  • Dr Ozello's Sports Medicine Report: Intercostal Muscle Strain
    Intercostal Strain
Intercostal muscular strain is the most common cause of muscular chest wall pain, accounting for up to 50% of cases. Risk increases during intense training or resumption after a prolonged period of rest (off-season). Among MLB players, strains of the intercostal or external/internal oblique muscles accounted for 92% of trunk muscular strains.
    Intercostal Muscles: Anatomy
Mainly involved in the mechanical aspect of breathing. Function to expand and decrease size of chest cavity to facilitate breathing. Three layers.
External Intercostal Muscles: Aid in normal and forced inhalation.
Origin: Ribs 1-11. Insertion: Ribs 2-12. Action: Ribcage expansion through elevation and separation of ribs.
Internal Intercostal Muscles: Aid in forced expiration. Origin: Ribs 2-12. Insertion: Ribs 1-11. Action: Decrease transverse dimensions of ribcage through rib depression and inward bending.
Innermost intercostal muscle: Deepest layer of internal intercostal muscles. Separated by a neurovascular bundle.
Intense sharp, stabbing, tight, spasm, stiffness, dull, deep ache and point tenderness in the intercostal space.
Shallow breathing due to inability to perform deep breathing.
Symptoms increased by stretching, deep inhalation, and coughing.
Limited range of motion in all planes due to pain and muscle guarding.
Slow and guarded motions. Apprehensive to move.
    Altered posture and/or Antalgic position.
Mechanism of Injury: Sudden concentric contraction of intercostals in stretched position or during activities involving excessive or repetitive lateral flexion or rotation.
Baseball, javelin, rowing and ice hockey.
May occur traumatically due to direct impact forcing ribs apart.
Treatment is symptomatic with relative rest and conservative management.

Hey doc, thanks for this content and for the time taken to answer everyone's questions. I've had a pain in the lower left rib area for a month or so now, and I think I've made it worse by prodding it to try and identify where it's localised. I'm planning on resting it now and applying some hot/cold therapy, as well as seeing a practicioner. Interestingly though if I find the spot, it hurts like a stabbing pain and then I can't find it again until I leave it alone for about 30 seconds. Is that normal with this type of strain? I also experience dull aches, especially while hunched forward at a desk.

    • @drdozellodc
      @drdozellodc  Před 2 lety +5

Tenderness is a symptom of a muscle strain. When hunched forward the front of the ribs are compressed so symptoms may increase.

Excellent video, I've been dealing with this condition for 2 weeks and I will be seeing a local chiropractor this Tuesday. I've been taking Advil and a muscle relaxer prescribed by my Doctor. I'm 62 years old and an active tennis player, so I hoped this condition would get better fairly quickly, but I've read that it can take several weeks to heal.

Living in Thailand its impossible to find a doctor or physio who speaks good English where I live. I've had pain from an intercostal muscle strain for 3 weeks now that has simply got more intense during that time. Breathing is no issue (think I use my diaphragm already), but most movements exacerbate it and by the end of a regular day I can barely move my left side without sharp stabbing pains. Anti inflammatories (ibuprofen 400mg x 3 a day) do nothing now. I have muscle relaxers but they didn't explain much about them and I don't like taking drugs without a good understanding of them. I'm starting physio and that didn't help. Actually hurt more. I would take any tips at all possible doc. I'm struggling to do anything physical now and feel like a cripple with no end to this seemingly in sight.

I've been feeling this for quite a while now. Specifically weeks ago. I'm 16 years old. I was consulted by 2 doctors and 1 nurse separately and on different weeks. I forgot what the first doctor diagnosed me with but it had something to do with my muscles. The nurse recommended me gaviscon since I might have acid reflux. The third recommended me warm compress to apply which works so far. I researched and it takes about a week to a month for it to recover depending on the damage. I think my ribs are still healing. Quick question,
    1. Is shaking of the hands and legs related to it?
    2. Can the pain happen randomly?
    3. Is warm compress good? What else should I do?
(Non related question) 4. What can I do to treat my sleepiness most of the time in the morning and my stiff neck?
The pain I feel now isn't much as it did my first few times of experiencing it. Whenever the pain happens, I would apply warm compress to it.

