Now I am able to use freestyle Libre’ (much more affordable for me) also made by Abbott. I very surprised there is not a loop system between Omnipod & Libre’. Great info and there is no doubt all of these devices are better than multiple injections per day. For me I do not see the logic of having to have a 3rd piece. Medtronic definitely had some drawbacks but you can't accidentally leave home and be unable to manage your condition because the pump and sensor do everything! I can leave home without my phone, rare but it happens. I can leave home without a PDM.

Question 1) Will the Omnipod 5 be available to children this age? 2) In the UK, once you decide on a pump, you sort of need to stick to it for 4 years due to funding restrictions. If we went ahead with the Dash, when the 5 is available, could we switch?

    Why is the target range so high! Very annoying when I have to add on extra carbs to make my own adjustments on my tslim. Phone control is dreamy though, definitely looking to change to omnipod 5 when my renewal time arrives. What is the situation of changing phone? Will you have to change patch when you set up a new phone or can you reconnect with the new phone after disconnecting from old?

    • @Nerdabetic
      @Nerdabetic  Před 2 lety +4

      Great question on changing phones- at the moment, you would need to do that when you changing your omnipod as each pods connects with one device ( like your old phone). On target, 110 mg/dL is in line with other closed loops. Personally, I focus on ‘time in happiness’- does the system reduce my burden and allow me to make diabetes a smaller part of my life but I completely understand that 110 mg/dL might be too high for you. I’m sure they working on allowing more customisation in the future.

      @@palaHD the problem I have here is that they see it as 1 pump (the current version) and you won't get the next version untill the 4 years are up. Which I think is ridiculous since the bulk of the cost is in the pods and not the cheap arse phone. Especially when they let you use the app.

I have a few friends using T-Slim and they love it but do express some disappointment as to bring down high blood sugars and one uses the "sleep" mode all day. I have used the Omnipod for 6 years. I attempted to use the Medtronic closed loops system for about 2 weeks. What a disaster. No support, alarms going off, bent canuala, yada yada yada. After the CGM this is the most exciting control item out there. I have had diabetes for 48 years so I go back to urine testing via a test tube. The pump has been strongly recommended by her diabetes team (Nhs) they advises all under fives to use a pump because of their age. Better management better quality of life. @@sharooney I'm fully aware, I'm a type 1. it's disconcerting people don't realize what a pump really is, it's just a big syringe pumping insulin. it's no better or easier than shots, one's adjusting pump basal rates. ISF, ICR's all the time. Type 1 diabetes is not linear or stagnant. Most people go on pumps to control their basal rates, usually b/c of DP or heavily active - athletic, etc..Can't imagine putting a 4 year old on a pump especially just diagnosed. There's NO way she could even be stable right now. Everyone thinks going on a pump is easy..IT IS NOT; tubes can come out, clog, pods can come off, not good absorption, scar tissue, painful inserts, rashes from adhesives. no way in hell any endo I know would put a newly Dx only 3 months baby on a pump, or even an adult for that matter. SHE'S JUST DIAGNOSED! My god, do you even know what you're doing at this point. Who's gonna be administering the pump, PDM, etc? Does she wear a Dexcom. do that first. I'm 10 years into this Type 1 nightmare, and it is a nightmare. Everything changes insulin needs, ESPECIALLY ESPECIALLY WITH A GROWING CHILD, SHE'S JUST A BABY. Do you know anything about pumps b/c I think you're not understanding the work involved. IT IS NOT EASIER!

    Are there any prospects for Omnipod (or ANY infusion system) to gain approval for U-500 insulin? I have good control of my type 2 with MDI, but it takes well over 200 units daily (like 230-250) - going through 2 infusion pumps per day isn't very practical, and I'm not very interested in going off label. Still 6 to 8 injections/day is a real pain in the...belly.

    • @David-ji7qh
      @David-ji7qh Před 2 lety +1

      @@SharpLife4 U500 is a fairly dangerous formulation, and not a straight 5:1 substitution for U100. Just as we all have slightly different activity curves for each "brand" of regular U100 (human replicate, analog, manufacturer), U500 is generally quite different and stacking is much more difficult to calculate and manage since the curve is generally much longer and more complex: it's not the straight 25%/hour that most of us use for U100. With U500 it's more like 25% 1st hour, 15% 2nd & 3rd, then 10% (or less) per hour to exhaustion. With MDI that can be managed with some difficulty, but not so much with continuous dosing. If the infusion software doesn't know what it's doing...well, that's a risk that neither ME nor my Endo want to take. Six or 8 daily injections (U100) is inconvenient and I still have occasional "dips" that need (oral) glucose/carb correction, but I've never felt the need to maintain a rescue injector - and don't want to ever feel that threatened by my therapy/treatment regime. Plus, I'm at 5.5 HbA1c on MDI; as my doctor says "Your A1c is normal: we don't mess with success." We certainly don't want to take unneeded risks when inconvenient is working this well.
      After all, if the pump makers and regulators thought there was a safe and effective way to do it, it wouldn't be off label.

    Thanks for the video! One thing I haven’t haven’t heard discussed yet (just listened to the Juicebox Podcast episode) is whether there will be a way to enter insulin taken outside the Omnipod, such as an Afrezza or IM dose. Or if not, will the Omnipod 5 be able to handle something like an Afrezza dose without getting completely messed up? Have you heard anything about that?

      FDA approval is used as a reference when submitting Omnipod 5 to Health Canada. Think of it as using your old manger as a CV reference. Now that Omnipod 5 has FDA approval, Insulet can submit it for Canadian approval.

      @@SharpLife4 But.......Is the Omnipod 5 authorized to use U200 insulin? Does the computer know to cut all of the insulin doses plus and minus by half when injecting them into your body? The Omnipod 5 is an automatic pump to keep your blood sugars at a set target. It changes the amount of basal based on the blood sugar reading from the Dexcom6. If your blood sugar is trending up, its computer will say give .15 units every five minutes. Of U100 insulin. But if you are using U200, you really only need .075 units. The Omnipod 5 has to be programmed to use U200 insulin instead of U100 insulin. Or have an option to choose. Or maybe the software will learn how much your blood sugar moves based on the amount of insulin given and figure things out over time. Maybe.

      @@russellseaton2014 I have the regular Omnipod. You have to double some settings and half down settings. Dr was very clear it had to be set up by a nurse who know how to do it that it. It’s prescribed off label. You can’t just tell the pump and it’s u200. You have to set it up as those your dose is half and down settings are Double for the right equations. But it’s only advised through the dr but works well. Many things can be done off label. Like how I wife uses metformin in type one diabetes.

      It's not the entire system on pod as a few parts are missing: enter carbs, alarms, change activity profile.
      It's more like a very advanced fallback in case your phone is discharged or out of range. Which is definitely a new level of safety. Also it can be useful for physical activities but still I'd prefer to have a control panel on smart watch.
      Looking forward to see distributed smart insulin delivery system (pod, cgm, smart watch, smart band, phone, smart glasses, cloud app) which works in any connection chain providing max time in range with minimum interactions.
