These nursing fundamentals of the GI assessment (GI System) for the physical assessment will guide your data gathering as you complete a basic abdominal assessment.

If the patient is an abdominal post op patient e.g. endovascular coiling due to hepatic bleeding and the pain is in control, can we still do that deep and specialised palpation to check the liver?

I use a Littman classic. At first you are listening for one thing: is there sound? If you hear no bowel sounds (gurgles), listen for a full minute. If you hear sounds, then are they faint and far between or so many that there is never a break. Chart what you hear

I have had mine for 15 years. I dont know if they have changed names. It is just the classic. I see a classic II that is like mine

Why did you do Percussion before palpation?
Hi! The palpation should be before percussion- in the abdomen the important thing to note is auscultation needs to be before palpation for a couple of reasons 1) it is said palpation can alter the frequency of resting bowel sounds and 2) if you palpate and it is tender - you may not have the ability to complete your assessment because the patient will refuse
i appreciate this very much. what would be great is to have a video that goes over the proper/expected report. Ex: when reporting color should you just say expected for ethnicity, or include no jaundice, cyanosis, redness, bruising, striae, etc? what would be the minimum acceptable, but still covering everything. thank you!

Under normal conditions you will not feel the spleen - :)

Auscultate the abdomen. Start scratching the skin over the liver and move down. As the sound changes, you have discovered the liver border

Why do you measure surgical scars?
If they are new surgical scars, we need to monitor them healing / or worsening tears. If they are old, it is for documentation.
