Reversing T2D with a keto diet - Diet Doctor Podcast with Dr. Sarah Hallberg

  • For decades, the medical world has seen type 2 diabetes as a chronic condition that we can only hope to manage with medications to delay inevitable complications. Dr. Hallberg and her colleagues at Virta Health completely changed that paradigm by showing us that we can reverse type 2 diabetes and we can allow patients to safely stop most if not all of their medications. How have they done this? With a ketogenic diet combined with high touch and high tech. Could this approach work for the millions of people suffering with type 2 diabetes? Dr. Hallberg certainly thought so, and in this interview explained why.

Dr Sarah Halserg saved my life literally. Her Ted talk that I heard in 2017 were she presented her study results gave me hope as I had just been diagnosed with breast cancer. As I had been type 2 diabetic for 10 years and was on full dose of meds and insulin I was in a very dark place. Her advice of no grains, potatoes or sugar in your diet helped me totally reverse diabetes while undergoing chemo and radiation. I believe this way of eating also helped me survive chemo with very little side effects. Now I am still off all meds and insulin with an hb1ac of - 6.0. I believe nutrition is also critical when being treated for cancer. Would love to see Sarah take her research into this area as well. Thank you for taking your message to the world and reaching me here in Australia. I owe you so much.

I changed to a Keto lifestyle 7 months ago, uncontrolled diabetic. Not that I wasn’t trying, frustrated on nothing working. I’ve lost 55 lbs so far and rarely have to take any insulin. Goal is to be off insulin completely in the next few months. Changing my way of eating has been the best thing for me.

My husband saw his cardiologist 3 weeks ago.In five years cardiologists have said my husband has a "high normal" bp. Well at the visit 3 weeks ago his bp was 128/76 and the Dr. looked at my husband and said "What are you doing"? Husband says "Keto and IF" Dr. says "That is why your bp is way down. The research is there for IF and is almost there on Keto and I am on board. Keep it up" We tell my husbands mother who is 83 because her cardiologist has always told her she runs a "high normal" so all she started to do is eat lower carb and eat all her days calories in a 6 hour period. Well her bp only after 6 days (she takes bp everyday) was reading 123/68. Needless to say she is totally sold. That is evidence

In 2105 I was diagnosed with T2D after the passing of my grandmother. She had type 2 as well, she had lost her eyesight, had multiple TIAs (mini strokes) dementia & eventually had a stroke that ultimately led to her death. I didn't want to go down that same path. My Dr put me on metformin & I immediately started keto, walking 5 miles 5 days a week plus working out twice a day. In about 7 months I lost 65 lbs & brought my A1C down from 8.4 to 5.2. Then I tore my calf muscle during a workout. I was seeing success, but I was overdoing it. Over the next few years, I fell back into old habits re-gained about 30lbs & continued to feel like crap. I had seen Dr Hallberg's TED talk a year or two ago & then recently came across Dr Jason Fung & Dr Pradip Jamnadas. So 2 weeks ago I ordered a good glucose monitor & started testing (average of 10x daily) & started Intermittent Fasting (18:6), cut out sugar & started reducing carbs. After about 4 days of 18:6 I moved to 20:4, then to OMAD. In less than 14 days I've gone from 229 to 215 & brought my average daily glucose down from 200 to 113 & the estimated A1C provided by the app with my glucose monitor shows that I was at 8% when I started & 7% now. I'm seeing better results than I did with the "eat less, move more" method that I had used previously because I was still eating too often, even though it was keto & reduced calories. I wasn't giving my body a chance to reduce INSULIN! I now, for the first time in my life actually have HOPE that I can avoid the same fate that befell my grandmother. I am still doing some exercise (I work from home, so I have a seated elliptical that I use while I work, pedaling 20-50 miles, 5 days a week) but I'm not killing myself. My sleep patterns have improved as well, so I can't wait to see how I feel as I get closer to my goals!

Medical doctors are getting it. My family practitioner is a cardiologist. I saw him when I was considerably overweight and pre-diabetic. He said he's seen 300 diabetic patients. He gives everyone Ketogenic diet. I went on the diet and still on it. In short, blood glucose is stable. I've achieved two weight goals. Lost 8 inches off my waistline. My blood work is great. I have to say, the doctors my husband and I have seen in South Australia, are not on board with this way of treating any diet / diabetes / weight issues. We keep getting the rants about cholesterol, and portion sizes, and calorie restrictions. Only one old doctor I've seen here locally says "yes, I've heard there has been some success with this", and hes on the verge of retirement. THERE ARE NO DOCTORS here I've found who support this life style change, I have looked and begged for support from them. We just have to go it alone, and these videos are a huge help. ty x

@@Yellowjacket45Wall I agree with your statement. She is no different than the dietary guidelines that she opposes. She advises approaching fasting with caution, something she does not support. Sounds very much like the ADA's modified dietary guideline to suggest that LC can be an option although they do not agree with LC. My A1C was 6.8 but after 2 months of Keto I lost 30 lbs of weight and after 3 months of Keto my A1C had dropped to 4.9. I attribute my success to the use of intermittent daily fasting in which I only ate one low-carb meal per day. I've been able to drop my use of antacids, BP meds, as well as gout medications.

She needs to look at the research in intermittent fasting too. I reversed my T2D with a whole-foods ketogenic diet but it definitely was the intermittent fasting that reduced my inflammation insulin resistance and weight that helped reduce my A1C from an 11.5 to a 5.4. Although, the MOST powerful healing of fasting was the massive reduction of my Albumin-to-creatinine ratio. Almost, in normal range as it has dropped with every urinalysis, further supporting the benefits of fasting as it healed my kidneys too!

