I had my gastric bypass 22 years ago. I had one of the first lapriscop surgeries in my area. San Francisco CA. I went from 238 to 135. My PCOS went dormant and i was able to bring 2 pregnancys to full turm.
You have so much to be proud of!
Did you keep the weight off after pregnancies??
@j2i8h Hi, yes, the pregnancy weight gain was not an issue. I had my daughter 2 years after gastric bypass. I gained 23 pounds with her pregnancy, and 2 days after delivering her, i had lost 20 pounds from the delivery. I only had 3 pounds remaining. With my son, he was born 6 years after my bypass, and i gained 25 pounds with his pregnancy and also dropped 20 a couple of days after birth. I regained 43 pounds of the 104 that I had lost 40%...13 years after gastric bypass when I turned 40 and had a car accident that left me with a brain injury. I was not able to exercise for 3 years. It affected my pituitary, and I also entered primerose, which makes it harder to take weight off.
I had gastric bypass surgery on August 21st and I'm currently 190 lbs. Starting weight was 400 lbs. Best decision of my life.

I Had Vsg aka Sleeve Surgery May 21,,2015 i lost a 166 pounds..Kept it all Off. I was in my 50'S Today i am age 64. post Op 7 Years!! Best Thing I ever did for me and my Health!!

I had the gastric bypass surgery 5/20/2021. I was dx with uncontrolled diabetes type 2, HTN, sleep apnea, neuropathy, asthma-my highest A1C was 16. My highest weight gain 302. I was sent home with no medication post-op. I am now 195lbs my A1C is 4.8. This was the best decision ever ❤️

Agreed! I am 43 and type 2 diabetic. I weigh around 213 and am 5’3”. I was diagnosed last year but it’s possible since my diagnosis of being gestation while pregnant in 2016 that I could have been pre diabetic. Be that as it may, my doctor who is helping manage with metformin never brought this procedure up and I have been mentally and emotionally drained. I have two kids and want to be around for them. For whatever reason, I had a tremendous amount of fluid on me while pregnant because I don’t over eat but I have slowly gained so much weight around my stomach and waste-line. I found out today that Kaiser insurance doesn’t pay for this surgery and I need to get this done because I most definitely want to reverse my diabetes. I have even looked into going out of the country to get it because I’m able to pay out of pocket and cost is more feasible. That’s a last resort though. Thank you again, I think the bypass is a better option. I just don’t want to look sick with my weight loss.
@@traven281 Are you my twin? Lol. I could literally mirror your comment. I had to scroll back up..... I was like I don't remember posting this. Anyhow, my situation s exactly the same. 43, 2 kids, 5'1, gestational diabetes/pre diabetic, hypertension and pretty much everything you said.
I go see my doctor on the 6th. I prayed about and had a long conversation with God and my family. I want live my life and not be affected by my weight and the other issues that come along with it.

@@JSDFAMILYOVEREVERYTHING I’ve had no major complications. Thrown up a few times but always my own fault when I eat too fast. Constipation and now I’m losing some hair but fortunately I had thick hair to start with. 5.5 months and down 75 lb.
My goal is 40-50lbs max. You Iost it in 12 weeks. I'm wondering if I'm a good candidate for wls with only a 40-50 lb weight loss goal.

Very informative! I’m currently at 365lbs and will be having the Gastric Sleeve done in 3 weeks. Super nervous and can’t even imagine the liquid diet process but I’m ready to take on the journey to save my life. This video couldn’t have came at a better time. Why would you do a sleeve?? I am 325 lbs and am going for the bypass on Monday.. My doctor told me if you need to lose more than 100 lbs it's pointless to do the sleeve. The sleeve is for patients needing to lose up to 80 lbs 🤔

I am a current patient. I had the gastric bypass surgery on 06/14 & just about 3 mos post op. to date & I've lost about 65lbs! His wonderful & his reassuring support before & after surgery is amazing. He takes his time & answers any questions or concerns you may have and his team is just as encouraging & supportive. It's not an easy surgery & I am still adjusting to my new life style & I have had to make a new relationship w/food but I have no regrets. This is by far the BEST decision I've ever made for myself. Thanks for your question. I would love to help with some duodenal switch information, but I don't plan a video until I have added that to my own toolbox. Still watching to see what medium-term results are going to be like with regards to nutrition and vitamin levels.
I just signed my consent for surgery last week and now just wait for an surgery date. My surgeon did a great job of talking me though the 2 options, BUT WOW, I am so glad I found your video, this was SO informative, so well put together. I must say even with my surgeons suggestion coming from a lap band that id be happier and more successful with the bypass I was still too scared of the Bypass. As soon as he said its the same to the body on a physilogical way- I said why take the chance of this reflux with the sleeve, when that is the precise reason why im removing the band!! Best decision I ever made! I was obese but had the surgery to fix a big problem with my esophagus. The problem was fixed and I lost over 100lbs, which was fantastic. You were my life-saver…thank you Dr. Pilcher!

