On a set they have an expert to deal with safety issues. The issue is that the gun was declared "cold" which means completely empty of any blanks and of course any live rounds because live rounds are not allowed on set. The depend on experts with weapons because actors are not experts. They failed on those protocals. There are times where you would point a gun on a set at people. In this case they were pointing it at the camera while they were working on blocking for the scene. You want experts on a set to determine safety and not untrained people. On set the expert also needs to understand the difference between real rounds, blank rounds, and fake prop rounds. Fake prop rounds being used on close ups like scenes of loading the gun. Baldwin was literally informed the gun was "cold" ie no blanks. the person that did that is at fault. Especially after confirming the chain of custody. However the production company will very likely get hit and hard with a civil lawsuit and should.
