Sir I would like to ask a question. A pt had CTO in Lad , we planned for PCI to Lad, tried with hard wire. We put another wire in lcx for support. We were struggling with CTO but suddenly notice perforation in lcx . What is the reason behind that?

Hard to tell without looking at images, but could possibly be due to distal vessel perforation due to excessive guidewire movement in the circumflex while trying to wire the LAD.

Had a doubt still regarding that a patient with Adhoc PCI stenting done in RCA after it place the wire in the LAD and major OM there is 70 % stenosis in the OM after stenting done in LAD wire removed then perform LCX pci good placement of stent is done before inflation after the stent inflation the LCX got occluded many way try to open that closed vessel finally the vessel is open no other post dilatation and other not done patient is stable after the occlusion check angio shows TIMi 2 flow. Then shift the patient into CCU after 1 hr patient experience hypotension chest pain immediately shift the pt into cathlab but check angio shows stented segment perforation what was the reason for? Still regarding that question mark in all ? Any stent problem we are using SLTL stent . Pericardiocentesis is done . Emergency CABG done after one patient death but the bystander and media cover that news .......its a fear experience from my cathlab....
