    Can someone have microcytic and macrocytic anemia together which results in a normocytic-normochromic smear? what would be the RBC count, haemoglobin and mchc values in this case? The patient is easily tired, pale, light headed, has mild orthostatic hypotension when standing up, has trouble concentrating, has frequent headaches and has the urge to take a deep breath every few minutes. All values from CBC test are normal except MCHC which is very slightly raised.

    in over distruction,"hemolysis" retic index>2.5, meaning more than normal reticulocytes in circulation, and it should be macrocytic anemia because reticulocytes are bigger than mature rbcs?

    Acute blood loss, uh doesn't that fall under Iron Deficiency Anemia as with bleeding, you lose iron and iron loss is one subset of etiologies of iron deficiency anemia?

    Hi! Just had a question on why Sickle Cell defect causes normocytic anaemia? Would the sickling of cells not alter the MCV of RBC's?

      It’s because sickle cell anemia causes hemolytic anemia, which is normocytic.
      This is not peculiar to sickle cell.
      Most causes of hemolytic anemia cause normocytic anemia (normal MCV).

    Is it possible for this to occur in pregnancy? I’ve been told by my OB that I’m anemic and to take iron supplements. CBC was normal at start of pregnancy. However, I noticed on my latest CBC’s that my MCV and RDW were always within normal range. RBC, HGB and Hematocrit were low. I have an appt with hematologist scheduled, but feeling concerned in the meantime.

    Hi, my MCV is 98.8, my MCH is 32.9 but my hemoglobin is 13.6 (I’m a non-pregnant woman). So am I not anemic bc my hemoglobin is normal?

    Sir I have Normocytic, Normochromic always shown on my CBC report for years. Every doctor that I have been to says it is normal. Can you suggest any treatment plan under general asumptions? I mean, what generally should be the treatment. Thanks.

      RBC 4.3 cmm (normal 4.5-6), Hgb 14.5 g/dl and HCT 41.4%. Other than that MCV 94.5 fl, MCH 33.3 pg (normal 28-32), MCHC 35.3 g/dl. Report further says, Morphology of RBC Normochromic-Normocytic and TLC count 6.7/cmm.
      DLC Count shows:Lymphocytes 48% (normal 25-45), Neutrophils 47% (normal 50-70), Eisinophils 2% and Monocytes 3%. Platelets 252/cmm.
      Please note that I have mentioned normal values in bracket where the corresponding values have come abnormal according to my test report normal range.

      @bob wa hi. These lab values that you have presented here I presume it's your, are all normal, except your RBC which is slightly low, but that will not create concern. Your hb and your mcv are within normal range. You will not require any treatment at this stage. Are you having any symptoms of anemia btw?

      @@sandra-wr6tt thank you for your reply. Are a doctor? I always show marginally elevated total Billirubin (1.0mg-1.2mg) which my Gastro doctor says is due to Gilbert Syndrome.My Whole blood Lead level has come 16 mcg/dl so I assume that I may be having slight hemolysis due to Lead. For clinical signs, I can do jogging without any negative outcome but I do develop exceptional brain fatigue on mental work which has forced me to give up my teaching profession. If you think Lab reports are normal than why always to have Normocytic, Normochromic?

    Why reticulocyte index doesn’t increase in acute blood loss and underproduction ?

      Regarding underproduction, it depends on the cause. If the cause is aplastic anemia, reticulocytes will be low, because the bone marrow is “lazy”.
      If it’s acute blood loss, the blood loss is usually very fast (within minutes), and the bone marrow will not respond that fast.
      However, in hemolysis, this process takes a relatively long time (even acute hemolysis), so that the bone marrow has time to “work like crazy” and increase the reticulocyte count.

      Starts as normocytic (healthy marrow), and later it becomes microcytic (marrow ran out of iron)...Hope it helps!

      cause the cells to appear smaller hence microcytic? In the case of normocytic cells, does this mean their hemoglobin is normal that's why they appear as normal size? Slightly confused. Cause I do understand that they're lacking in RBC, but why are they normal sized?

      Normocytic is not an infection. Normocytic is a description of the size (volume) of the red blood cells.

    Wait... I thought normocytic anemia is caused by two main etiologies??? Peripheral destruction and underproduction?
    1. Peripheral destruction is >3% of reticulocytes. Which means bone marrow production is good.
