How to Do a Tracheostomy Exchange at Home. How to Change a Trach Tube. Life with a Vent

  • Doing a tracheostomy tube exchange (also known as a tracheostomy tube change) is an easy procedure to do at home. You just need a handful of supplies and some confidence. The biggest thing to remember during the procedure is to stay calm. During this video, I will explain the supplies you will need and will explain in a step-by-step fashion how to do a tracheostomy exchange. I will also address some complications which may arise and solutions to these challenges. #lifewithavent #tracheostomy #respiratorycare
    Supplies needed to do a tracheostomy exchange:
    Trach tube
    trach tie/trach tube holder
    scissors
    lubricant
    gloves
    gauze
    mirror (if doing trach exchange on yourself)
    Steps for doing a trach exchange:
    1. WASH YOUR HANDS!
    2. Carefully open the tracheotomy tube package.
    3. Open the plastic sleeve of the trach tube holder. Adjust the trach tie around your neck, cutting off any extra with a scissors.
    4. Take gauze and clean your tracheostomy area.
    5. If you have a cuffed trach tube, deflate the cuff.
    6. Wash your hands again. Put on gloves.
    7. Carefully remove the obturator from the package, only touching the round end. Insert obturator into trach tube.
    8. Once the obturator is inserted into the trach tube, place a small amount of lubricant on the pointed tip of the obturator.
    9. Disconnect from the ventilator.
    10. Cut the trach tube holder with a scissors.
    11. Pull out the old trach tube. Use gauze to wipe off the tracheostomy area.
    12. Insert the new trach tube. You will need to insert the tube at an angle, push in and then twist the trach tube into the correct position.
    13. Once the trach tube is in place, immediately remove the obturator.
    14. After the trach tube is in place, attach the trach tube holder. Once the trach tube holder is attached, reconnect the ventilator if needed.
    15. Clean up the trach site with gauze.

    My wife is new to a trach nearing 9 months now. We finally got her home and this is the one thing that I have not been shown how to do. I will say that I was scared watching you perform this and know when the time comes for me to do this for my wife, I will be a nervous wreck.       Hello. Thank you for sharing some of your story with me. It may be possible for the first trach exchange you do to have it supervised by someone who is trained on how to do it. If your wife has a respiratory therapist who does home visits, arrange to do a trach exchange when the respiratory therapist is at your home.
      Another option is to ask whoever is managing her trach tube if you could come into the office for the first trach exchange and have someone available to help in case you need it.
      If someone does supervise your first trach exchange, please do as much as you can (preferably the whole procedure) by yourself. The person who is supervising is just there in case you have a question or forget what to do next.

      Hi Robert There is not much help out there. I am trying to teach others as well. The key is break the inset into 3 stages, it will become one seamless move over time. Put in sideward then push in and twist down and drops into place. while trying to look calm for your wife. I wish you well

      I usually get a doctor’s appointment and let them change mine because I’m paralyzed and have limited use of my hands. My trach fell out once because I use a dog tag chain as my trach tie, and it disconnected somehow. I had to learn how to insert it on the fly.

    My husband currently in hospital. First had ventilator through his mouth then has through neck. Unlikely this will be long term fortunately. Your videos are helping me to understand more so I can be a good care giver for him.

    This is wonderful, my husband has a fairly new trach, the docs are suggesting it be changed monthly (he has recurrent aspiration pnuemonia, I think they should extend the antibiotics longer than a typical dose). Anyway, he wanted me to change his trach, I watched this twice and felt confident enough to do it myself. It went wonderfully. I have subscribed and began to watch more of your videos.     Thank you so much for the demonstration. Tomorrow will be my second time replacing the tracheostomy tube on my father. I'm trying to find the courage to do it again. The first time was pretty traumatic. Hopefully this time I will feel more comfortable doing it.

    It has been a year since my mom is in coma and has her traecheostomy tube for about yhe same time.For us,we get the help of a respiratory therapist to change the tube,but every time i am scared like crazy.Watching you do it yourself has given me the mental boost for her next change of tube..God bless you ❤️

    My husband has MS and last year in September he had a seizure and respiratory failure another respiratory complications.
    He got a tracheotomy and started to use oxygen support and now ventilator during the night and trach collar during the day.
    The doctor says the trach and ventilator will be for the rest of his life because the weakness.

