The most important things to consider is the air seal and the tension rings.
The best tension rings are the erecaid tension bands and can be used with most cylinders. The erecaid tension band loader is specifically designed for their very expensive pump systems. You don't need the loader if you have enough strength in your hands. If you can open a jar of pickles then you are strong enough. Look up on their website to determine the best tension band for you. I found these available for about $23 USD including shipping on the internet, just google them and you will find them. Just make sure you load them on the base of the cylinder with the U notch oriented at the bottom of your penis.
The next important thing is the air seal. The things that will affect your air seal is the quality of the pump and release valve, the use of lubricant around the base of your penis and how much pubic hair you have. Pubic hair will interfere with the air seal at the base but you can mitigate that with lubricant. Most medical grade pumps come with lubricant but Astroglide and other lubricants intended for lubricating genitals will work. You may also find an inexpensive lotion also works but don't use one with alcohol in it. Remove all pubic hair from your penis and outwards about half an inch to an inch from the base and up the shaft. While plucking is the most uncomfortable way to do this it is the most effective though watch out for ingrown hairs. Shaving will cause stubble which may be uncomfortable. Do not use hair removing creams unless you have a high tolerance for burning sensations. Trimming the hair close but not short enough to result in stubble will take care of about 80-90% of air leakage from around the base and the lubricant will bring that to about 95%-99% leak free but you will most likely still have a little leakage. No matter how much you pay for your pump, it will still leak some air and that goes for the valve as well. But more expensive pumps will result in less air. This is what I found. The $20 pump/cylinder/gasket system that I bought on Amazon worked 95% as good as the $400 system I bought two years ago when I used the erecaid tension bands, kept my hair trimmed short and used enough lube. So if you think it's worth the extra $360 just to get a 5%-10% improvement in air leakage go for it. Also note that air leakage if you follow my tips isn't a big problem because your penis should only be inside the pump long enough to get it erect and put the tension ring on. Once you've got the ring on the pump has completed it's task and you just press the release valve and take the pump off anyways. At this point the tension ring is what is keeping your penis erect and usable for intercourse.
I have diabetes which has resulted in nerve damage that completely prevents an erection. but with a pump following the above I can achieve an erection and maintain it for roughly 15-40 minutes. Don't leave the tension band on any longer than that and less than 30 minutes if you can. It's not good to restrict nerve and blood flow for long periods of time and I don't recommend using a pump more than two or three times every twenty four hours and even that can cause some bruising and possible chafing of tissue. 3-4 times a week is ideal but if you have diabetes you don't heal as fast so I would not recommend more than 3-4 times per week but consult your doctor they may have additional information for you. If you don't have a medical condition that contraindicates the use of Viagra or similar drugs you should have no problem combining Viagra and a pump for added effectiveness. Again consult your doctor.
So bottom line is you don't need an expensive pump if you use lubricant, trim your pubic hair close and use a good tension band correctly.

Please be aware that every pump works differently and the key to maintaining an erection with a pump is the constriction system. I’m using the Bathmate and it is amazing! This pump works! I’m in my mid 40’s and started to have ED. My penis isn’t pretty big too 4 1/2” length and 4” girth. Wife was hardly ever satisfied and I have always have to please her by oral and a dildo to finish her up. My wife had went under surgery so we can’t have anymore children.