@@Iamnatalierose I ended up getting a laparoscopic gastric sleeve completed in late November (3 weeks ago exactly). The surgery took less than an hour. I was in moderate pain unpon waking up soon after but was given pain killers and IV during my 24 hour hospital stay. I had zero pain after 24 hours, but I did feel weird in the stomach. For the first 2 and a half weeks, my abdomen was "bubbly" (for a lack of a better word) which I assume was just the healing process. Diet wise, I will transition into soft foods tomorrow, but I literally eat 1/8 of a teaspoon before feeling satiated.
I've grown extremely sensitive to smells. I smell things others don't, and I'm a bit grossed out by food sometimes, though the grossness has faded lol.
What surprised me is how difficult it is to drink water; even taking small sips has been difficult for me. Water feels VERY heavy from the moment I swallow it. For this reason, I was dehydrated a few days ago and thus needed IV 3 times in a span of a week-nothing serious tbh-just lots of fatigue because of the dehydration. Drinking as I walked helps tremendously. It is very important to follow the post-OP diet strictly to the T and that you walk a minimum of 30 minutes/day to ensure proper healing. I also strongly suggest learning how to eat intuitively and learn the "healthy" way of eating (i.e. nickel sized bites, putting your spoon down between bites, learning your bodily cues, etc.).
I started taking a multivitamin, iron, and vitamin D immediately following the surgery to prevent any deficiencies. Finally, before the surgery, I was 227lbs. 20 days later, I weigh 198lbs. My family says they see the difference in my body already.

Dr. Pilcher, I came across your videos recently after considering GB and after talking to a friend who had the surgery as well as her Mother who have no regrets having it done. I really appreciate the information on this video to give me some idea of what it is about and things to consider. Today I am going to my primary doctor to see if they would give me a referral to see a surgeon in order to know if I am a good candidate for the surgery. I have many medical issues that the surgery maybe beneficial for me besides just want to be smaller and able to move better. Thank you for making it not seem as scary as people or the internet make it out be but certainly life changing if commited to the process. Your calm demeanor and kind spirit is very reassuring if I decide this is the right path for me. The liquid diet at first seems overwhelming but to know many people have been sucessful with the help of a GB seems worth it. I wish I lived in San Antonio so I could work with you put I am in Los Angeles. Thanks again

I had the gastric sleeve done in November 2020 and lost 195 pounds in a year and a half and have stayed consistent since then. I went from 370 to 175 pounds and as a 6’ tall guy my weight is right where it should be for my height. I was 28 years old when I had it done so that might have helped some too. I chose the sleeve because it was less invasive compare to completely rerouting everything but I also didn’t want to deal with the dumping syndrome and other stuff that can come from the bypass. I’m happy with the outcome and would recommend bariatric surgery to people who are serious about following through with what comes after. If you’re not going to be serious about it because you think all your weight is going to magically disappear without doing anything then you won’t be successful with it and be part of the statistics of people who didn’t lose much weight. Good luck to everyone who’s thinking of or has had it done. The transformation can be amazing if you have the mindset to it.

That’s the one I want to have in singing the consent tomorrow but I hear some people saying they threw up a lot or reflux and I’m wondering if is the surgery itself or just bc they probably eat wrong stuff than fell sick ? What do you think ?

OH MY GOD! I'm so happy I watched this in its entirety, it has EVERYTHING I wanted to know answered. Initially I was looking into lap band, then wanted vsg (it's trendy now), now I'm leaving leaning towards byspass. I am 31, and importantly it's reversible if needed. I wanted lap band because I can lose it when I want but it doesn't work apparently and is high maintenance.

