Why Does My Nose Look Bigger After Rhinoplasty? | Dr. Jonathan Sykes

  • drjonathansykes.com/ -- When we perform a rhinoplasty we lift the skin off of the infrastructure, which is the cartilage in the bone. After we change that infrastructure, we re-drape the skin. The new contour emerges as the nose heals, but right after surgery the skin thickens and swells, particularly if the patient already has thick skin and big pores.

It can take a while for the skin to re-blanket the nose and for us to see the definition, especially if we are narrowing the tip. Because of that, the nose can look bigger. However, If the patient has thin skin and a very bony, structural nose, once we take down that structure, it immediately looks smaller.
How long does it take for your nose to look normal after rhinoplasty?
The time it takes for your nose to look normal after rhinoplasty depends on the structure of the nose, skin thickness, and what’s being done.
Not all rhinoplasties are the same. Some are first time rhinoplasties while some patients are on their first or second revisions. Some people have thick skin while others have thinner skin. Some patients are just looking for a tip revision while others have a bump on the bridge, a long nose, or wide nose. All of these factors contribute to the complexity of the surgery and the time to recover.
If a patient is having a revision rhinoplasty, the skin will thicken more post-surgery than it did after their primary rhinoplasty. That being said, it typically takes longer to see final results after a revision rhinoplasty. A patient with thick skin is coming in for tip definition, they won’t see results right away, whereas a person with a long bony nose will see changes rather quickly, even though the final result is not apparent yet.
When should I consider revision rhinoplasty?
Since it takes 12 to 18 months to see the final results from a rhinoplasty, and since the nose changes even more after that, you shouldn’t consider a revision rhinoplasty before a year to a year and a half. When I see a nose where too much cartilage has been removed in a prior surgery, I realize that this nose can continue changing for several years.
Because the nose continues to change, a second surgery can be avoided if the patient waits to see if they like their final result. On the flip side, some patients might be initially happy with their nose job, but over time they become dissatisfied. The nose might look okay for a year or two after surgery, but as it contracts over the years, it can become pinched or overly upturned. That’s typically when I see patients, but sometimes people become impatient and if their nose is not perfect when the bandages come off, they are unhappy.
I just removed the split off of a patient’s nose, and as soon as they looked in the mirror they said, “I’m already considering a revision”. What? This patient had large pores and that larger pore formation is going to prevent the skin from shrinking and re-draping right away over the new nose structure. I made sure the patient knew this before surgery, but many times my words are forgotten during the excitement of a patient seeing their new nose for the first time, but it’s important to remember that the nose will continue to get smaller over time.
The most common question that you will see on the Internet from those who have had a rhinoplasty is, “It’s three weeks after surgery and my nose looks too big. Will it get smaller?” Yes, it will get smaller. Will it be exactly what they want? I can never say that for sure, but I do know that it’s going to get smaller. It’s going to wound-contract and the swelling is going to go away.
If you have an issue from a prior rhinoplasty and you’re not particularly confident in what’s going on, I’ll be glad to talk to you. Call or email me any time. I help patients that have had prior nasal surgery all the time. Give me a call.

My doctor never told me any of this. I had a revision rhinoplasty, septoplasty (6.5 hours). He fixed a mess from the last doctor and I had a deviated septum. I’m worried that my columella is not going to be flat. It wasn’t flat out of surgery, after everyone else’s rhinoplasty vlogs that I see, show their columella flat, mine wasn’t. That was my main concern. I’ll be 1 month post op this week. Trying to stay positive and to be patient. 🙏

I keep rewatching this video to comfort myself. I had an osteotomy septorhinoplasty. I’m 5 days post op. My nose is like a pig snout, baked potato, a massive thing in the middle of my face. Even massive between my eyes. I am psychologically traumatised, I see photo of my old nose and I feel so sad I’ve lost a part of me in place of an upturned potatoe. I find it difficult to imagine this Nose to disappear in a months time. In the meantime I’ll keep this video on repeat.

Looks unnatural on my face and when I try to smile, everything is pulling and I feel like Micheal Jackson. Devastated. Psychologically traumatised and im keeping this threat of hope that things will be different at the 3rd month mark.

Hey all, an update. I am in my 6th month. Looking back at previous photos, I can assure you my nose was still swollen at 2/3 months. I would say I am now feeling mostly normal. I guess we all heal at different rates and it really depends what kind of work was done. I am not sure I am happy with the shape of the nose however Im not going to try correcting it as it's not too much of an issue for me as its not botched job. I don't want to go down a rabbit hole of chasing perfection. Ironically I miss my old droopy nose. I think what helped me a lot in the healing process is when I focused my attention on work... anyways I know people get their nose fixed for valid personal reasons, but I think we all need to accept the unique idiosyncrasies of our faces.... those who are fresh out of surgery - don't look at the mirror, focus on what you enjoy and be patient.