    RN here with a trach for 20 years. Great video. Couple of pointers. When I set up the new tube, I open the tube package like you did, but then I squirt the lubricant onto the inside of the top, which is sterile. The container or packet is not sterile, so you want to do this before putting on gloves. After putting on gloves and putting the Obturator inside the tube, I then touch the tip to the lubricant and twist to apply lube. Also, you touched the glove box with gloves on when putting it aside. The glove box is not sterile. Otherwise great info.

    My mother suffered a CVA/ICH last year December.. we got discharged this November. She still has a tracheostomy tube and requires regular Suctioning/Dressing. It's about time to exchange the tube and i don't want to call some Doctor to do the exchange but rather do it myself. When she was at the hospital, her Doctor used to do it, so for the first time it'll have to be me. I've been practicing the hand motion using an old trach tube and stumbled upon your video. Kudos for being so extensive and detailed with your explaination and sending you my good wishes.. all the way from Delhi, India

    Thank you so much dear it’s a very helpful videos. I’m a Healthcare Assistant I have been working with tracheostomy client last one year normally trachy nurse changed the trachy but for every day we are taking care of those client. But suddenly once client had an accident .the trach was come out accidentally one of my colleague tired to insert the new one but maybe it was in wrong position the client had heavy bleeding then she called the ambulance .

    My daughter has her tracheostomy tube since 2005 and we change it every 3months by bringing her to the hospital now that is pandemic I requested my friend doctor an ENT to changed it for her but I would like to do it myself but scared . My daughter has no balloon. I'm still scared
    She could not talk ...and non ambulatory ...thank you for this video I might try it though. Jesus help me

      Hello. The manufacturers of tracheostomy tubes recommend changing the tracheostomy tube once a month. For exact details regarding your dad, please ask his medical provider when the tracheostomy tube should be changed.

    Just found your channel. Thanks Soo much for sharing! Very brave of you to do this in front of the world. Couldn't you squeeze the lubricant onto the inside (sterile side) of the trach kit package to just dip the trach tip into. This way it's done before you don the clean gloves, so your clean hand isn't touching the outside of the lube tube or lube packet?

      It depends which trach tube is being used. Some trach tube cuffs are filled using water. Some use air. For specific details, consult the manual which is include with the tracheostomy tube.

    Curious on the insertion. You said put it in at an angle then when the curved part goes in then turn it and go straight down. What is the reasoning for that?       Thank you for your question. If you insert the trach tube straight in, it will hit the back wall of the trachea due to the curve of the trach tube and will either be very painful to insert as it scrapes against the trachea, or it will get stuck when it hits the trachea wall. Turning the trach tube at an angle when inserting it allows the trach tube to slide into the trachea. Turning the trach tube back to the upright position midway through allows for the rest of the tracheostomy tube to be inserted.

      Usually, bleeding does not occur. However, if the person has granulation tissue around the tracheostomy stoma, the tissue may bleed when changing the tracheostomy tube.

      Hello. Thank you for your question. When my trach was placed, a size 8 was used. Sizes for females range from a size 5 to a size 10. It is based on height, weight, body structure, anatomical structure, etc. If you have questions about which size trach you will receive, please ask your medical provider.

      To donate the trach supplies, if your daughter uses a medical equipment company, ask the company if they accept donations. My medical equipment company readily accepts anything as long as it is sealed and in its original packaging. They distribute it to those who need it.
      Another option, ask the medical provider who manages your daughter's tracheostomy care. They may accept donations.
      Another option, call area hospitals. They may accept donations.
      Lastly, if all of the above do not accept donations (and if you live in the USA or in a place where there is an ALS group), contact the ALS Society. They accept donations.

      @@latikagupta1345 Great question. Tracheostomy tube manufacturers recommend changing a tracheostomy tube at least once every 30 days. However, people with an infection or children may need to change the tracheostomy tube more frequently. For specific guidelines on how often to change a tracheostomy tube, please ask your medical provider.

