  • Can FSH or AMH prove and predict your fertility? In this video, Mark P. Trolice, M.D., a board certified reproductive endocrinology and infertility specialist and founder of Fertility CARE: The IVF Center shares his insights.
“Follicle-Stimulating Hormone (FSH) has been the gold standard for many years of ovarian-age testing,” says Dr. Trolice. “FSH is a pituitary hormone used to stimulate the follicle that’s going to develop the egg and ovulate that month. The higher the level of FSH early in a menstrual cycle, the lower the number of eggs.”
“However, FSH is not a very reliable test,” Dr. Trolice continues. “It can fluctuate from month to month and it is very limited because it has to be done at a certain day in the menstrual cycle and the test has a number of false negatives. Even if a FSH level is normal, you could still have ovarian aging that wasn’t detected that month.”
“Anti-Mullerian Hormone (AMH) is the test we use exclusively at Fertility CARE, and most reproductive centers have stopped using FSH and now test only with AMH,” says Dr. Trolice. “AMH is a hormone produced in the cells surrounding the egg. Therefore, the lower the number of eggs, the lower the AMH, and the higher the ovarian aging.”
“The question is what do they really reflect? Can they predict fertility?” asks Dr. Trolice.
“A recent study from North Carolina published in the Journal of the American Medical Association, looked at women between the ages of 30 and 44, who had lower levels of AMH and FSH and compared them to women of the same age with normal FSH and AMH levels,” says Dr. Trolice. “And there were no difference in pregnancy rates between the two.”
“But there are certain caveats though,” says Dr. Trolice. “First off, they did not include any fertility problems in the man or the woman. Second, they did not include patients who had subsequent use of fertility medication, so we did not know if that was included. And third, the pregnancy tests were just the positive pregnancy tests. They didn’t talk about ongoing pregnancies or live birth. We don’t know if low AMH or high FSH would have resulted in a higher rate of miscarriage.”
“The bottom line,” concludes Dr. Trolice. “Unless you’ve been trying for adequate time to conceive, I think getting tested for AMH or FSH levels is premature. The younger you are, irrespective of these test results, you should still have good success because you still have good quality eggs.”

Testing ovarian reserve - the quality and quantity of a woman’s eggs, can help you decide whether you should consider pregnancy sooner rather than later. If you should freeze your eggs, or whether fertility treatment may be successful. There are three tests that doctors use to predict ovarian reserve. Like the one Bio tex use. FSH, AMH, and AFC. FSH (follicle stimulating hormone) is a hormone that is secreted in the cells of the ovaries and at one time was the gold standard for predicting ovarian reserve. While it can be a predictor of conception. It fluctuates during the cycle and therefore is tested on day 3 of the menstrual cycle. A day 3 FSH level of 10 is generally considered normal. A day 3 FSH level above 15 is considered “high FSH” or low ovarian reserve. AMH (anti Mullerian hormone) is a more reliable predictor of ovarian reserve if done properly. Because there are eight different assays, tests that are done to determine AMH levels - standardizes and normalizing results to correctly interpret AMH levels. AMH levels continue to drop as a woman ages, and are essentially non-existent at menopause. AFC (antral follicle count), a measurement of follicles on the ovaries, viewed with a vaginal ultrasound, is just as good as indicator of ovarian reserve as AMH. One caveat with testing AMH and AFC, he adds, is that you cannot be on birth control pills when your levels are measured; they decrease when you are on the pill. Levels can be measured at least 30 days after you stop taking the pill and after you’ve had one normal menstrual cycle.

@@mariapizarrosath8147 That number of 10 is a cut off point for fertility clinics only, not your own natural ovulation. They use this number when considering harvesting your own eggs in an assisted fertility treatment cycle (ie be your own egg donor). At above 10 point you might only get 1 - 4 eggs which usually results in 0, occasionally - 1 healthy fertilized egg which has a low chance of implanting. With such low numbers they will advise you to get an egg donor if you wish do do fertility treatment. If you are just cycling normally, yes you may be ovulating every month and have a better (but still very very slim) chance of getting naturally pregnant if your partner is fertile compared to fertility treatment with your own eggs (at this stage of science development, near nil). At your age your choice is best try every month to get pregnant with a fertile partner, or trying fertility treatment with an egg donor (highest chance of pregnancy). Over 10 - 12 AMH at your age is not worth the cost and medical risk of doing fertility treatment with your own eggs as you barely have any left and what they do get will probably not fertilize and implant.
