Nice review of aplastic anemia! Just wanted to add that sometimes you can get a hypocellular MDS that looks similar to severe aplastic anemia. Usually cell morphology and cytogenetics will help you differentiate the two (hypocellular MDS you would expect more chromosomal abnormalities, aberrant megakaryocytes + megs will be small in appearance), however clinically they can look very similar. Thanks for the video!

Today i have to learn aplastic anemia for my tomorrow hematology class and this video was absolutely amazing and very helpfull.
I got only one question.. I have read that the hypothesis that aplastic anemia is a state before mielodisplasic syndrome is not so strongly approved according to the new findings in the genome. I dont know how true this is but i read some good arguements

Good day Sir, so I have a speculation as well as to why MCV is high is aplastic anemia.I’d love to know your thoughts.
    If MCV= Hematocrit(ht)/[RBC]
This would mean that a decrease in [RBC] or increase in Ht should give high values of MCV right?
We also know that in aplastic anemia, while Ht might remain the same (since all the other hématopoïetic cells are also affected), [RBC] is definitely low, hence giving high MCV values.
Just a speculation, please correct me if I’m wrong😊.Thank you.

MCV is also calculated via forward light scatter in flow cytometry. This gives each cell a direct reading of size

Do any of the pathologies we've seen so far affect lymphoid cells? Thanks for everything :D

How can we differentiate between the clinical presentation of megaloblastic anaemia and aplastic anaemia when both have pancytopenianin common????????

    • @MedicosisPerfectionalis

Clinical presentation will be the same.
If there’s vitamin B12 deficiency, you might find neurological problems.
The difference is on the bone marrow biopsy.

I was diagnosed with aplastic anemia with low rbc, low hgb, low hct, high MCV, but high reticulocytes, if bone marrow is not producing enough cells, why is reticulocyte count higher than normal? (Bone marrow cellularity is at 5-10%)

Could be relative reticulocyte count cuz its a percentage of whole blood. If it decreaces less than hct decreases then it’ll appear to increase. Theres a formula to get the adjusted retiuclocyte count in his earlier videos

I'm thinking I was diagnosed wrong. They said I had leukemia but it came back. But I was on nefdipine, And ani seizure meds for migraines. I was also diagnosed with thrombocytopenia, Meloydysplastic anemia. When they would give me iron transfusion my platelets would drop so low that I would need a platelets transfusion. Just didn't make sense to me. Please if anyone as an insight to what is happening. Please let me know because I have a appointment with a new oncologist/hemotologist next week.
