Being a hairstylist of almost a few years, I can share with you you, that you're not supposed to apply strong hair shampoo on your hair.
I have noticed hair lines of numerous people, and I noticed that individuals who were using harsh shampoo cleansers generally had thin hair line.
So I suggest only to use mild shampoo only. I personally use and recommend to apply litttleextra cocconioon shampoo only.
Also, do hot oil massage twice a week as it enhances circulation and encourages healthy hair growth.

The anti androgen effect is not through the inhibition of 5ar enzyme, it doesn’t not block the aromatisation of testosterone to dht. From theory it should locally bind to androgen receptors in the scalp, beating DHT and Testosterone, potentially halting miniaturisation of the follicle. However the theory around it’s localised presence can be debunked by the fact that if it can be absorbed it will inevitably go systemic. It is a weak non steroidal anti androgen… not a 5ar inhibitor. It may work for people who are not genetically prone to AGGRESSIVE androgenic alopecia (99% of males are prone to hair follicle miniaturisation to a certain extent) but it will not be an effective compound if used as stand alone mono-therapy for androgenic alopecia. Keeping in mind with that, the use of finasteride will also not successfully halt androgenic alopecia because even though you are running your DHT into the ground there is an increase in testosterone. Testosterone has a lower androgenic effect at the hair follicle. Meaning the speed of miniaturisation is decreased but not completely halted. This is where there is potential found in the use of better more potent non steroidal anti androgens to combat hair follicle miniaturisation on both the androgen receptor front and then using finasteride to decrease DHT and indirectly reduce the level of androgens the anti androgen needs to beat to the receptor

Just a correction - inflammation does effect the Male pattern baldness not just a temporary shedding, it shortens the Anagen phase which accelerates the permanent loss of the follicle, also not sure if the comparison with finasteride/Minoxidil was necessary as Ketoconazole Shampoo's are a complimentary treatment best taken with finasteride.

Canadian waiting lists for various medical needs is wild and most Canadians have no idea that you can get in to see doctors like dermatologists for as little as $70 US when you're not seeking treatment in egregiously priced cities like New York. Obviously finasterite and minoxidil are the drugs used as first line, but Ketoconazole has shown in studies (small sample size and low quality) it hsa potential to help in hair loss. Thus a 3 month trial on it every 3-4 days, i dont think would be bad given the person uses it with realistic expectation.

I have been using minoxidil for a month now after consulting a dermatologist.. i will get on a topical minoxidil+finasteride+procapil treatment without re-consulting the doctor from tomorrow.. the doctor refused to give me finasteride.. but i did thorough research and i am pretty sure i am the perfect candidate for finasteride.. I am guessing he doesn't have much knowledge about finasteride and it's probability of side effects and ways to alter doses.. it sounds bad but i think i am gonna take things in my own hands..

This could potentially be better in regards to everyday use, keep in mind anti androgens don’t decrease dht they beat DHT and Testosterone to the androgen receptors in the scalp, KT is a weak anti androgen so the way it would beat DHT and Test to the receptor is not by binding affinity but by sheer number of the anti androgen. However you have to think about the risks of satellite interactions with other receptors in the case that it goes systemic. You could argue that with it being weaker it shouldn’t have any satellite interactions with let’s say receptors in the musculature. However if it cannot bind to receptors in the body in the case it goes systemic it will most definitely not heat DHT and Testosterone at the scalp

Risks: heart-related anomalies, brain fog, dark bags under eyes, wrinkles etc.
Rewards: studies have shown Minox to be 4 times more effective with microneedling (could be due to the repair mechanism rather than increased absorption).

you cancel most of the DHT conversion from testosterone. But proviron is littarly the same as DHT in the body just made in a lab. That DHT/proviron finasteride would not help against as it does not deal with the androgene reseptors, or the DHT itself, but inhibit the enzyme 5a who creates DHT from testosterone.

I saw very good results using Alpecin + Nizoral 6 days a week, minoxidil once a day for 6 days a week and on the 7th day microneedling 1.5 mm without any drugs on that day (don’t want anything to go systemic). I started the microneedling recently, saw good results even without it, so if you don’t want to do it... you don’t have to, but studies have shown it to be very effective. The results have been amazing. Alpecin is probably the least effective part of this treatment though. I still use it since it’s as cheap as any regular shampoo and I need to wash my hair anyway. The important part with any shampoo is to let them on the scalp for 10 minutes before washing them off. So, maybe go in the shower apply Nizoral + Alpecin, wash the rest of your body, then wash off the shampoo from your hair at the end.

Hey Doc. Im a 19yo Boy and had an itchy and oily scalp, often came with dandruff. There were some red spots on my scalp too, exactly there on my vertex, where my hairs shedded. Those spots disappeared after one month and i showed my derm those pics and she said: nothing but worry but you can try Ketoconazole Shampoo. One week after starting it, my hairloss went from an average daily loss of 100hairs to 20/25. I track them while taking shower by approximation. But it has reduced the amount of lost hairs significantly. I still see the pattern of MPB, but its slowed down extremely.

Yes I am aware finasteride needs at least 6-7 months to start having effects. I will be patient and hopefully see good results. My realistic expectations from finasteride is to prevent any future hair recession and hopefully “stabilize” my hair. I also have a few thin weak hairs on my hairline that I would like to revitalize but I’m not sure if that will happen.
As for a transplant I’m still skeptical about committing to a procedure. I will take time to reasearch good experienced doctors in my area. My hair loss is not too bad it’s mostly a hairline problem. I would be looking to get hairs transplanted into the corners or edges of my hairline. This is all really stressful and depressing but I will try to stay positive.
      Thank you for your insight

Does ketoconazole go systemic when applied topically? It is a pretty potent antiandrogen when taken orally.

I do knkw that there can be some itching, flaking and sometimes folliculitis that accompanies with FPHL, so I wonder if prescribing it for the treatment of that aspect would be beneficial, but for it to be a treatment for FPHL, yes I agree it does not treat that.

There are signs your breast tissue is starting to get "inflamed" and enlarged. But it doesnt mean you will grow permanent tissue. They can grow puffy and hard but its not enough to cause permanent change.
      Those signs are:
      - you will feel a itch around your nipples that you cant scratch away
      - there will be hard or harder tissue below your nipples

I as a female actually used to use this shampoo (Nizoral) about 20 years ago on my hair, but probably more so to have a clean effect on my hair than for hair loss since I wasn't peri-menopausal at the time. But as I am older, wondering if I should go back to trying this again and curious to see if it works? Wondering also after using Minoxidil on my husband, maintaining with this shampoo. I realize as a doctor you have to push the pharma and office procedure agendas, but it just seems like you are against all things natural. It would be nice if you could confirm success stories without medication use.

try any other good quality shampoo that has ketoconazole instead of nizoral cause nizoral dries out your hair so much. I have been using one called regenepure dr and its not harsh in your hair at all, doesn't dry it out but leaves it clean and I have grown more hair than before

Because most natural things simply don't work because if they did then scientists wouldn't waste time and money developing treatments. They'd simply extract from natural sources and sell it rather than doing expensive research and development. Not everything that is natural is good for you and not everything man-made is bad for you. Many studies showcase natural remedies have a very low efficacy if any at all.