I understand why they want more evidence for fasting, but I do not lose weight or lower insulin only on a ketogenic diet. I'm one of those people who is obese, but not diabetic. I am hyperinsulinemic -- 16.6 fasting insulin after 2 years of strict keto. And ONLY fasting at least 36 hours seems to be effective at lowering that insulin. I can even gain weight on less than 20 carbs and time-restricted eating. But, I still appreciate everything Dr. Hallberg is doing for Type 2 diabetics.

I have a lot of respect for Dr Hallberg. However, I was surprised when she made a point of saying that she absolutely uses statins and believes they are necessary for some patients. I would love to know if she has truly researched the studies that led to this widespread belief. I have seen too many docs that show where the data is NOT clear for their use, particularly for women. Also, that would assume that elevated LDL is concerning and that is now being disproved.

A fasting period of 48 hours is completely safe and I have done is many times with over 50 pounds lost so far. I am in ketosis with blood ketones over 2 but my weight loss has stalled. As a patient you have the right to take drugs or not. Doctors only provide advice and deal drugs, but cannot force patients to take them. They tell us to take them as standard of care. They cannot be sued under any circumstance as long as our conditions meet the criterias to have the drugs prescribed regardless of the outcome. If he does agree to stop certain drugs and something bad happens then that can be malpractice. In other words they have their best interest before their patients

According to Tom Brimeyer from Forefront Health, a lo-carb diet will make your thyroid worse. Tom has studied the thyroid for over 20 years. Tom says if your brain doesn't get the sugar and carbs it needs, your body will save your life by using the adrenals hormone, and in turn using cortisol, a stress hormone, which makes fat, especially belly fat. We need a complete, balanced diet with 50% carbs, 25% protein, and 25% fats. Diabetes has nothing to do with overeating carbs. My Dad lived to be 93 and ate candy, cookies, and loved ice cream daily. He was never diabetic.
If you have Hashimoto's, or are hypothyroid, please don't go lo-carb. It will reverse diabetes in the short term. (I got my fasting BG down to 98. Now my glucose is in the high 200's.) But long term, your diabetes will get worse. I've been following Tom's advice for 3 months and am starting to see results.

When you use a starvation study to try and disprove fasting, your credibility takes a huge hit Sarah. The cahill study was a low calorie study that deliberately under fed subjects throughout the day which is fundamentally different from fasting which is not necessarily low calorie, but restricts the number of meals throughout a given period without necessarily restricting calories per meal.
With that said, Fasting to a certain degree does create lean body mass loss among those without adequate fat stores (leaner people like the ones in the Cahill study). But for those people who have a tremendous amount of energy already stored (people with a lot of fat), Fasting as Jason Fung does with many of his patients works because it isn't a low calorie feeding program since the bulk of the calories consumed is coming from the persons own body fat. It's such an easy pathway to understand but when you refuse to understand, you start fights instead just like what Steve Phinney and his team have chosen to do. NO reason to argue but they chose to do so which to me is just a fantastic sign of deep insecurity.

    • @wmartonejr
      @wmartonejr Před 5 lety +1

Yeah, I get your point, Phinney and even Westman are not strong supporters of IF and seem to be influencing Virta Health, Heal Care and Dr Hallberg. Keep in mind that Virta Health & Heal Care are care based clinics trying to treat the T2D (who don't know or care about all this stuff) with a revised standard of care. So just getting a carb addicted T2D to change to low carb high fat diet is the first step to their own long term progress which will eventually get them to achieve what the rest of us have already regained - metabolic health! Slow & steady wins the race.

I took some time to review the 2 year results and these are what stood out to me. Could the very slight worsening of a few things, from 1 year to 2 year, be compliance problems? The main thing I checked right away is the c-peptide, as that is giving us solid information on hyperinsulinemia, and that looks to be the downstream juncture after mitochondrial dysfunction.

Stephan Guyenet also suggests that diabetes is the only chronic metabolic disease for which there is a proven cure and prevention.. but his #3 is not low-carb :
    1) Low-calorie diet (Lancet 10.1016/S0140-6736(17)33102-1)
    2) Bariatric surgery (NCBI PMC4566335)
    3) Early insulin administration (NCBI PMC2811460)
So wouldn't it be necessary for it to be said that a low-carb or keto diet *reverses* or cures type-2 diabetes, the patients would have to go back to a more typical diet (50g carbohydrate/meal) and/or do a Glucose Tolerance Test to see if they no longer have high levels of blood sugar? Otherwise the Virta study only suggests that maintaining a low-carb/keto diet, is effective in *managing* diabetes?

    • @Yellowjacket45Wall

Shallow thinking just to go back to carbs which caused it to begin with just to see if it can cause it again, if you burn your hand on fire why would you want to try it again, to see if you dont get burnt each time

Everyone body is different, some eat a ton of carb and have no diabetes, some do need zero carb to reverse diabetes. A typical diet give many people diabetes, yeah, one can manage diabetes with insulin and high carb diet. We are talking about different options and tools here.

Just as drinking or driving can be done separately, but not together - the same thing could be applied to being predisposed to insulin resistance or consuming a carb/sugar rich diet, they could be done separately, but not together. I wouldn't attempt to solve the problem of drinking and driving by having people just "manage" their alcohol consumption while driving or teaching them to drive drunk - it would be best practice to eliminate the practice all together. Perhaps the best practice for those who suffer from metabolic issues, is to just eliminate the carbohydrate content from the diet all together.

The FREQUENCY of eating contributes a huge amount. We started eating processed carbs AND eating 7 times a day at the same time. Not to mention thr amount of added sugars...
We had never eaten ALL day in history before. The insulin resistance becomes more or less inevitable
