Nerve Pain after Knee Replacement Surgery. WHY and HOW to Manage Effectively

    Nerve pain can be described as zingers, pins and needles, jolts, burning, numbness, or electric feeling.
    This can be common after knee replacement. Usually the two main sources of this pain is irritated peripheral nerves in the leg and close to the knee joint or from sciatica and radiating symptoms.
    What can be done to manage this pain? The best thing to do is give your body time to heal and focus on your range of motion and mobility in the knee, but also in the leg and low back in general.
    There are also specific nerve gliding and nerve mobility exercises and stretches that help improve nerve mobility and decrease irritation. You can check these out by following the links in the corner of the video and at the end!
    Another pain relieving stretching is TENs or electrical stimulation. Your PT can help instruct you with using this pain relief modality and potential get you setup with a device to use at home.
    If your nerve pain is significant, your doctor may prescribe certain anticonvulsant medications such as gabapentin or prescription pain medicine.

    I'm 1 1/2 years out of a left TKR. I still have lower leg issues. Doctor states the X-ray/MRI looks fine. Looking fine doesn't help me. There is something going on with the nerves, I can just tell. Numbness in my foot, still (although not as bad). Lower leg pain/discomfort along the IT band and Fibula. I have 130 degree range of motion - no issue there. Can scar tissue be causing the pain? They can't pinpoint the issue. I'm truly disappointed with the whole thing.

      Scar tissue by itself wouldn't necessarily cause the pain. I would consider seeing a PT to assess for low back issues / sciatica contributing to your pain and get some exercises and treatment for the low back. Issues from the back can leading to referred pain along the leg and foot. The replacement itself likely is okay, but you may have an adjacent issue causing your symptoms.

      Luke!! Howdy from Mt Shasta!!! I had Lapiplasty surgery/ bunionectomy right after Christmas, nerve blocks up in my quad (3), and enjoyed 7 days of numbness but now it’s wakey wakey with nerves doing all kinds of dances mostly around my foot which feels like I have a tight bandage all round it. I have partial numbness from my knee down. Current therapies are: NSAIDs galore, Arnica oil topically, Arnica Montaña pellets orally, laser therapy and acupuncture. It was super swollen due to nsaids- still somewhat swollen, of course. I’m soaking with Epsom salts and cold compress
      Big question: I’m on heel only weight bearing, can barely wiggle toes. Does ROM help nerves respond normally? Should I be more aggressive with gentle massage and passive ROM? This is new territory for me/ Lots of surgeries, just not educated re: fired up nerves. God be with y’all. 🙏🏾😉👊🏽

    I love your explanations, had both knees replaced 7 weeks ago and the left knee is so painful and stiffish.
    First up is to make a appointment to see a GP and see how to continue with the physcio.

    Neither you nor my surgeon told me that a knee replacement requires that the main nerve that goes down the front of your leg is severed. Yeah! So some of us are really going to suffer. Even my physical therapist didn’t tell me! I saw a doctor here on CZcams who told us all. He is awesome. Why don’t the rest of the medical community tell us??? When i questioned my therapist, he just said yes. Wow!

      The nerve that is severed is a sensory nerve that provides sensation to the front and outside part of the knee. This is why you experience a numb patch in this area. For most people this numb patch diminishes over time but the numb patch can persist.

    I’m three weeks post TKR. The swelling has decreased and my ROM is above 125, but I’ve been experiencing the pens and needles type pain. Thanks for the explanation and recommendation about using the TENS.

    Why does no one talk about this after total hip replacement? Unbearable pain for weeks that actually made me weak. Burning, throbbing pain in my whole operative leg that swelled and bruised badly. 6 weeks post now and is slowly subsiding. I have been totally ignored by my surgeon and his team, including the PT. Thank you for explaining this...and behind my knee is still swollen and sore. Exercise twice a day and started getting back in the pool.

      I had terrible bruises and terrible nerve pain ., sheets even irritated my nerves. I think the nerve pain was worse than the surgery. I got it under control using my Tens Unit, PT and ice. But I agree they don't talk about it enough.

      Hi Mary, I am 4 months post anterior hip replacement and I am much better. The only thing that helped me was taking Alpha Lipoic Acid and B12. I still have a little pain in the top of my foot, otherwise, getting much stronger working out in the pool. Glad you are better.

