Esketamine is actually an enantiomer of Ketamine (a mirror image molecule). They use esketamine in the nasal spray instead of ketamine because Esketamine is novel enough to patent it. Ketamine has been in wide use for decades and the claim is that Esketamine is the same because it is just an enantimoer but chemistry doesnt work like that. It’s obviously different enough to newly patent but realistically too they really wouldnt be considered the same chemically therefore there is no way to base treatment with Esketamine on all the experience we have because it is with K not Esketamine. Bottom line, like anything else, it is a money grab.
