Respiratory System History Taking - Common Symptoms for OSCE Revision - Dr James Gill

  • Revising for Respiratory System Histories for your OSCEs can be made easier by focusing down on common symptoms that patients with respiratory diseases often present with.
    Common Respiratory System histories for medical school examinations tend to follow themes:
    - Cough
    - Wheeze
    - Haemoptysis
    - Chest Pain
    - Shortness of breath
    By focusing your OSCE revision to these common symptoms you can optimise your revision time and remote the fear of coming up against a respiratory case.
    These common respiratory symptoms have the benefit of crossing over into other specialities as well

    10:20 It's such a blessing to know that you have a doctor that is actually taking care of things. My partner went to a clinic because she had been sick for 2 months with gastrological problems which in turn worsened her asthma. Her doctor shooed her because she thought she was putting the clinic in danger. Gave her antibiotics without even examining her. After some days we went to another doctor who got her history in the short time of 3 minutes ( she said that she had already seen 5 doctors prior to him because nobody could find the reason she had extreme pain and dysphagia )... He gave her psychoactive drugs for a month and told her it was all in her mind. And then we went to the hospital where they gave her 2500ml Methamizol for the pain and told her to leave.... Methamizol , because she has had a Thyroidectomy, completely messed her up sending her into pharmaceutical induced Hypothyroidism. I hope nobody goes through this. It has been a test for us and really tought us what it means to have a good doctor. Please keep doing what you are doing.

    My husband is turning 40yrs old this year. Last year, he started getting dyspnoea just going up the stair at home, also his BP started getting higher and higher. He went to the cardiologist, he changed his meds and asked for a Doppler echocardiography. Then they asked for the transesophageal and we found out why he was so tired. Apparently he was born with an atrial septal defect and the right side of his heart was just tired from trying to keep up with the inflow from the left for all these years. He had a minimally invasive surgery in February and it has improved a lot. So, yes. Dyspnoea can come from anywhere

I had asthma when I was younger, up through early middle school, (not sure what the school system's like over there, where I am, middle school is 6-8 grade), but it went away with no recent relapses. I was wondering if there is a risk of it returning, and what could increase that risk. I run, and I take it pretty seriously, so I'm in good shape in that regard. If I were asked if I have any current medical conditions, should I say asthma?

    Oh yes pleurisy is painful. I've dealt with it a few times and never got any answers about what could have caused it. Basically I was given a prescription for painkillers and told it would eventually go away on its own.

      @@DrJamesGill next time (assuming it happens again) I'll try to push for a better explanation, especially since it wasn't a just one time thing. I appreciate your videos providing the medical point of view not only because I simply love learning about anything medical but also because it helps me understand and ask better questions when I end up being the patient in the situation.

    Question, is haemoptysis only when you are being blood up from the lungs? There was talk of the mouth and such being other possibilities, would this count as haemoptysis as well or as something different? And does it nessesarily have to be blood bring up via a cough or could it also be blood sitting there if they were, for some reason, not coughing? Curiosity has the better of me haha.

    • @DrJamesGill
      @DrJamesGill  Před 2 lety +1

      Technically haemoptysis is the coughing of blood up from the lungs, or the bronchial tree
      Technically not the mouth etc, but if a patient said they had “coughed up blood” I would consider this haemoptysis until proven otherwise