    23:17 Can you breathe while you're doing that? I know that you have some muscle strength left so you can breathe a little bit, just not efficiently without the ventilator on. I ask that question because I thought you wouldn't be able to breathe at that specific part (because of the hole in your neck being 'open' around this point, kind of like a leak) but you look[ed] so calm!
    Changing a trach is a bit more complicated than I thought it was in terms of all of the things that one has to remember about sterility. I notice a lot of similarities between your trach changes and my feeding tube changes but the trach change is definitely a bit more complicated. With a feeding tube technically things are best done as cleanly as possible however I could hypothetically drop the new tube on the ground and go rinse it off or wipe it off and all would be fine when I pop it back in. (I've never done that! But it would be fine, haha.)
    I really enjoyed this video. I use the same ventilator non-invasively however I enjoy seeing you talk about how having a trach has changed your life. I think with the support needs that I have and cannot receive at this moment that a trach is really not the best for me right now however I've known a few people with the same condition that I have (similar to yours but different gene defect) who have received trachs and they talk about how well they're able to breath. I love having non-invasive ventilation however I don't love having a mask on basically all of the time and I also don't love having to stop in between breaths so often. I noticed that you said that was one of the things that changed when you got your trach... I'm not rushing the day that I may need/get one however if a trach is in my cards, I look forward to that day when I can hopefully talk just like you. Without all of those pauses, ha!

      Yes, I have some muscle strength left and can still breathe when off the ventilator. If you are placed on a vent due to muscle weakness, you will be started on a vent before you lose all your muscle strength. Once started on a vent, your muscles will be able to rest and not be tired all the time. Some people once they get a vent only have to be on the vent while sleeping or 12 hours a day. Their muscles rest during the time they are on the vent and then they can breathe without any assistance for the rest of the day. Most people with muscle weakness can breathe without the vent for short periods of time until they lose all their muscle strength in their respiratory muscles. Then they will become completely dependent on the vent.
      Yes, there are a lot of steps for changing a trach tube, but it is possible to do. It is not good to place a non-sterile trach tube in your trachea, but if you drop the trach tube during the exchange and do not have a back up, sometimes you just have to carefully wash it off with distilled water and hope for the best. Most of the time nothing bad will happen.

    Great video! My wife is on a trach and ventilator and the doctor says willl be for life. She has a muscular disease called myotonic dystrophy which weakened her lungs to the point where she cannot breathe very well on our own. I am taking her home after 1 year being on the ventilator. one of my biggest concerns changing the trach every 3 months. She's not able to walk so I will be having my hands full. I'm curious are you ever going to be able to come off the ventilator?

      @@ariella4444 No, I do not constantly have sepsis. I rarely take antibiotics. I have only had sepsis twice in my life. Many years ago, a massive ovarian cyst the size of a large cantaloupe ruptured, which caused sepsis and organ failure. I had emergency surgery. Afterwards, I was given two broad spectrum antibiotics to make sure any remnants of the infection were killed. The second time, I got sepsis when my IV line became infected. This did not require strong antibiotics. It was a VERY WEAK infection and was killed by using penicillin.

    Forgive me, but as a nurse I couldn't help but notice a couple of things. I noticed that after you washed your hands and put gloves on that you were touching the glove box and the trach kit box. The outside of those are not clean. I would change my gloves again before touching anything in side of those kits. I am grateful for your videos. My husband has ALS and a trach which is 4 months old. The doctor wanted to change it herself, but I am unable to get him to her office. I believe that it needs to be changed. I feel confident that I can do it, and your video answered some questions that I had. Only thing that you didn't mention is how much air do you use to reinflate the cuff? I read 5 to 8cc.

      Hello. Thank you for your comment. You can change your gloves as often as you would like. If you are concerned about contamination of the new trach tube, you will also have to change your gloves after taking out the old trach tube and before touching the new trach tube to insert the new trach tube into the airway. However, I would not recommend doing this. (Once the trach tube is removed, I try to get the new trach tube in as fast as possible to avoid complications.)
      Please feel free to change your gloves as often as you like. When I have my trach tube changed by doctors or respiratory therapists, they put new gloves on at the beginning of the procedure and only take them off when the trach exchange is finished.
      The gloves are used to lessen the possibility of infection from the micro-organisms present on the hands of the person who is inserting the trach tube. The gloves many people use are from a box and are not sterile. They are exposed to the air. The gloves most likely have environmental microbes on them--the same microbes which are probably present on the trach tube box and packaging.
      Trach tube exchanges differ from sterile procedures such as central line dressing changes. When I care for my PICC line, the gloves are sterile. They come in special sealed packaging. Every time I touch something which is not sterile, I take off my gloves and put new gloves on. The bloodstream is a clean environment. The smallest exposure to a micro-organism can cause infection. This in contrast to the airways which are constantly exposed to environmental micro-organisms in the air. Non-sterile air is going in and out of the airway all day long. The respiratory tract is always battling micro-organisms and has an excellent immune response. This is not to say we should be careless when inserting a trach tube, but frequently changing one's gloves is not necessary (unless they become soiled, i.e., you cough into your gloves or blow your nose with the gloves on).
      But again, if you feel more comfortable changing your gloves during the trach exchange, please feel free to do so.

