Herbal medicine can be used to increase and normalise gut motility. We call these herbs herbal prokinetics. They include herbs like ginger, globe artichoke, iberogast, turkey rhubarb, immature bitter orange and magnolia bark among others. I will use herbal prokinetics in a number of different functional digestive disorders including gastroparesis, or delayed gastric emptying, small intestinal bacterial overgrowth, or SIBO and irritable bowel syndrome.
I laughed when you said "bucketloads of ginger" 🤣 it's the truth- I use a ton of ginger, too (as long at the patient doesnt have gastritis, in which case they will likely hate ginger). It works really well! And in addition to being a prokinetic it is anti-inflammatory, anti-histamine, and is a biofilm disruptor for bad critters. Ginger for the win!
Hi. How much fresh ginger per meal would you advise please? And is it before or after meals?
Will it work in powder form?

I found using fresh ginger for tea gave me horrible long lasting heartburn. I had some supplement capsules & opening the caps into hot water for a cup of tea worked for me - no burn. Thought if anyone else is having this issue it might help. I suppose I could use the powder from a supermarket herb but I figure the measured dose in a cap is probably ideal & possibly the production of a supplement may be higher quality.

Great move for motility. You may need some stronger antimicrobials to shift the bacterial balance too

Hi, excellent video as per usual. I'm half way through my 8 week course of antimicrobials. (Oregano and Allicin) im not really seeing any improvement and my only symtom was bloating (no pain). Would it be okay to start prokinetcs now or shohld you wait till the antimicrobials are finished?
I read a couple of research papers that said that iberogast is dangerous to the liver.
What do you do with your patients who take iberogast to decrease the risk of liver problems?

I purchased Iberogast but it seems a bit too harsh for my sensitive system. Maybe I just need to go slowly with it. In the past bitters have helped me a lot, but then all of the sudden they started causing reflux. The same can happen to me with too much ginger, but a little can help a lot. I practically life off of DGL and have found that a cold infusion of marshmallow is nice. I've also tried slippery elm. I wonder if they can block the absorption of nutrients, though? I have H.Pylori and I may have SIBO so I've probably got enough trouble in that department already. Seems like the methane variety as I tend toward constipation. I heard you say that Iberogast it can decrease " visceral hypersensitivity" I thought- aha! that sounds like the name for what I feel. I am going to give it another whirl, thanks. Haven't ever tried meadowsweet but it sounds lovely.
Same here with ginger, acid reflux and heartburn. Did you try artichokes?

Helpful video! The celandine in Iberogast has been linked to liver damages and liver failures in patients in the past. Bayer had to add a warning label a couple of years ago.

Aha, that's why it is "not available" in any of my Swedish pharmacies...just says "out of stock"...Yet on Amazon Germany I found Iberogast that says it's made by Bayer and there is no celandine in the ingredients list. Maybe Bayer changed the formula.

Magnolia bark is the best for sleep. How high of doses do you recommend? There are all these articles trying to attack herbs saying if you take too much its bad for your kidneys etc. Seems like you can find one on almost any herb unless its cookie cutter chamomille or something like that.

Wondering...since it is impossible for you to work with those of us who are international, is it possible to create an online course? That seems to be the approach used by many people now. I understand it leaves a lot up to the individual to interpret and be their own therapist, and there is probably lots of error. But there are many of us who are committed to helping ourselves and would do the work as well as listen to reason. It does involve taking responsibility for ourselves instead of relying on someone else with more training and experience (the down side) but it can be the doorway to a better, healthier life.

You're right on. I have sulfur problems and unfortunately I cant digest and absorb taurine. I tried it before and it just feeds h2s bugs instead. I may try it sublingually under the tongue. Hoping it doesn't taste bad in powder form lol

I tried the motility activator from Integrative therapy. Artichoke leaf extract and ginger. I eat a lot of ginger already. I discovered an allergy to Artichoke leaf extract. The warning states if you have the Ragweed allergy. I know now. My eyes itch and my sinuses ache.

Just thought I'd write and say I tried the garlic,fennel,oil of oregano,and atrantil and it cleared up my stomach full of ulcers.is there anything that you would use for TMAU and SIBO?Also stubborn odors?
Great input! So glad you found help with the ulcers. SIBO can be complex and generally takes a layered approach of herbal medicine, probiotics and prebiotics too
@@ByronHerbalist Hello Doctor,Any tips for non alcoholic fatty liver and pancreas "fatty"tumors,and it took a little time but 9 ulcers,now that's impressive.p.s.they aren't sure about type of pancreatic tumor,but assume since I am overweight, forget the back pain I have had since before 2018,it must be fatty also.
How long are you supposed to take prokinetics for? How many drops of iberogast do you use and how many times a day? Same question with how to use ginger and artichoke?

I'm a herbalist in Canada and am trying to find evidence for how progesterone (Prometrium) impacts the motility of the small intestine? Have you happened to come across anything on this? Specifically, I want to know if a peri-menopausal woman (with vinculin antibodies) diagnosed with hyperplasia WITHOUT atypia, should avoid taking progesterone?

Hey byron, since 2 years my start of poop is hard remaining is normal, tried many things only lactulose suits me.. Can u suggest something herbal in my case to have soft poop, without having side effects and for long term use. Thankx
Had constipation and took unsweetened yogurt. No more gas and hard stools. They have almond, oats and lactose free yogurts also. Heard celery juice works also. I just started this.
What about Triphala?

Is getting raw ginger and raw artichoke from grocery store different from getting in a pill in extract form? Would you recommend getting it in pill form?
Supplements are much stronger but food can really help!

Artichoke leaf extract is free of carbohydrates and this has zero fodmaps

Im always bloated and constipated 😢... ive cut downleafy greens, specually broccoli 🥦......i have tried keto and carnivore diets and i think it does helpnwith the bloating but ive noticed that i have no motility if i cut the carbs. So i eat carbs and bam i can poop 🎉🎉🎉 lol

I am on motilpro after SIBO treatment but I don’t feel it working at all! I am so bloated and it’s causing me to be constipated sometimes. What’s another natural prokinetic that I can try? I don’t know if the ginger is causing me to feel inflamed

well this sucks, can't find anything about Turkey Rhubarb in the Netherlands. Only about regular Rhubarb. Can't find any store that sells capsules on them or anything. Will keep looking.
Most Chinese medicine practitioners will know this herb. Try there asking for da huang. At higher doses it is more of a laxative too!

Todd How much ginger do you recommend a day? I've been taking 400mg twice a day. Maybe that's not enough to get things moving. Any advice thank you?
It depends on the case but anywhere up to 1-2 grams can be helpful, sometimes bigger doses in one go can speed things up on the motility front. It won’t help as a laxative so much, we have other great herbs for that issue!

Try triphala from Ayurveda
I don’t agree with taking iberogast until your sibo is clear. Iberogast has licorice in it which is a mucilaginous herb. Due to the high level of mucopolysaccharides in licorice, that could encourage bacterial regrowth.
great input! I haven't found licorice to be an issue in these doses for most SIBO cases.
Why use turkey rhubarb if it can only be used short term? If some one is having motility issues wouldn't the symptoms just return after they stopped using the herb?
treat the root cause and you won't need herbs ongoing

Buy the root, scrape off the skin with the back of a large spoon, cut about 1/2 inch & chop, then put in a cup/mug add boiling water, steep as long as you like (I leave it in the mug) & drink - way cheaper & more soothing than caps.
Ya you could buy bulk ginger powder and add it to smoothies
