Doctor Explains the Latest Vegan vs Keto Head to Head Study

  • A new study published in January pitted a ketogenic, low carb, animal-based diet against a plant-based, low fat diet in a fully controlled environment. A lot of dietary studies are simply not even worth reading, but this one merited a quickie video to share the interesting result as it was done well. The carbohydrate-insulin obesity hypothesis has been widely accepted but not tested. Thankfully non-COVID research is starting to make up more and more of medical news once again.
Argh, this is what happens when you try to write/record/edit/upload on the same day. Typo at 00:07:14 - should read "this does not imply it is the ONLY option..."

A quickie down by the river; one-take explainer on an interesting study. I'll be honest with you, after the first few hours I'm not going to check the comments on this one. But I dream of a day where ketonauts will be able to join hands with veganists as brothers and sisters, not divided by what it is on their plate, but united by the fact it is free of processed food. Keep it civil please.
Edit: thanks for those pointing out the typo at 00:07:14 - should read "this does *not* imply it is the ONLY option...". The downside of trying to do the whole video in a hurry in one day!

What happened to the insulin sensitivity of the keto-group at the 2-week switch? A common argument from the vegan camp is that fat-intake increases likelihood of diabetes due to fat accumulation in cells preventing insulin signalling and glycolysis.

Honestly, I really appreciate the POV of this video. It’s true that there are a lot of videos on diet, but many of them (even many that go into the scientific literature) end up pushing one solution, and ignoring literature which suggests another diet might also be effective. I think it’s an open question as to what the optimal, long-term diet is, but it’s reassuring to hear that your main priority should be to eat healthy, unprocessed foods.

The biggest problem with such trials is the time span: sure the first 2 weeks of weight loss on keto is water, but once that is lost, the body has to break down body fat. It's so frustrating do have studies for such short time spans, as the body and especially the gut takes some time to adjust. That is true for all dietary changes.

    "Both groups lost weight"
This is just a minor detail in this video but reminds me why I'm so fed up with all those diet debates: many people treat them as weight loss competition. I'm underweight and always have been, and I get really annoyed when people start praising their specific diet because they lost weight, compared to actually important consequences like overall health or metrics that apply to everyone - but of course that's more difficult because it's more than just one number and requires more knowledge than knowing where to buy some scales.

    • @daveyhintzen53
      @daveyhintzen53 Před 2 lety +10

Her theory is the lifestyle. Americans are always in a hurry, when hungry they chug down a protein bar or some fast food and rush onward.
In Czechia people actually take time at work to leave the office, go to pub and have some actual food, maybe a small low-alcohol beer and chat.

Pretty sure if you want your body to function at it’s best it’s going to be keto. Evolutionarily speaking I find it odd that our bodies would not prefer using high density energy as it’s main fuel source. More density = more efficient to obtain the energy. Then, the body is clearly extremely capable at storing extra fat and carbohydrates as fat tissue. What this sounds like to me as an adaption to the over consumption of food and storing it for later… but on a high carb diet we funnel resources into carbohydrate metabolism instead of fat metabolism. And the fat just collects over the years.

