- Thyroid Medical Examination - Thyroid OSCE Station - Medicine Explained - Clinical Skills - Dr Gill
This Thyroid Exam Deep Dive looks at the details of how to establish the thyroid status of a patient, whilst highlighting what is being checked for during special tests and examination findings
We’re going to focus on WHAT we’re doing during the exam, HOW we’re doing it, and crucially WHY you need to #putabitofpaper on the patient's hands!
In this video, we demonstrate how to perform a clinical examination of the thyroid for your medical school Clinical Skills OSCE. Endocrine examinations are quite common for medical finals
For a passing grade in your Clinical Skills OSCE, in a Thyroid exam follow the approach of:
- Inspection
- Palpation
- Percussion
- Auscultation
- Other tests
This video has two other thyroid assessment videos associated with it:
• Thyroid Clinical Exam ... - Thyroid Assessment Demo
• Thyroid Assessment - M... - Thyroid Examination Deep Dive
Some people may experience an ASMR effect from this video
------------
Please note that there is no ABSOLUTE way to perform a clinical examination. Different institutions and even clinicians will have differing degrees of variations - the aim is the effectively identify medically relevant signs.
However during OSCE assessments. Different medical schools, nursing colleges and other health professional courses will have their own preferred approach to a clinical assessment - you should concentrate on THEIR marks schemes for your assessments.
The examination demonstrated here is derived from Macleods Clinical Examination - a recognised standard textbook for clinical skills.

Putapitopaper feels like an old friend you haven't seen in years when social media didn't exist yet
🤣🤣 honestly, I have no idea why people notice it. In my head I have so many verbal quirks, that isn’t one!
@@DrJamesGill it's all part of the legend of unintentional asmr, embrace it!
I miss the "any double vision" family though, just one of them this time
@@DrJamesGill I never heard someone refer to a piece of paper as pit of paper. American here. So I think it's the way you say it quickly without enunciation. If I said "put a piece of paper" there would be more enunciation in "piece of paper" than "pit of paper"
That's my interpretation of why everyone loves it.
@@Berengier817 he’s saying “bit of paper,” people are saying “pit” as kind of a cutesy way of phonetically spelling what it sounds like.
I’m convinced Dr Gill is the Bob Ross of medicine
True! 😅
Definitely!
and right over here we have a happy little spleen
I was thinking a new Dr. Who.
Happy little examinations.
I'm no expert in medicine but i always liked when doctors explained me what they were doing, not in a technical way but just so we can understand what they were searching for, you doc are a perfect example of this, i believe it's a good thing, just like when I'm taking my car to the mechanic, i really don't understand what they did to the car but at least they try to get some sort of basic knowledge to the basic client
This is actually such an informative video. Came for the asmr, stayed for the info.
“Abby Tutt, 7th December 1996”
I feel like that sentence is branded onto my brain and I announce it out loud every time I see her in a video.
True star if the show
Video really starts at 1:13
Word to the wise: Remember Pearl Harbor!
Just don’t accidentally say it to your GP if they ask that question, you may get a strange look
I cant believe today i was able to listen a new putabitopaper. I feel like today is Christmas Day. Thanks Doc
I absolutely admire this new format of explaining what is going on during the examination. Thank you again!
It seems to be working well 😊
@@DrJamesGill Indeed it is way more interesting when the audience knows what is going on!
Abby always has the most infectious smile! Good to see her and the doctor again!
I feel like I'm half way to my Md watching Dr Gill's detailed and relaxing videos
“I’m the bearer of a very small brain so I like to work in straight lines” 😂 don’t we all doc
i thought i had misheard him when he first said that! it definitely got me to giggle
As someone with anxiety from sensory issues I totally appreciate this video 1,000%! I need to be aware of when/where and how deep I’ll be touched by others because although I have a high pain tolerance I’m also very sensitive to certain types of touches so I love that you warn her that you’re about to touch her from behind, great job!
I love that you're still out here dispensing medical facts. You've become something of an ASMR legend.
I thought I was literally loosing my mind before they diagnosed my thyroid wasn't working. raging panic attack/anxiety, weight gain, not sleeping, etc. Synthroid is a hell of a drug!
Sounds super scary , thank god at least you were diagnosed. In my country doctors would tell you it's menopause
@@EughhBrothereughh Russia?
it took me almost a decade to be diagnosed with Hashimoto's disease. My thyroid has been through the ringer.
Abby is in immaculate condition he never finds a single thing wrong
Mint condition
Wonderfully well done in depth exam
👍 hopefully it helps people understand what is going on 😊
The bumblebee socks are FIRE.
abby, your shirt is delightful!
dr. gill, this is my favorite format for these videos by far. i wonder if you’ll ever do one with an SP that is trained to “have” certain symptoms that will indicate some diagnosis? i do realize that’s not possible with more “visible” signs and symptoms, but it would be an interesting exercise to “test” the viewers’ knowledge as you go about the exam.
Saw the piece of paper on the examination table and knew what was coming 🤣
100 thousand Doctor Gill! Congrats, thanks for being such a fantastic educator!
i will never EVER get tired of watching dr full content
I love this way of explaining the examination. The why and how in the same video with a demonstration. Brilliant!
Abby is my favorite patient.
Great partner of doctor Gill.
Fantastic duo is back! Thanks for these didactic and relaxing videos.
Hope they help 😊
Worked with this young lass when she was a second year? med student at the George Eliot Nuneaton a few years ago. Hope she's doing well
Honestly she has done so well. She’ll be an exceptionally good doctor

