    I’m so glad you mentioned LOCAL anesthesia. I was reccommended surgery due to the fat protrusion. The eye bags are genetic and my ophthalmologist said that I’m not a candidate for fillers cause I already have a VP shunt . Thank you for making me feel comfort in a blepharoplasty

      In addition, and as detailed in the video, I suggest you ask for transconjunctival blepharoplasty by name, which involves no external incision in the lower eyelid for visible scarring and potential damage to lower eyelid support structures; and insist on no lower eyelid skin removal to treat wrinkles, as this can cause skin shortage that will pull down the lower eyelids. Under eye wrinkles can be treated non-surgically to improve skin quality.

      I don't recommend at home thermal devices because people using them are not trained to limit heat, exposure, nor do they know about the healing time needed between sessions.

    I am amazed at your knowledge and impressed in how you present it. Unfortunately many of us are not in New York where you are. Is it possible that you give referrals to surgeons that are as skilled as you are with options available? I have a fear of facial surgery, but your video actually made me feel calm and positive.

      Lower eyelid surgery can be minimally invasive, so you'd be surprised at how easy it is on the patient. Being minimally invasive with transconjunctival blepharoplasty is more of a technical challenge to the surgeon, and becomes routine with experience, but most eyelid surgeons aren't trained in the technique.

    Dr. Prasad: If the main cause is fat herniation (prolapsed fat), why is fat removed rather than the surgeon "pushing" it back into place? I would view this conceptually as slightly different from traditional fat repositioning, but perhaps what I am proposing is simply not possible/advisable? Thank you.

      Orbital fat prolapsing also occurs because of tissue changes that once held the fat, so it isn't practical to push it back into place as weakened tissue can also cause the fat to prolapse again. I hope this clarifies things for you.

      Sorry, but radiofrequency or any other thermal device will not have that effect on the orbicularis oculi muscle. Prolapsed fat cannot be pushed back into place.       Transconjunctival blepharoplasty can be considered less invasive as the transcutaneous approach, as there are no incisions through the muscle and skin, which is not required, but it does avoid common complications like lower eyelid retraction, which is a considerable additional cost with revision surgery (revision surgery usually costs more than the primary surgery as it's more extensive).

    Hi Dr. Prasad, I am in my early 50s... when I was in my early 30s, I had a moderate tear trough deformity, which I was able to have corrected with filler. I now have mild to moderate eye fat herniation and can have it corrected if I get filler in my cheeks, unfortunately, it takes 4 - 5 syringes of filler to give me adequate volume to correct the herniations and tear trough. My question is, if I have surgery to remove the herniated fat, can some be repositioned to also take care of the tear trough issue? Also, I am assuming even with a facelift, I would have to continue to obtain filler for my cheeks or a cheek implant - the issue now is cost due to the amount of filler I now need for correction. I hope this makes sense and thank you so much - your work is beautiful!

I have cheek fillers from about two years ago but they are definitely there. Should I get them dissolved before my lower eyelid surgery?       Your surgeon should be able to locate your existing filler, but you should let them know about it prior to the procedure so they can be planned around, without having to dissolve the filler. Prolapsed orbital fat that causes eye bags is different from cheek fat, and the effects of your filler, so your surgeon should be able to plan around the existing filler volume.       I'm not familiar with doctors in India. If you are looking to address under eye bags with surgery, I suggest you ask doctors for transconjunctival blepharoplasty by name. This procedure is less prone to complications since there is no external incision, or skin removal, and so results look completely natural, but fewer surgeons practice this technique. I had a thick hyaluronic acid filler placed in trough lines (after crying a lot over the loss of a loved one, a women offered to freshen me up in my facial) It was the worst mistake of my life and now have terrible bags and swelling. I look 10 years older, and my face looks sadder than ever 😭. I've had two failed attempts injecting the area aswell to remove it. If you could offer any advice in the meantime, then it would be greatly appreciated as this has quite ruined my life at present.       The results of lower eyelid surgery can last many years, and if fat were to prolapse again, it would be new orbital fat, not the same fat that was treated.       Sorry to hear about your problem. Kidney issues can cause temporary swelling that can appear like eye bags, but they are not always puffy. Eye bags that are always visible are likely to be caused by lower eyelid fat prolapse, as explained in the video. I only treat eye bags that are caused by prolapsed orbital fat, as I can sculpt and reduce the fat with surgery, or camouflage it in milder cases with filler. The cause of your eye bags needs to be diagnosed before I can recommend a specific treatment for you. I hope this helps you.