    Thanks for the video. I am 5 mo PO from TKR and having a horrible time with my piraformous, hamstring , calf, and foot. Electricity shock radiates down my leg in a swirl almost like a barber pole. Pain is so bad that it cramps the calf at night, leaving the muscle tight and compressed the next day. Frustrating! PT 2-3 x per week.

    Thanks for the video. I am 1 and half year from my (R)TKR and I am still dealing with scar tissue build up and nerve pain. When I flex my knee it feels like pins and needles sticking in my muscles. I feel the pins and needles, sharp stabbing pain burning through out the day everyday and I have numbness to the right area of my knee as well. My knee flexion is 100 at the most and I have to push and stretch to get over 100 with enduring pain. I have pain and stiffness constant. My thigh and calf muscles are tight and stiff and get cramps after exercise. The doctors says my Xray looks fine, did blood work no infection. It does not hurt to walk but it hurt when I flex my knee. What are your thoughts???

    I am from Malaysia. I did Tkr on my left leg. Its been 5 mth. Yet knee is a kind of porking pain and on top of it , my feet is like porking needles. Pls advise.

    My pain management put me on a low dose of lyrica 2X prior 3X after I’m scheduled this Wednesday

    The nerve pain is so bad in my knee. I can’t stand to be on very long. I have sharp, stabbing pains pain is very bad. I’ve been seeing a doctor but not mean shot in my knee two times but it’s not helping. Will a nerve block Burning help

    I have nerve pain in my left knee. I had a total knee replacement, but still having real badpain i’ve had physical therapy. They have gave me two different shots two different times in my knee. I’m in severe pain. Can’t stand very long can’t walk I need help.

      This description is sometimes used by individuals experiencing nerve pain or nerve damage or it may be from damaged vasculature... This may be secondary to surgery or another medical diagnosis not directly related to the surgery. I recommend consulting with your personal doctor / surgeon.

    I'm 16 wks Post-Op on my Left Knee TKR. I hope to schedule the Right knee for TKR when the Left feels strong enough to support its recovery. My Left Flexion is close to 120 and maybe within 5-8 degrees of my Right Flexion. Extension is ok at 0 - 1 degree. My main concern currently is pain & stiffness on getting out of bed in the morning and also after sitting for 20 mins or more. It feels sciatic in nature and takes a few minutes to subside to where I feel I can move smoothly. This article was helpful in letting me know these symptoms can occur many weeks to a year in Post-Op.
    Since, according to the post, the Peripheral Nervous System is involved, I'm thinking perhaps Meditation and Slow-Movement/Balance exercises(ie. Tai-Chi, Qi-Gong) might be helpful.
    Got any insight on the effectiveness of this approach?

    I have undergone two surgeries in my right knee after an accident. My right foott hurts a lot. I feel strong tingling sensation with sharp pain in my foot. It pains a lot. Pls tell me how long will this pain will be.

    Hello Sir,
    My mother had knee replacement surgery of the right leg around 3 weeks back. She is having sleepless nights because of nerve pain on thigh, calf & side of feet.. the pain is very high at night. She also feels as if some pin is being pinched sometimes that you described.. when we press the nerves a bit she feels better. I hope it's not a permanent problem & she would recover soon.     If a nerve near the knee has been irritated or damaged during the surgery, it can be considered as inflamed? Are there sensory nerves near the knee, how does a surgeon distinct them so as not to cut them?

      Yes when I say irritated I’m usually referring to inflammation, compression or some sort of stress to the nerve. Yes there are sensory nerves near and around the knee joint. With the incision along the front part of the knee there is actually a branch of a sensory nerve that is cut, this is why people experience a numb patch on the lower outside part of the knee. This usually goes away over time as the nerve heals and regenerates. Otherwise actual nerve damage or cutting is rare and is more so usually a result swelling, inflammation and changes to your range of motion from the surgery

      At 18 months post-op, I would first have a doctor or PT screen your low back as this is most likely the cause of nerve pain down the leg and outside the knee. Thanks for watching!

    Six weeks out from knee replacement surgery things have been going well -Doing physical therapy and now all of a sudden excruciating burning pain in my knee more in the middle of kneecap (for lack of a better word) which is really uncomfortable-
    I have emailed my Dr and just wondering is this normal at this point?

      I think it is a good idea to consult with your doctor. It is likely there is nothing wrong with the knee replacement or surrounding area, and this pain will probably subside as your rehab progresses... six weeks out is still relatively early and different pains in and around the knee are not uncommon... but never a bad idea to double check with the doctor especially since the pain is new and sudden.