Putabit of papers back. Woop woop 🥳
The good 3:43 Putapitopaper, but what about Loose and Floppy 14:15 ❤️
Calm and informative video; thanks, you two!
Why am I happier with Dr. Gill and Abby in my you tube world? don't know and don't care. Just am.
14:20 many years ago, in 1998 or so, we were hanging out in a pub and we started talking about this reflex. Each one wanted to try on his own, so we ordered like fifteen glass bottles of coke, which we drank in a gulp. When the waitress returned at our table she found fifteen people quietly hitting their knees with empty bottles.
We explained why, and she joined.
Well, u all could order 1 bottle and take turns
@@MATSULEIGO where is fun with that
I love fictional stories.
Feel proud to be a part of this family 🤗
Brilliant stuff, all in one take too!
🤣🤣🤣 I wish
@@DrJamesGill I imagine there was a lot of giggling here. She has a cute shy smile that suggests a bit of giggling from just having a good time and like you mentioned a bit of anxiety. Fantastic video as always the both of you.
Love watching Dr Gill.
I have hypothyroidism, but my doctor has only ever checked my TSH so all of this examination was fascinating.
Yes! Putapitopaper! I saw the paper sitting there but had doubts he’d do a callback to the old video. I shouldn’t have doubted.
The most useful asmr on CZcams as always)
I'm very interested in medicine, as someone who is not related to it can be. And it's kinda sad that my level of English do not allow me to easily understand all the content of the video, so I might both relax and learn simultaneously.
I'm definitely can relax to these videos and hope I can have some bits of useful information while doing so)
p.s. a bit of paper returned!
Man like James Gill. Pure relaxation in your voice, man was fuming until I listened to it. I’m a big fan, keep doing these videos it stops me from having Psychotic Episodes
Good luck on your exams, Abby!
Godlike quality, as always.
Looks like you're killing it as always.
Thank you Dr. Gill, you put me at ease when going to my regular medical checkups.
That’s very positive to hear.
Is there any other ways we can help?
It always makes me more comfortable to know what the doctor is doing to me. I'd love to have this person as my doctor.
Thank you for the great video, Dr. Gill. Who would of thought that my best professor is across the pond and my class is at home. Very grateful to your knowledge, explanation, demeanor, and hard work. I've become a better RN for it 👍 grateful from Texas.
👍
This is very timely for me since I'll be going for a CT scan of my chest on Thursday since a cyst was discovered near my thyroid during an unrelated MRI scan of my spine
Crazy how this is an actual medical informative video. But I'm always here for the ASMR. 😅
Thyroid disease is common in my family. Have an unrelated appointment next week but I’m going to ask some more questions about a goiter that was found. All because I watched this video!
Spiffing job sir , bravo , very well done video , be having a pleasant day now 😀

