would be a lot different in that scenario. They were not thugs and I totally understand why they went up there but I also understand why the punishment was so harsh. You absolutely can NOT go into the stands fighting people especially when you don’t even know who you’re looking for. Artest literally ran right by the guy that actually threw the cup. All the other fans got involved because they were hitting innocent people. That’s my vibe toward it all. The potential for much worse was very much there

Payton Manning was getting along with the Colts at that time too, so it happened quickly, they lost the city from that moment the team embarrassed the community, and the NBA so lopsidedly favored the Pistons, and the Pistons acted like sore losers period.
