I had this happen to me while scorekeeping a youth game... it was going off at 1 minute intervals and I had no idea how to fix it
I came in wondering how a recording kept going off unintentionally. Then I realized it was the period horn lol.
That's a Daktronics all-sport 5000 controller. My guess is that the external clock controller is having issues, because they're pretty finicky. I had this happen during a basketball game I was score keeping for. The clock wouldn't stop.

Hey I was listening to the game last night I never heard the horn keep going off. But I’m proud of the lightning for winning in overtime.
Weird how the 1st three times it went off it was after a whistle. That horn is only for the end of the period. Maybe there was a faulty wire with the time-stopper? What else do those guys touch after every whistle?
The problem is they didn’t do anything about it until it affected the play, when it was clearly an issue prior but they didn’t seem to care. When you clearly have a technical issue but don’t fix it because it hasn’t caused any issues, you’re just asking for it to cause issues

so question then : i know sometimes if something within the arena causes an issue like this the home team can get a penalty (i grant usually its because something was thrown on the ice). but in a situation like this, does that have the potential to draw a penalty?
I don't think so because that horn is either automatic when clock reaches 0:00 as specified in the NHL rule book or operated by NHL staff (obviously the NHL is unbiased, in theory lol). And that horn is not operated by home team staff or home arena staff, so I can't imagine the NHL staff getting a penalty
I can't think of an example of a penalty like that besides fans throwing stuff on the ice. Can you give me an example of in arena malfunctions resulting in a penalty?
I can't think of any specific instances offhand, though I think glass might have done something? I wasn't aware that the horn is operated by a neutral party
I remember one time years ago someone picked up a phone in Target and yelled "AFLAC!". The entire store heard it. I was cracking up. I guess it's not quite as amusing to sound a horn during a game because it stops play, but this reminds me of that.

That is the same scoreboard machine that I use at the Indianola High School in Indianola Iowa for basketball games and it never does that. Maybe just unplug the remote part of it.
