The joke is that when Macchio played Larusso in KK1, he was just not quite as big (muscular, plus some height difference as well, but mostly Macchio was still very slim, etc.) as Zabka-that there was a bit of a mismatch there, same thing with how big/muscular the other Cobra Kai were (more or less) compared to Larusso...
Of course, John Cena is freakishly muscular, and is obviously exaggerating the "80's bad boy" voice... This whole sketch is just humor and exaggeration. They don't mean to disrespect the Karate Kid movies. Its respectful satire, basically. The KK movies were more dramatic BECAUSE of the size difference, etc. between Daniel and the Cobra Kai guys. You don't see that muscular size difference in the Cobra Kai CZcams shows. That's why the Cobra Kai shows are more "realistic", rather than dramatic in that original train of thought, etc.
