Insurance company issues the cover note booklet to Insurance Agent.
Agent in turn keep this cover note booklet, which is already stamped and signed, in his office. Or sometimes it is handed over to some large transport company.
Party or transporter fills up the details, take their copy and policy is issued.

Think about the person, who will wait for a medical claim refund from 2010 to 04th June, 2013. I had twelve times admitted to hospital and insurance approved my claim 40% & 50%. Why?