Plant-based: lost weight but always hungry and had to eat HUGE portions (did it for one year), skin did not look great (mind you, this was a very low fat, vegan diet which may be the reason I was always hungry and had skin issues). Always felt like I was being punished. Keto: Lost 80 lbs, went from Stage 2 hypertension to normal, reversed pre-diabetes, lower back pain I've had since a child gone. Achy joints gone. Only need to sleep 5 to 6 hours per night. Do not obsess over food and only need two squares per day. Skin looked amazing (reduced wrinkles and age spots disappeared). Keto: still going and loving it. (I do have an occasional treat meal on holidays and what not -- gotta enjoy yourself sometimes). Mind you: keto is not stuffing yourself with gobs of meat. it's small portions of meat with huge servings of low carb veggies. Eggs, some nuts, a little cheese and healthy oils: olive, avocado, butter, sesame. Also on plant based, I'd have to eat a good 3000 calories per day to not feel hungry all of the time. On keto, I eat about 1200 to 1500 calories per day and do not go through hunger pangs throughout the day. Everyone has to find what works best for their body. We are all different. I am finding something similar. I've lost around 25 lbs on a largely plant-based diet over the last two years (I kinda lost a lot of the same weight 3 times, due to repeated illnesses, antibiotics and steroids), but I would feel very full on a low fat meal for a couple of hours, then very suddenly be ravenously hangry. I battled constantly with intense cravings for junk food. I always felt better after binge eating.
For a couple of months, I've been realizing my body does NOT want to be on a very low fat diet. I've been looking into a largely plant-based keto diet (hence finding this video). I've been eating high fat, low carb for only 8 days, and despite a couple of unpleasant symptoms the first three days, I haven't felt this good in probably a decade. Cravings for junk food are gone. Binging is gone. I can stop eating before I even finish my meal because I finally feel completely satisfied, and after a few days, I don't NEED coffee in the morning to function. I've also lost 6 pounds.
I still eat lots of veggies and a very high fiber diet. No fruit, but LOTS of nuts, I'm not afraid of oil anymore, and I'm eating a little meat and eggs (which I would end up doing when I'd binge eat, anyway). I told my husband, "When I was eating vegan, I would be full but not satisfied. Now when I eat I feel satisfied but not full."

To a mammalian body, there is really no difference between ketosis and the starvation state. Your 5-6 hours a night are more likely due to chronically high stress hormones than any kind of evolutionary preference to burn fats over glucose. 'Keto adaption' is a nice concept, but it's just a band aid over pre existing metabolic dysfunction. Cells need to burn glucose for energy (hence gluconeogenesis). They will break down lean tissue to get it just to keep you alive. Essentially there is plenty to indict the Keto diet, but you won't find that information in the Keto space.

It's a very interesting topic! I've done very well in terms of health and weight loss on a very high-carb vegan diet, I've managed to reach goals that I could never do before (blood tests, stamina, muscle gain, weight loss, mood) just by simply changing my diet to whole foods plant-based. I researched the topic a lot before making the switch from being an omnivore as I was really worried about nutrients and everything on a vegan diet (as I think everybody is). I really liked everything I read + soon enough I had the experience myself. For years, I was a firm believer in the high-carb vegan lifestyle, and I still am! Based on my research I thought there is nothing else truly good for the human body. I still think it's very nice compared to other diets because of the high fruit and veg intake and overall healthy approach. But for the past year, I started intermittent fasting (for quarantine weight gain) and that's why I started looking into keto because I heard the two works best together. Because I want to remain open-minded I did a lot of research on (healthy) keto as well and I like the concept, so I'm going to try this diet, too. I guess my point is: people don't be narrow-minded, lots of different things can work on lots of different people, unless you have a medical condition that omits you from trying something, don't judge until you tried. And you don't have to do restricting diets to have results, before this whole thing I just ditched processed foods, sugary drinks and aimed to eat fruits/veggies at least once a day (compared to almost never before that) and it helped a lot. Combo that with a half-hour walk every day and you will see results without a doubt.

Look at bread from a supermarket and see the ingredients, preservatives, emulsifiers, and so on compared to a freshly baked loaf from a baker with less than a handful of fresh ingredients.
I have zero education regarding this, but I can only assume any overly processed foods don’t leave much for the digestive system to work on. From the chewing of food to the large intestines ability to absorb any remaining nutrients. If food is over processed, most of the system doesn’t really need to work hard if at all. I believe (once again, with zero education and knowledge) that this plays a significant part in the amount of digestive system issues in countries where foods are generally more highly processed. More advertising regarding IBS, or heartburn, and so on. Not to mention general health of ones stool.
I’m always open to learning more about diets, having been vegan for more than half my life. I try to balance my diet with my morals as well as environmental impact in addition to personal health and I notice that foods I eat can really affect how I physically feel.

