Question why TP LP as the antegrade MC and not the TP Spiral that may have crossed.

Great point - we used Turnpike LP because we were expected that we would have to go retrograde, but you are right that Turnpike Spiral might have helped.

Anchor balloon could also give strong support.
Reason for previous failure? Wire not crossed or balloon not crossed ?
