Both Anti CCP & RF play significant role in the diagnosis of Rheumatoid arthritis ( RA ) .
However Anti CCP is more specific in the diagnosis of RA.
If Anti CCP & RF : Both are positive
The patient has Rheumatoid Arthritis ( RA)
IF Anti CCP & RF : Both are negative
The patient most likely does not have RA
IF Anti CCP is Positive & RF Negative
The patient is most likely having RA , but it may be in the initial stages. Needs clinical correlation on physical examination
IF Anti CCP is negative & RF is positive
The patient needs to be diagnosed entirely based on the clinical signs & Symptoms.

Chances of Rheumatoid arthritis is less possible , However, clinical features are the single most important factor for you in the diagnosis.
Anti CCP is more specific test when compared to RF test. Meaning , chances of some one having Rheumatoid arthritis if Anti CCP is Negative is unusual .
RF always helps in supportive diagnosis but not a diagnostic test. It can be increased in so many other conditions

@@interpretyourlabtests7734 hi Dr, i also hav the same probl. My RF is elevated exceeded doubled the cut off. My anti ccp is 0.5 which i believe is negative. However my feet and hands are stiffs & painful throughout the day. Izit a case of beginning stage of RA? Is there any tests other than RF and anti ccp to further confirm the diagnosis? Tks.

I tested positive for Anti CCP with a test of 12.9, the reference range was greater than 2.9 positive. Everywhere I look online though it says a value of greater than 20 is positive. Why the different reference ranges? They all measure in u/ml.
In the U.K., over 7 is positive. They just don’t have a consensus yet, which is daft.

My rf was 14 and my ccp was 16 i have hard knots and tiny red dots pop up on me during the ending of my flare and running 99.5 fever the flare was terrible lasting 8 weeks i have been diagnosed with fibromyalgia in 2019 but flares are not like fibromyalgia its deeper im not sure but all my symptoms come up as RA
You are wrong on one important point: you state if RF and anti-ccp are negative, there is no RA - 20% of RA patients return normal results on these tests.

I need more clear information. My Anti CCP result is of 166.4 u/ml HOW???? I see some people here in the 5-20 range and a few in the hundreds... How is it possible that there is such a big gap? Is being in the hundreads really concerning? Like more than knowing we have RA?

I have had the anticcp test and a RA factor test and exrays, all came back negative, however I’ve been feeling stiffness in my fingers in the morning along with one of my big toes....I’ve been waiting almost 3 months for my RA Specialist appointment.....what does this mean? Negative Anti CCP and Negative RA factor almost always rules out Rheumatoid arthritis .

It is just the same as Anti CCP. "Anti " indicates antibodies against cyclic cit peptide . There are 2 types of antibodies used in diagnostics. One is IgG and the other is IgM . The antibodies which are used in rheumatoid arthritis is of IgG type. Hence it's called as Cyclic Cit Peptide antibody IgG or Anti CCP

Anti CCP is the best test to make the initial diagnosis but not to monitor the disease.
There is usually no need to repeat Anti CCP levels every time, because it is of very less clinical value. Levels of Anti CCP usually will stay elevated for more than 3 years.
Disease progression is decided solely on the symptoms like joint pains and swelling.
Please consult an Rheumatologist for further evaluation.
