That scene with the boy has always stuck with me. It may not be the goriest scene out there, but just seeing that poor kid get framed for the murder of the girl he loved, and then get slowly tortured to death while the entire town just stands there and watches is honestly so sad :’(

He didn't really explain it well but the deity feeds on worship not blood sacrifice. All that blood sacrifice was corrupting and poisoning the deity. That's was why the deity couldn't provide harvests anymore and all babys and baby animals became stillborn.
The deity became addicted to blood after being captured and unvillingly fed blood by the cult. The deity couldn't heal itself after being fed blood for so long so it sought a succesor for it's position and then to die so a new uncorrupted deity could protect the island. It's implied that the cult leader learned his lesson and will worship the new deity guy properly.
I think it’s explained at some point in the movie that the leader and right-hand man mistreated the Goddess and misinterpreted the worshipping process. Thomas may still have to be a cannibal, but without right hand’s influence I think the leader may serve as a better follower of the new “God”. So he may be helpless, but likely not abused.
You know what, I think I may be conflating the movie with the Ending Explained. I think the conclusion I point out above is supposed to be inferred based on the images we see drawn in the cave.