      I think you are doing the appropriate things by consulting with your doctor. I'm sorry the shots have not helped. A nerve block may provide better relief depending on the source of your symptoms. I hope you are able to find relief.

    I am seeing a doctor that’s been putting shots in my knee for numbness. It is not healthy. They were going to try to do a nerve block burning in my knee. Will that help?

      I'm 10 days post opp. When I'm up walking pins & needles arent bad. After sleeping or laying down for any period of time when I get up It is literally pins & needles on fire or as if they are electrocuting me as I put my foot down. It only subsides after I've started walking. I'm hoping this let's up & was hoping to hear about how long into the healing process that this begins to subside. The Surgeon that did my surgery is one that will do both knees at once. I'm so glad I chose not to do that as it would literally be hell on earth to endure. I'm considering doing the robotic surgery for the other knee as it claims less swelling. I didn't have the intense bruising until 3 or 4 days post opp. My fist day at physical therapy 2 days post opp I breezed through the physical therapy doing all the exercises pain free. 6 days post opp PT was very painful doing leg lifts. Became difficult because of the pain with lifting the leg. bruising started out mild an inch below the dressing to inch & half above. leg is purple now from groin to heel. painful to walk on the heel.. Feels like an intense headache in my entire leg now. Elevating is painful as it seems there's not a good angle that relieves the pain. Went from Hydros 1 every 4 hrs. Was not even taking the edge off. Switched to percocet finally slept more than 3 hours but woke up next day weak light headed and slightly nauseated. Made for a rough day the next day. I can't tell that gabepentin I have has helped in the slightest with pins needle intensity. I have very little help as no one is here with me but am counting this time as an opportunity to build Character. When Younger I decided zif I became a mechanic I'd likely b hell bound with the foul language associated with that. I have now considered Knee surgery to be added to that list but still hav the other knee next. Thank God we only have 2 knees rather than 4. I hear the majority of people who have this done claim it's Amazing I'm obviously not at that stage but hoping it comes sooner than later Has anybody ever had one knee replaced non robotic and followed up the other robotic? if so was there significantly less swelling & bruising after robotic?       There is no reason you can't do this after knee replacement, but it may not be the most appropriate exercise for you to address your back pain and symptoms. I recommend consulting a physical therapist who can do a personalized assessment and provide the best exercises specifically for you and what you are experiencing.

    I’m one month post op ACLr and meniscus repair. These stings started in my lower shin/upper heel 3.5 weeks post op. Is this too late in the surgery to have these feelings? If not how long could it take for these to go away? It’s impossible for me to sleep. Thanks!

      It is not uncommon to still have these "sting feelings" It's difficult to provide a specific timeline but usually the sting sensations will decrease in frequency and intensity slowly over time, usually a number of week to a few months.

    hello, i m 22 years old(F) and i had ACLR with hamstrig graft 3 days ago, all of my shin and medial maleola are numb and every time i put down my leg i feel a burning unbearable pain all over the numb are , i m very scared it won t go away and i won t be able to go thru rehab as i can t even go to the bathroom because of this sensation, what can i do? and do you believe its permanent?

      It is difficulty for me to tell what exactly is causing this without an in person assessment. My recommendation is to contact your surgeon's office for specific consultation and also you can consult with your physical therapist which I'm sure you should be starting soon if you haven't already.       @@KneeReplacementTherapists i started some phisical exercises, my surgeon told me it s normal to feel the numbness and gave me vit B6 for the burning sensation but didn t tell me anything else and everytime i was asking he only told me it will get better, which is not reassuring for me:( i m scared

    Hello I had my right acl surgery and lateral meniscus repair 6months ago and everytime I do leg extension or try to put my leg straight after workouts I have a very bad nerve pain that runs from my knee to my buttocks area ! Is that normal ? And what can it be ? Currently going to pt twice a week as well

      This is not a normal sensation 6 months after surgery. It is difficult to provide an answer without a thorough in person assessment... you may have sciatica, a pulled hamstring muscle, or something else. I would definitely share with your PT what you are experiencing and hopefully they can perform an assessment and make appropriate recommendations.

      There is a branch of a sensory nerve that crosses along the front of the knee where the incision is made and provides sensation to the outside lower part of the knee. This is why it is normal to have a small numb patch in this area, but this numb area usually get smaller over time or essentially becomes unnoticeable.