I am someone who had a boney/large dorsal hump. I’m two weeks post op and the first thing I noticed when my brace came off was that I still have a bump. I don’t have an arch at all like my simulated photo had. I was told it is callous and will go away on its own in a few months and if it doesn’t I’ll be given a steroid injection. Aside from that my nose looked really cute and then about 4 days after brace removal my nose dropped more than I expected and my nose essentially looks exactly the same as my old nose. Not only because of the bump but also the tip as well!

I watched this after my first nose job and I did end up having a revision rhinoplasty one year and five months later. I do have thick skin but my surgeon admitted he just didn’t make my nose as small as I had asked. Not on purpose of course, I guess I just wasn’t clear enough with him.

I'm 4 weeks post-open rhinoplasty { "true thick skin" } & this rings true for me. When the cast came off, my nose looked HUGE. It was a discoloured potato for the first few days, but with the help of tape, the swelling dramatically subsided within the week. Now 4 weeks in, it certainly feels & looks like 90% of the swelling has subsided. I know this still isn't the end result, but I am already getting a very good idea of how it's going to look, & my nose feels much, much stronger. If you have thick skin, you definitely want to tape & keep the tape on for as long as you can. Take daily walks,. Gently exfoliate. Sleep slightly upright.

To anyone reading this who's going through or are about to go through this process or even if you've past your 1 to 3 month mark. I'm 6 days post-op and my cast comes off tomorrow! I'm excited because I can't believe it's already been a week pretty much since I laid myself down on that surgery room bed.
I came in to this process with a very core understanding that beauty is pain and that I shall never let myself believe my nose results will be what I see immediately after the surgery up to 3-6 months. Surely I'll be excited to get to my 3 week mark because I understand that by then a significant amount of swelling will have dissipated. Not accepting that as my end results either but part of the continued process of healing. From there look forward to my 1 month and then 3 month and then 6 month and so forth. Because I know that little by little my nose is attaining the shape I have gone under the knife for and that my best bet would be to look forward to the 1 year mark. But in the meantime I'm enjoying the process. See??
I think it would help to remind yourselves that no matter how awful it looks straight out of surgery to the 3-4 month mark that That is just a progression into what you are ultimately desiring in the end and all you need to know is that it will come but of course it will also take time for your body to show you how far you've come along 😊.

This will help in getting rid of black and white heads, decrease oiliness make the pores smaller, especially if your taping your nose (as oil and dirt might build up). Do note that your skin will be extra sensitive to sunlight as a result of your surgery. and with the use of acids it will be even more sensitive, so avoid sunlight and wear your sunscreen all the time. I had septorhinoplasty 3 weeks ago and everything what the doctor said here is true. Your nose will get smaller and smaller every single day. I have a tick skin and right after the surgery I saw the bridge completely different (straight and smaller), but there was a lot of swelling in the tip. But then everything started to change. So don't worry, you will see a lot changes in the first 8 weeks or more. Just be patient and good look :)

@@farahmajeedrasool4544 everything will be ok :) few more tips, the procedure is painless, but the week after it will be uncomfortable(you will not be able to breathe through your nose) . Talk to your surgeon regarding the shape you would like to achieve and the size. Remember that if you decrease too much the size it might affect your breathing, so hopefully the doctor will advise you here! And if it is slightly bigger you can always make a revision after a year. Also I had closed septorhinoplasty and probably the recovery is slightly faster than the open one. So take also in consideration what kind of sergery you will have. And finally be patient, don't judge the result before 6-8 weeks. Sometimes I see my nose really small and sometimes(after hot shower) it's way too swelled. Good luck and I'm pretty sure you will be happy!

@@contactshahid8614 There are so many times I feel like I've made the mistake of my life. I often feel like it looks coarser than my old nose. When my edema is felt very little, I am not very dissatisfied with my surgery. But even my bridge feels very swollen sometimes. I have scar tissue on the tip of my nose and on the sides of my nose, which I can feel even when my edema is low... Even the skin on the bones where my nose meets my cheeks doesn't seem to fit on my bones because it's so loose that it collapses when touched with my hand. Moreover, the middle part between the bridge and the tip of my nose looks sunken compared to the hollow bone and the tip. After all this nonsense, my mental health isn't the best. It's not exactly the look I want right now. Except for the appearance of my nose, I have no problem breathing. The cartilage inside my nostril can be felt in my hand. The right hole looks smaller than the left hole... There are no stitches on my wings. The doctor didn't touch my wings from the outside either, so I don't understand why this is the case right now. Moreover, when I put my finger into my nose to look at the tip of the right hole, there is another cartilage. I did not find any cartilage in the inner tip of the left hole. ..There are more ridiculous situations, but I don't want to talk about it anymore, my own nose was more beautiful. Moreover, I waited 8 months for this nonsense to be fixed. In short, I regret