    Thank you for sharing this. My husband had a heart attack 10 days ago and they are putting in a trach tomorrow. How often do you have to do an exchange? I am just trying to gather information so I know what is to come, because I’m sure I will be doing exchanges. Very scared don’t want to do anything wrong and hurt him or make his situation worse.       Hello. The first trach exchange should be performed by a medical professional. The tracheostomy site needs to be checked to make sure it has healed. (After the first trach exchange, and if the tracheostomy site has healed, it should be safe for you to do his trach exchanges.) Ask your husband's medical team how often his trach tube should be changed. According to the manufacturers of tracheostomy tubes, they suggest changing the trach tube once a month.       No, I use different sized trach tubes because the size varies with different trach tube brands or different trach tube styles. The current trach tube I am wearing, I cannot wear a size 8 because the size 8 is too long for my airway. I had to change to a size 7. If I use a different trach tube, it might be a size 7 or it might be a size 8.

    I think the changing the trach has variations .. doesn't it. On some videos I have seen it being changed by caregivers and the patient was having a lot of difficulties to breathe he was in a lying position. please let me know the f differences. thank you.

      Hello. A tracheostomy tube can be changed while sitting or lying down. If a person is changing his own tracheostomy tube, it is usually done while sitting since he will need to change the tracheostomy tube while looking in a mirror. Doctors and caregivers may have their own preferences for doing a tracheostomy tube exchange which will dictate if the patient will be sitting or lying down. Some patients have very low muscle tone. It is safer for these individuals to lie down during the tracheostomy tube exchange. Also, young children often have their tracheostomy tubes exchanged while they are lying down.

    This is the first time replacing my sister’s tracheotomy. Your video is really helpful, but I’m extremely nervous. When my sister was in the hospital, I went through a training. My sister was the guinea pig and her trache wouldn’t come out. What a nightmare!!! They ended up calling a respiratory doctor to do the replacement. Any words of calming encouragement?

      The first trach exchange is usually the hardest one to do. It is not uncommon for the trach tube to get stuck and to be hard to remove. Most first trach exchanges are done by respiratory therapists or physicians. My first trach exchange, the doctor could not remove my trach tube. He is a big, strong guy. He had to pull and pull before the trach tube broke free and came out.
      My second trach exchange was easy. I did it myself. One of the most important things to remember when doing a tracheostomy exchange is to stay calm. If the tracheostomy tube will not come out, don't panic. If you cannot remove the old tracheostomy tube, try your hardest to pull it out. If it will not move, leave it in place. Contact your durable medical equipment company or physician to ask what should be done if the tracheostomy tube cannot be removed.
      It might be helpful to print off instructions for doing the trach exchange. There are instructions in this video's description. Feel free to copy them and print them off.

It’s much slimmer then the other, but still cuffed. I guess I’m just nervous. Where do I get a bronco dialiator?

      Hello. Any man made hole in which a tube is placed (trach tube, feeding tube, etc.) will rub against the skin and cause irritation. The body responds by producing a tissue called granulation tissue. Granulation tissue has many blood vessels in it and bleeds easily. I have granulation tissue on the outside of my tracheostomy site. When I do a trach exchange, the granulation tissue frequently bleeds. It is not painful.

      Hello. The size 8 shiley flexible with taperguard cuff has an inner diameter of 8 mm. If I was 5'3" tall and 140 pounds, I would most likely be using a size 6 trach tube. If the size 6 is not the right size, you can change the size of the trach tube. It is not uncommon to change the size of the trach tube after getting a trach. This video on how to do a trach exchange was filmed 2.5 years ago. I currently wear a size 7 trach tube.
      If you are concerned about the size of the trach tube, please discuss it with your doctor. One issue with trach tubes is the bigger the size, the longer the trach tube. If you are only 5'3" tall, a larger size trach tube may be too long and may not lie in the correct position in your airway.

    I would add to dry your hands with paper towels. Towels grow bacteria in them, especially when they have been used.

    I wonder how can you speak very normal with the tracheostomy.
    My understanding is that once tracheostomy done it is difficult to speak even a fenestrated tube or speaking valves. How it is in your case?