I am thinking of having surgery for yrs but I am scared with complications. I am glad I found this video. It helps me a lot with understanding. I am 265lbs 56 yrs old with diabetes 2,sleep apnea, and allergy asthma. I am walking with my dogs nearly everyday as my exercise , I don’t want to go to the gym because of shame. I am not a big eater, no matter what diet I do I’m not losing any weight. I was told I’m gaining weight due to my asthma medications. I was also diagnosed with panic disorder. Im so sick of being sick. I used to be 120-145lbs until 2000. I gained pounds all these yrs and I’m lack of confidence. Thank you for your video it helps a lot.

I’m 55 and I was shocked when my Bariatric surgeon said bypass. He said Since I’m a type 2 diabetic it would be better for me I have a host of other comorbidities I’m medium obese and I thought the bypass was for ppl that was severely obese but science is learning more and more with the studies of these surgeries new info is learned I think this is the best video comparison on the surgeries. I just had my gastric bypass surgery last week and seeing my weight come down a little everyday..start at 323 day of surgery and dwn to 309.6..continuing my full liquid diet until I can advance. My surgeon Dr Carlin is amazing.

Unfortunately my surgeon and his staff were not very thorough in explaining the food limitations and I was overeating and making bad food choices like fried foods. My surgeon also failed to tell me about post bariatric surgery Telogen Effluvium (hair loss). In a period of a month I have lost an enormous amount of hair. I believe everyone goes through that. Regrettably not all surgeons are as thorough as Dr. Pilcher, so be careful of your surgeon choice and ask many questions. The nature of my problems were excruciating pain after eating, dehydration (never thirsty) and severe constipation. I have learned a lot from Dr. Pilcher's videos and thanks to his explanations I am now aware of many things that I should have been told from the start.

My surgeon did advise bypass but he was going to do an endoscopy first before deciding on the final surgery (sleeve vs bypass). This video was so helpful and also calming. I was leaning towards the sleeve because I'd heard horror stories about the bypass and heard that a sleeve is "less invasive". After watching this, I'm definitely going to get the bypass as I already get acid reflux. Although it is minor, I don't want to make it worse and like the fact that a bypass will actually help with it. Although I won't be able to use NAIDs, I don't see this as being a huge issue for me as I only use them during my period and I have other ways I could manage that pain instead. My sister in law literally almost passed on after having her bypass 7 years ago in a great hospital in Boston! Out of my 5 family members and 2 friends and myself who've had the sleeve, none of us had complications. So, I'm sorry but I do believe that bypass is more risky from life experiences!! My sister in law was given her last rights and everything. Her surgery itself went well but her body was rejecting the surgery. She couldn't hold down not even water. It was a miracle and amazing Dr's that saved her life. I'd never suggest bypass to anyone, especially when they can simply get the sleeve if w.l.s. is what they want and need!! Much like how back in the day doctors Swore by the lapband...now look! That'll be the way we look at bypasses one day, I just know it. It'll be gone with the wind just like the days where doctors told pregnant women that smoking during pregnancy was a great way to keep her figure.
Glad your sister-in-law made it through, and glad that you and your family are doing well with your sleeves. In spite of your personal experience, the numbers tell a surprisingly different story as outlined in this video.
All depends on your sister in laws body at the end of the day. All surgeries are risk.
I had mine in 2016 ( by pass ) my weight was 290 now it is 180 , muscular 14% of body fat , the secret is diet, exercising and keep vitamins on level . ❤

Being from an European country in which English is not the mother language I learned English in school for 5 years and had some practice talking it in Asia but on a very low level I"m very happy with the English dr. Pilcher uses. Searching for months for information to make my decission for gastric bypass OR gastric sleeve this video gave me the most clear and far going information I found in months reading, and looking video"s, and talking with people who had a gastric surgery. And, important for me, in a kind of English I understand. So now I know much more then I knew before, and even in English, which is my third language. I "m much more ready for the conversation planned with the bariatric surgeon planned in one month to make a final decission. So Dr. P(h)il-cher (joke) for me you are a hero. Thanx so much!

I’m so happy I did this surgery & would do it again if I need to, thank you for explaining the difference between both surgery…. I had the gastric bypass done 3 yrs ago went from 224lbs to 135lbs but now I’m at 179lbs. A little worried about that but will see my surgeon & Dietitian soon…

Gastric bypass is for me far superior. I had sleeve ten years ago and got the revision to RNY November 2021 . Never been happier , losing more and feel so much better. I had wanted the DS for revision, the surgeon did not recommend it for me , I believe due to my age, and I didn’t have a huge amount to lose , I suggest people listen to their surgeons advice, they do know what is best ! RNY rocks !