The first warning sign looking at the study is that the main finding seems to be that the LF group ate less calories than the LC group. Note that they didn't find any significant result in how much fat both groups lost, just look at the results in the paper. This paper is basically arguing for what ketoers has been saying "a calorie is not a calorie".
      some othe points
      1) 10% fat is not keto, 5% is keto. And especially for beginners, you should start with less than 20g of carbs to be on the safe side. Looking at the figure containing how much calories the participants ate, the average "ketoer" ate over 2500kcal, which is more than 50g of carbs. This is not keto.
      2) Short term weight loss is worthless, most people gain the weight back and then some. It's all about the long-term viability.
      3) Ketogenic is not a quick fix, it takes a month or two to fully adapt. You can count on eating a lot the first 2 weeks since your body is screaming for sugars and the only way to stop it is to eat more. This holds truer when you're a random participant in an experiment and have no knowledge about the keto process and don't utilize discipline.
Again, this paper only says that the LF group ate less calories than the LC group. It didn't show any significant result in terms of fat loss. I don't think we should discount the study, ketoers should be encouraged that they can eat more and still lose the same amount of fat as the LC group.
I find it strange , I don't know if Medlife Crisis knows what the word significant means, but he seems to use it when he pleases. He said that LF lost more fat than the LC, but there was no signficiant result which he didn't mention. He was quick to mention that the keto group lost water weight and no significant fat loss. He also said "there was no body fat loss", but looking at the diagram you obviously see some loss of fat mass. It's so manipulative to try to come off as "unbiased" while selectively chosing what to convey and not convey. This is not an unbiased video.
He didn't even show the insulin and glucose charts which heavily favored the LC group. He showed all other charts.

Problem with studies like this is the people setting up the study do so at their understanding such as supplying not true keto foods. He said there was no change in hunger or feeling full. That tells me the people were not truly in ketosis. I can easily go days without eating and not be hungry.
I lost 40 lbs and over 10% in body fat in about 3 months on the keto diet and when in ketosis, there is very little hunger if any at all.
He also said its not about reducing heart attack or stroke risk. After going keto docs pulled me off my heart meds, blood pressure meds, diabetes meds, constipation meds and said I no longer need to be monitored for strokes. I went on keto after having a stroke.
People on keto diet eat veggies, vegans don’t eat meat. Keto doesn’t have the nutrient deficiency veggie only can have.
High insulin levels have many health issues, there is only diet that lowers insulin levels.
Both diets are limited by what you eat. Being a vegan eating bad non orgainc food is not going to help. Same goes with Keto and eating bad fats like canola or soybean oil. Potatoes are vegan and one of the worst things a diabetic or prediabetic can eat. Worse than pure table sugar. Glycemic index is almost 50% higher.
I’ve talked with hundreds of people on the keto diet, every one who is doing it right has had significant weight loss and health improvements. Same with vegan.
The particular diet isn’t as important as how well you do the diet. If you don’t do any diet but eat organic, healthy fats, healthy meats, cut sweets and watch your quantity you’ll be better off.

Also, my relationship with food was better on plant based because I felt I was really nourishing my body rather than restricting food. The only thing I missed was eggs so now I eat them too along with the occasional sweet treat. It doesn't have to be crazy all or nothing. It is far better than the heart attack diets most people are eating today, and there is no way the world would ever go full vegan, and I understand that. But if everyone would eat like in the blue zones, we all would be happier, healthier and we would have a much gentler impact on our planet.

I noticed my clothes were baggy but didn't think much of it until I realised I'd lost about 10kg in only a few months. One year on, I've lost 25kg and I find it odd how much progress I've made without trying all that hard. This is after years of many failed diets - so I'd given up, and started accepting things weren't going to change... My diet "strategy" these days is simply cooking foods I enjoy eating. Your point about the lack of processed foods is very on point! This is a very refreshing perspective. Two very important things that a keto diet gets you to do is cut out added sugars and processed crap in general, and to stop overeating. Both of which are obviously going to make you feel loads better (once your body adjusts). But once you've done that, I think the good doctor here is correct, you're not going to necessarily do better on the keto diet than on any other balanced, whole-food diet. A lot of people can probably do with cutting their carbs down at least a bit, but it's probably more important for most to eat whole-food carbs instead of processed ones... It's also not a bad idea for most people to fast from time to time, which is something else keto is teaching you to do. But I'm not sure we should be doing it continually...