I like this idea about swapping places 👀👀👀

Clicked for curiosity and got info and asmr, your soft voice is perfect to relax to and what you’re saying and showing is interesting. Congrats on your possibly unexpected fandom sir if the asmr is unintentional 🤣
If you benefited, learned something, and perhaps understand a little more about your own body, I’m over the moon!
Hope you're feeling better Abby.
i know these videos arent for medical use(real life situations) but i keep it in mind just incase
awesome channel and digestible information keep these going!
Congrats on 100k
Would It be so nice If you bring an pharmacist friend to talk about some cool stuff about the drugs used on the treatment of the conditions you talk about in your videos. Keep doing this amazing work 👊
Welcome Back Abby!
The trilogy is complete.
Best volunteer and doctor 😂❤
Good Video, Congratulations ! 🇧🇷
I guess EMT school wasn't enough cuz here I am binge watching all of these medical videos from Dr. Gill
Well, Dr. Gill I need to book you for a thyroid examination, as I haven't had one as thorough as you did in this video.
More jokes this time... I like the laid back Dr. G! Great and informative (and funny) video!
I cannot tell you how many times I've watched this video already (don't ask why), but I JUST noticed that Abbie magically puts on shoes and a different pair of socks there for a few seconds ;D
🤫
Thank you

Happy birthday Abby!
The DGCU returns!
So, the late Marty Feldman's famous "pop eye'd" look was due to hyper thyroid issues? Is that sort of thing treatable with modern therapies?
By this point I don’t think you need to confirm Abby’s name and date of birth, you must have it fresh in your memory
Number one, as an aspiring PA, I love your videos. They're really going to help me out in the future.
Number two, I've been concerned about my thyroid and when I went to the doctor, she wasn't nearly as thorough as this. That's concerning, I kinda want a second opinion now.
Did they do any bloods? That would be an effective second
@@DrJamesGill yes they did do some blood work
As Dr Gill pointed out you get the diagnosis from thyroid function tests/history. You'd be hard pressed to find a doctor that examined for every rare extra thyroid sign
I was diagnosed with hyperthyroidism almost 3 years ago and I’ve been in treatment since then. This medical exam is what my doctor does in my periodic check ups, I wasn’t sure what she was checking in my hands but now I know 🤓 The only part of the exam my doctor does not check is the part of the tongue. This video is very informative, I had almost all of the symptoms mentioned for hyperthyroidism, we found out about it because I got tired real quick with just some steps, my vision used to numb black and started to have tachycardia, at first I went to a cardiologist think I had heart issues and he diagnosed me with hyperthyroidism. While in treatment I had just one eyeball with back inflammation, it was really tough because my face looked really asymmetrical, I went trough a 10 week treatment and got a little chubby but then I lost the weight. I’ve been weighting 36-38 kg since I was 15 and I got to 40 kgs for the first time last year with almost 2 years in treatment and I’m 25 years old now. I’m not a doctor but I really enjoy these videos as they help me to understand a lot of exams and get to know what symptoms can show through these conditions. Thank you!!
I’m really glad you’ve had good care!
Where were you looked after?
@@DrJamesGill Thank you so much, I’m from México, I was treated here by a really good cardiologist Dr Luis Eng, and he sent me with an also really good endocrinologist Dr Verna Mendez. Since I was a teen I went through a lot of medical appointments for years because of my very low weight and they used to tell me it was hereditary because of my parents weight until I was 23 and was properly diagnosed. My thyroid was really inflamed and sometimes it hurt. I started taking 4 and 1/2 pills of 10mg thiamazole daily, now I take 1/2 of 5mg 1 day and 1 day not, and my thyroid is not swollen anymore, I’m gaining weight, I don’t have heart/pressure issues anymore, and I sleep really well now, I have a little of double vision, I take a daily pill of selenium (my doctor says it’ll help) but my double vision was worse so I think I’m getting better 😎👍🏻
Lovely shirt Abby!