    • @drdozellodc
      @drdozellodc  Před 2 lety +3

Thanks for watching the video and for the questions. 1) Shaking of hands and legs is usually not directly related to an intercostal strain. 2) Yes, the pain can occur randomly but usually occurs with specific motions which either contract or stretch the intercostal muscles. 3) Heat therapy will help increase blood flow to the muscles which will help relax the muscles. 4) Here are links to three videos that may be helpful.

Could it be an intercostal strain? Or is broken ribs more likely? I can breath ok but it hurts to sleep on both the left and right side. When I move my right arm, there's some clicking sound in my right rib cage area. It feels and sounds like when you've had too much water to drink and it gobbles around in your tummy.

Great video thank you so much! I have had a dull pain in my back right ribs for the last year or so (when twisting, deep breathing, or moving my shoulder forward), and I believe it was caused by sitting in a twisted position for several hours without moving while working at my computer. Can this be a cause of intercostal muscle strain?

    • @drdozellodc
      @drdozellodc  Před rokem

Thanks for the feedback on my video. Prolonged sitting in an improper position can be a contributing factor to the development of an Intercostal Muscle Strain. It is possible that your condition is an Intercostal Muscle Strain but there are other possible conditions. It depends on the exact location and type of pain. very interesting question. First of all, If I were you, I would see a medical professional immediately to have my spleen checked. I would not hesitate. In most cases of an enlarged spleen, there is a sensation of fullness. In an intercostal strain, the symptoms increase with ribcage movement.

Great video. I was doing chest supported rows yesterday and let myself fall onto the bench and I also overextended my chest at the top of the reps. It hurt when i finished my set but during the rest period it went away. I was able to do my final set without any discomfort. I didn't think about it for the rest of the day but woke up and my chest is in a bit of pain. Is it likely this is a strain that will recover quickly given that I was able to do another set at the time?

Hello Doctor, just wanted to ask some questions about the treatment for a severe strain, I will be going to the doctor but I don’t have the money or insurance to pay for unnecessary scans. I am a construction worker. I’m guessing the repetitive motion of lifting wood caused me to slightly strain my right side intercostal muscle, I kept working because I had no choice and tried not to lift as much but as my job is heavy I have to. The pain was manageable. Then it started hurting when I laughed or twisted too fast or took a really deep breathe but it didn’t hurt to touch or put pressure on the area. Then today after work I was just laying down and I stretched and heard a pop in the same area. I screamed in pain and since then I have limited movement or else I get intense pain. I have no difficulty breathing but it does hurt really bad when I sneeze or cough or laugh. I’d say the pain is 6/10 but in those instances or a twisting motion or anytime I have to use that muscle the pain is 11/10. I refuse to go if I just go to get sent home with painkillers and a prescription to rest.

Hello Dr. Is it possible to strain the interior intercostal muscles? I have been told I probably have costochondritis but a lot of my symptoms don’t add up to what I see. When I exhale that’s when I feel soreness in my intercostals and behind my sternum. We’ve already done x-rays and MRIs and CT scans and I’ve been told my heart and lungs are fine.
I believe I probably hurt myself when I did a weeks worth of extensive mopping at a commercial building. Sounds ridiculous but I think that’s what happened. I’m pretty much over the most painful part of it all but still recovering and this happened back in late October!
I also would like to add that my thoracic spine was extremely stiff and since stretching it on a foam roller and a back pod as well as physical therapy it has loosened up so much that it now pops in cracks with deep breath‘s whereas before it was too stiff to even move. Also when I get down on the foam roller and get a pop out of my back my ribs hurt much less!