      I don't use Plexr as I have more precise and versatile lasers in my practice. PRP, Plexr, or laser treatment are not appropriate treatments for puffy eye bags, but they can help improve the quality of lower eyelid skin to treat issues like wrinkles, and dark under eye circles. Eye bags that are visible all the time are caused by prolapsed orbital fat, and ideally surgery is needed to sculpt and remove them, while some slight to moderate eye bags can be temporarily camouflaged using filler.     Im glad I found you! I want a more permanent solution. I think I'm more hollow. As a person of color and nervous. I rather have surgery but cant find after pics of people of color. Dr suggest radio frequency 😒. Maybe filler would be best??

      It's not clear if you have hollowness, or discoloration since your doctor is recommending radiofrequency. While filler may or may not be the right treatment for you, permanent fillers are very prone to permanent complications due to migration and foreign body reactions, while temporary hyaluronic acid fillers are much safer because hyaluronic acid is already naturally present in the body, and are safely metabolized with time.     What happens to the skin after the fat pads are taking out? I do see some people who had the procedure done and then they end up having more wrinkles where the bags once were. Are there any treatments for that? Otherwise it would be like choosing between the lesser of 2 evils lol

      Lower eyelid surgery is an art where some fat should be left to prevent a hollow appearance, and also prevent the appearance of wrinkles. You can watch the video to see my patients don't have increased wrinkles after their eye bags are gone. For patients who have eye bags, and wrinkles due to diminished skin quality, I perform additional collagen stimulating and skin resurfacing procedures to minimize the appearance of wrinkles, such as laser treatment, and PRP.       Medicare isn't applicable to elective cosmetic procedures like lower eyelid surgery, and our office generally doesn't accept insurance even for procedures that are health or vision related, such as eyelid ptosis correction surgery. I hope this clarifies things for you.

      Tear trough implants require surgical placement, so they are more involved than injectable tear trough fillers. While silicone tear trough implants can be a long-term treatment for under eye hollowness, many people don't find them comfortable, and softer hyaluronic acid fillers as a more comfortable. Tear trough implants are not permanent, as facial dimensions do get smaller due to aging, so eventually they won't fit, and will need replacement.       Thanks for watching! I recommend you see an allergist for treatment as I only treat eye bags that are due to prolapsed orbital fat. I would have to examine if you have eye bags also due to lower eyelid fat prolapse. I only treat the fat that causes eye bags with transconjunctival blepharoplasty, which is featured in the video, and is done without any external incision on the eyelid skin, and no skin removal so results look natural, and complications due to skin removal, or complications due to incisions on the eyelid skin are completely avoided. Surgery can be done in people with dry eyes, as long as you have a doctor experienced in performing surgery in people with dry eyes.       For under eye bags, I suggest you ask for transconjunctival blepharoplasty by name, which involves no external incision in the lower eyelid for visible scarring and potential damage to lower eyelid support structures; and insist on no lower eyelid skin removal to treat wrinkles, as this can cause skin shortage that will pull down the lower eyelids. Under eye wrinkles can be treated non-surgically to improve skin quality. I hope this helps you.

    what about sunken hollow eyes with lots of under eyefolds/lines/wrinkles < that I got from scratching my eyes / under eye peeling / flakeyness (allergies or dermatitis, not sure)

    I started having fillers under my eyes 4yrs ago and i had my fillers every year then now i had that tyndall effect. My under eye bags worsen because of that bluish color. Is the fillers can be dissolved although it’s already 4yrs ago when i had my first filler? Thank u so much

      If you had a hyaluronic acid filler, it is possible that the filler became encapsulated, preventing normal metabolization, so it remained. In these cases, the filler can still be dissolved if the injection reaches the encapsulated filler. I hope this clarifies things for you.

      Smoking can cause malar bags, which are puffy bags on the upper cheek caused by trapped fluid in tiny chambers. Malar bags are more of a challenge to treat than eye bags that are caused by prolapsed orbital fat (that are shown in this video).     @AmiyaPrasadMD From all my reading, it seems that Hashimoto’s or certain hypothyroids could attribute to these under eye bags. I also read that some doctors refuse to do this procedure on patients with hypothyroid. Has this been an issue in your practice? Is there both fat and protein fluids filling the under eye space? I want this but confused if I should expect bags to come back since I take thyroid meds.