Ok think found the explanation. Both diets in this study were devoid of proteins. Either 75% fat 10% carbs (keto) or 75% carbs 10% fats (vegan). Since Protein Leverage Hypothesis dictates that humans will seek out ca 20-25% protein ad libitum, as every species seeks out unconsciously a certain protein percentage intake. The plant based option was more filling and provided more trace proteins, plus protein sparing effects of carbohydrates. Hence the protein intake was met earlier than the Keto group. A higher protein, healthy keto diet could have shifted the result the other way.

True, but I think the "eat as much as you like" part has a significant side to it as well. When I prepare food, whether I cook or take something ready-made, I tend to eat the whole package - regardless of what a "serving" is supposed to be. In this experiment, people would stop because they felt like it, not because the bowl was empty. Perhaps streamlining leftovers would help with any of these three categories of diet (but probably less so with the ultraprocessed products literally designed to make you not stop).

I did both diets for 14 months. On the plant based diet, I dropped from 242 lbs. to 186 lbs, and a measured body fat level of 5.8%. The problem was, I sort of felt brain fog from time to time, and I didn't look well. I feel that the plant based protein was insufficient, either in it's bioavailability, or the amount. I also lost weight on the low carb diet, but even with all the protein, I lost muscle. My workouts in the gym were VERY good on the Plant based diet, but suffered with the low carb approach. I lost both strength and high intensity endurance capacity with low carb. My body composition ultimately, with the low carb, was not good. Apparently the body does not produce ATP fast enough to keep up with the demands of high intensity training. What I finally ended up doing, was doing neither, but instead taking the best from both approaches. Now I do about 25% Protein/ 55% Carb / 30% Fat approach, of a Whole Foods Diet. I get all the benefits of all the vegetables and fruits and nutrient dense foods of the vegan diet, along with a higher amount of healthy fats from the Keto approach, along with some plant protein, and some animal protein...(hopefully Pasture raised meats and eggs, and wild caught fish). It's basically the combination of both diets. I'm the leanest and hopefully healthiest, and for sure look the best, that I've ever looked. So, that is my thoughts on it. The best of both these approaches is the way to go.

And after now almost 3 months i lost about 11 Kg and only eat once a day. I never felt hungry or low on energy and am very happy to have started with it, i always test the keto percentage in my blood so as to be in the optimal range. But the main thing is that i have completely deleted sugar from my diet and it really makes a huge difference. But I wont argue with anyone that keto is better or worse than vegan or anything else. One thing I would like to point out is that, for a person who has never followed a ketogenic diet before, getting into ketosis and adapting to that way of eating takes more than 14 days. When I first started keto, I consumed quite a lot of calories, especially for the first two weeks. But as time passed, my need for food reduced significantly to the point that I don't really need more than 1 or sometimes 2 meals a day. The best part is that my hunger and cravings reduced significantly, without much need for willpower to control my eating habits.
I would love to hear your thought on this.

    • @nitfitnit
      @nitfitnit Před 3 lety +23

Most studies show people on keto eat fewer calories once they've adapted, that's the main reason they lose weight. 14 days is way too short to draw any conclusion on how a diet will work in the long run, especially in the real world where you aren't locked up with someone bringing you meals they've chosen. I've tried both low-fat vegetarian and keto. My weight didn't change much but my blood lipid profile looks much better on keto, and mentally keto feels better with fewer ups and downs in mood.

If the eating habits were not changed--and they were expressly not changed in this study--then it's obvious that American eating habits (three meals, plates filled) with calorie-dense foods will result in a higher daily caloric intake than the same eating habits (three meals, plates filled) with less dense foods.
They gave the study subjects no information or instructions and the test wasn't long enough for habits to change naturally.