If I take time off I’m worried I’ll lose progress but I don’t want to make this worse. Any thoughts? I am happy to just go for walks for a week or two while it heals but damn… I’m lifting like 10lbs per exercise just to feel a bit of resistance while I work on form. My chest feels like something is heavily compressing against it. When I sit down, and lay back in my recliner, I can’t breathe too well. I can’t even sit regularly, with good posture. I’m constantly having to slop forward to breathe. I can’t perform my usual exercises anymore, because the chest pressure is unbearable. I’m no means out of shape. I exercise regularly. Well I did, till this issue occurred. The crushing feeling completely diminishes when I lay down on my side, or stomach, and my breathing rate is good. Do you have any idea as to what that might be? my muscle ended up taring after two years of feeling a sting on my chest.
I am in a lot of pain.
My chest gets swollen if I stand more than a few minutes I can’t even stand anymore.
I believe it’s caused by bad posture while breastfeeding my kids for many years now.
I am now on complete bed rest and I take 600 ibuprofen, 1000 paracetamol and 50 tramadol 3 times a day.
Will this injury heal ? Will I be able to stand again ? Do you know Anyone that has experienced this type of muscle tare injury and how they healed?
I am so scared. It’s very scary not to be able to use your body for more than a few minutes.
My chest injury is on my left side right under my left breast close to my sternum. It’s warm to the touch. I have now been on full rest for 3 days and been able to get the swelling completely down.
It still hurts quite a lot in the evening even with pain killers. And I try to move very little on the bed. This is awful. I have a 2 year old and a 7 year old. Will I be back on my feet ? How long will this injury take to heal in your opinion ? I find it quite extreme that I can’t stand for more than a couple minutes and I I have to be very careful with my movement to not cause more damage … because when I have moved a bit more I just get worse.
I should mention it does not hurt when I breath only if I breath really deep and expand my ribs a lot which I don’t do with regular breathing .. it also does not hurt when I sneeze or cough.
I did go to my doctor , to the hospital and called a ambulances in the last 2 weeks. They checked did a ecg 3 times , checked my lungs , did 2 X-rays and 2 ultrasound. They can’t see the tare but I feel it it’s clear it’s there and it’s a internal wound.
Warm to the touch and if I would walk more than a few minutes at a time it gets swollen and I can move even less.
Can you give me advice please ?

Hi Doctor, thank you for the video! I started experiencing slight dull aches around my lower right rib area about 5 weeks ago, didn't think much of it and continued my usual swimming sessions and workouts which i think may have made the aches worse. I have stopped exercising recently and the pain seems to have lessened, but i have a couple of questions:
    1. At the start, it used to be painful when i pressed my lower right rib area, but now it's no longer painful when i press it. I only feel pain there when i laugh, cough, sneeze or bend/twist my body in certain positions. And i also started to have this feeling of stiffness/tightness in the surrounding area now. Could I check if this sounds like intercostal muscle strain to you? And if so, does the change in symptoms sound like I'm recovering since the pain has lessened?
    2. If it is likely intercostal muscle strain, I couldn't figure out what may have caused it. Could it be caused by my routine swimming workouts? Poor posture? Or the fact that I only sleep on my right all the time? Not sure if any of these could be the cause.
    3. Generally how long would intercostal muscle strains take to heal, and when can i start exercising again?
Thank you so much for your advice!

Excellent question. If sounds like an intercostal muscle strain since the intercostal muscles contract and relax with the aggravating factors you mentioned. There is a possibility you may have a rib stress fracture or another condition so If I were you I would see a Doctor of Chiropractic for evaluation, imaging, diagnosis and treatment. Most likely the cause was a combination of the factors you mentioned. The length of recovery of an Intercostal Muscle Strain depend on severity. If it is a mild strain you should be able to restart your routine in two to four weeks. Start slowly and gradually increase intensity and volume. If I were you, I would schedule to see a Doctor of Chiropractic for evaluation, diagnosis and treatment. I would ask if an MRI of the Thoracic Spine is needed. I would also see a massage therapist and ask them to concentrate on the intercostal muscles. Here is a link to a video for Oblique Muscle Strain Rehab. You can apply the strategies to an intercostal Muscle Strain czcams.com/video/6PoMQseDxNY/video.html

I wonder if Covid-19 could play a factor is this… I got that stupid virus in January 2nd and went back to work on the 14th when I didn’t feel anymore symptoms. I’m feeling a sharp stabbing in my lower rib and it flairs up when I come from a sitting position to a standing. It’s to the point where I can’t move for a bit. I noticed a slight pain when I was coughing 2 days ago and now it’s driving me to the point I had to leave early from work yesterday. I’m resting for now but this pain is annoying if I make any sudden jerky movements lol… It’s making me wonder if I should even ride my bike today or just lay off anything that’ll cause me heaving breathing.

Thanks for watching and commenting. Coughing and sneezing can cause an intense contraction in the ribcage muscles and may cause a rib to become subluxated (out of place). If I were you I would see a Doctor of Chiropractic for evaluation, diagnosis and treatment.

You may have been lying in a position that placed a larger than normal amount of stress on the intercostals. Most likely, the muscles were fatigued due to prior activity, then you slept in an awkward position.
