    A lady wants to improve the appearance of puffiness under eyes, and loose lower eyelid skin. She tried filler, which left her under eyes looking lumpy. She’s considering peels and laser treatments, but wants to know if surgery is the best option.

    I just emailed your office. I live in NYC and had a consult with a doctor who basically said I am not a candidate for surgery because I have little to no fat on my face. SO he suggested either check implants (I do not want that) or maybe filler, which is okay I guess. After watching this video I feel like you might be the better person to give me more options. I would be happy to email your office some pics and maybe you can see what is needed. TYIA and I am excited to speak with you soon.

    I really don't think filler is too good for this undereye application -- it worked great for me, until it didn't. On about my eighth round of filler (maybe 4 years), it eft me with a festoon that never went away ... even after a lower bleph. I'd have been better off getting a lower bleph that I needed in the first place. Also not happy my oculoplastic surgeon didn't bother to remove the filler BEFORE my lower bleph. He had no idea how much skin to remove, since there was so much filler left in. The fact that there is no basic standard of care for this is absolutely maddening from a patient point of view. It is not a matter of "aesthetic judgment" to fail to start from a clean slate, just because the surgeon finds it more expedient to delay filler removal until surgery. The real crime is that HE PUT IT THERE; not like he didn't realize I had it. Ugh.

      Sorry to hear about your problems. The results of filler treatment are mostly dependent on the placement technique of the doctor, and in relation to that their technique in dissolving filler when needed.
      Also, I generally don't excise lower eyelid skin with blepharoplasty, as shown in the video, but rather improve the quality of lower eyelid skin to improve the appearance of wrinkles, and dark circles. Avoiding lower eyelid skin removal avoids problems like skin shortages, eyelid retraction, and changing the shape of the eyes. I hope this clarifies things for you.

    Dr. Prasad, in your experience, what's the recovery time when doing a transconjunctival blepharoplasty (from inside the eyelid) compared to the 'standard' blepharoplasty (transcutaneous or cutting around the eyelid)?
    Also, what's the cost in average?

      Hi geek2330,
      Recovery time for transcutaneous blepharoplasty is longer, although you can still return to work within a week (depending on technique and anesthesia used). Transcutaneous blepharoplasty involves more bruising, trauma, bleeding, and the eyelid skin incision, when compared to the transconjunctival method, so there are more aspects to heal from.       We use platelet-rich plasma (PRP) which is derived from your own blood, which can be injected into lower eyelid skin to improve thickness. It's often used as part of the treatment of under eye dark circles, which can be seen in these videos:

      Technically, plastic surgery is restoring appearance due to an accident or injury, which cosmetic surgery is about improving appearance. Terminology doesn't matter so much when it comes to eyelid surgery, because what matters most are the experience and skill of the doctor performing highly technical and precise eyelid surgery.       I only recommend skin tightening done by experienced professionals, as using thermal devices without training can cause damage to the skin, which will cost more to rehabilitate than having your skin treated by a professional. I hope this helps you.