Gastric Bypass is what I am leaning to as well, but I read you can't eat red meet afterwards, what, never ever? Is it no longer digestible? I don't eat it a lot now, but occasionally I really crave a steak, & I couldn't give that up for ever.
@@user-vu7rv1xf1l Thats totally false about red meat. At the beginning its hard to digest, and not recommended as time goes on you can have a little, meaning all dense meats will fill you up. Chichen being very dry.. will be the hardest one in its plain form to swallow.. go for it and you can eat almost anything down the road. Just account for it in your plan
@@zenmaiden1 thank you thats good to know. I know I will have to cut down on portion sizes & have to be on a special diet during the healing stages, but ultimately I want to just be able to live a normal life long term, without too many restrictions, just small sizes.
I wish I had seen this before my surgery! I got gastric bypass in Mexico in April 2021 and I have lost 115lbs, hitting my weight loss goal already, But the doctor said that bypass had malabsorption for the rest of my life. He recommended it due to my thyroid being completely destroyed in malpractice. I'm glad I did it but this information would have made it so much easier for my anxiety!

This is a great video, I work in the OR and scrub for these procedures on a regular basis. I can really tell that you care about your patients and them being educated about these procedures . Well done you seem like you would be a surgeon I would love to scrub with!

I have similar issues and also considered the sleeve and mimi gastric bypass. However my surgeon
advised me to go for the full bypass since with a mini there’s only one connection and something about the bile juices collection with a mini wouldn’t work with my hyperacidity issues.

For someone with PCOS , anemia and reflux is gastric bypass a better option. My weight is 120 kg and 165 cm height. Thank you for explaining everything so clear. I've been struggling with the decision of having the surgery for a while and I think I'm convinced now.
My healthcare team made me go to 10 weeks of orientation for my desicion, you should ask your healthcare if they offer that, it doenst comit you to the surgery, it makes it so you can make an informed choice.
Pcos having 35 year old mom of 2 here to say that I'd go with the sleeve. My sister in law almost passed after her bypass surgery, it's more risky. 4 family members, 2 friends and now myself have had the sleeve and after 8 years of the person with the longest surgery I know the sleeve people have been amazing. From weight in the 400s to the 200s.
Thank you for the info. I'm 68 years old and 6+ years out from Gastric Bypass due to acid reflux. No problems , yes some dumping usually due to what I did and still have my restriction,, 108 pounds lost and through the years 20 weight gain, but about 90 pounds seems to be my set point.

i had the gastric sleeve and whilst i lost 80lbs and have maintained for now 2 years, i think for someone for my original size (320) a bypass would of been a better idea. However, if i eat proteins mostly i still have very good restriction so i know if i get my head right i can get there :D but very good explanation

Well...that July 2020 study explains what happened to me (I am the 8 to 10% affected)! It has been 9 years since my sleeve procedure; and I will celebrate my Bypass 6th Anniversary on Tuesday 08 Sep. I am a little over my goal weight but hasn't EVERYONE gain the COVID ten after being quarantined and trying out all those CZcams recipes! Brainstorm...film Heidi or Linda preparing a protein enriched meal! But even with the COVID quarantine weight gain, I am still mission ready to do my job for my country. (OBTW - Nice American Flag pin on your coat sir!). It was for that reason (and diabetes in family) that I mainly sought surgery. I recommended my colleague to Dr. Pilcher and Sage Bariatric for the same reason - too big for our job! Three years later, we are now going to be working together. Luckily we are both healthy and ready to serve. Thank you LTC!

-already in practice for a few years
-emphasizes long term care, long term follow up
-has a team that includes a dietician on staff

I am almost 2wks post op from sleeve surgery. I only want to lose 100pds max. No pre-existing conditions/meds. After surgery, I haven't had any cravings for sweets or soda which were both my biggest vices. I have already slowly started back to regular foods bcuz I despise shakes. So far no issues other than nasausea (which im ok with as long as I can enjoy a little food sometimes). I'm hoping that I do well long term with this procedure and see results.
I'm also addicted to sugar. I'm confused as to which one I should get. Has your sugar cravings been under control since the surgery??

Doctor, which surgery would you recommend with GIM(gastric intestinal metaplasia)? Originally, I wanted gastric bypass but what scares me is I wouldn't be able to do yearly check-ups for the whole stomach pouch. I would defer to the advice of your surgeon, though I do see advantages to a sleeve which will remove part of the troubled stomach, and will not interfere with future evaluation.
